L’insuffisance cardiaque est un syndrome clinique caractérisé par une intolérance physique à l’effort par diminution du débit cardiaque et la présence de signes de rétention hydrosodée. Les deux grands mécanismes responsables, dysfonction systolique par altération de la contraction et dysfonction diastolique, entraînent une diminution du débit cardiaque. On distingue donc deux types de mécanismes physiopathologiques en fonction de l’altération ou non de la fraction d’éjection du VG qui peuvent schématiquement se distinguer en deux entités :
• Insuffisance cardiaque gauche à fonction systolique altérée (ICFEd).
• Insuffisance cardiaque gauche à fonction systolique préservée (ICEFp).

Abstract
Background: The prognostic impact of heart failure relative to that of ‘high-profile’ disease states such as cancer, within the
whole population, is unknown. Methods: All patients with a first admission to any Scottish hospital in 1991 for heart failure,
myocardial infarction or the four most common types of cancer specific to men and women were identified. Five-year survival
rates and associated loss of expected life-years were then compared. Results: In 1991, 16 224 men had an initial hospitalisation
Ž . Ž .for heart failure ns3241 , myocardial infarction ns6932 or cancer of the lung, large bowel, prostate or bladder
Ž . Ž . Ž .ns6051 . Similarly, 14 842 women were admitted for heart failure ns3606 , myocardial infarction ns4916 , or cancer of
Ž .the breast, lung, large bowel or ovary ns6320 . With the exception of lung cancer, heart failure was associated with the
Ž .poorest 5-year survival rate approximately 25% for both sexes . On an adjusted basis, heart failure was associated with worse
Ž .long-term survival than bowel cancer in men adjusted odds ratio, 0.89; 95% CI, 0.82᎐0.97; P-0.01 and breast cancer in
Ž .women odds ratio, 0.59; 95% CI, 0.55᎐0.64; P-0.001 . The overall population rate of expected life-years lost due to heart
failure in men was 6.7 yearsr1000 and for women 5.1 yearsr1000. Conclusion: With the notable exception of lung cancer,
heart failure is as ‘malignant’ as many common types of cancer and is associated with a comparable number of expected

1. Introduction
Heart failure has emerged as a growing health
problem likely to reach ‘epidemic’ proportions in de-
w xveloped countries 1᎐3 . This has been attributed to
the combination of an ageing population and more
effective treatment of its major precursor, myocardial
w xinfarction 3 . Not unexpectedly, heart failure-related
hospitalisation rates have been steadily increasing in

w xthe past decade 4 . Whilst in-hospital mortality rates
w xappear to be declining 4 , the quality of life and
prognostic outlook for those heart failure patients
w xwho survive an acute hospitalisation remains poor 5 .
Unfortunately, heart failure continues to be the
‘Cinderella’ of cardiovascular disease states and is
rarely identified as an important health issue in the
public domain. This probably reflects, in part, an
under-estimation of its prognostic significance and the
perception that it is almost exclusively confined to
older individuals who would derive little or no benefit
from more effective treatment.
There are some data to suggest, however, that heart
