NASHP is seeking applications for Consortium membership from states with emerging Medical Home Initiatives. This year-long program will offer assistance to states seeking to strengthen, expand, and sustain their Medicaid and CHIP-focused medical home initiatives. Interested states can submit an application by February 2.

communities. Learning community members will provide
input on topics of support.

5. Request for Applications
NASHP has released a request for applications that will allow
additional states to join the 3rd Consortium.
Applications are due by 5:00 p.m., February 2, 2011
Preference will be given to states that:
Have committed resources to their medical home effort.
Target a comprehensive population.
Are currently implementing medical home initiatives.
NASHP is seeking diversity in the selected states.
Geographic and urban/rural States
Delivery/payment system diversity
In total, we expect to support up to 17 states in the
Consortium through invitation and application.

6. Conditions of Participation
Consortium States will:
Form a core team of 2-4 members
One member must be a Medicaid official with
decision-making authority
Complete a survey prior to the Baltimore kick-off meeting
Attend the Baltimore kick-off (March 30 – April 1, 2011)
Develop a work plan
Achieve goals from the work plan with the technical
assistance provided by the consortium
Join one of three learning communities
Participate in all group and individual technical assistance
programs, as planned for selected learning community
Complete a survey update at the end of 12 months
Complete a participation agreement that summarizes
these expectations

Consortium states will join one of three learning communities:
Multi-Payer
Learn how to bring new payers to the table
Expand the population of your programs
Homes-to-Neighborhoods
Develop and strengthen community networks and
teams to support medical homes
Public health integration
Field Trip to Vermont
Innovative Models of Care
Explore new payment and service delivery models
Funding opportunities within the Center for Medicare
and Medicaid Innovation (CMMI)
Field Trip to Baltimore

Assure sustainability of current programs
