in (offshore and onshore) for construction and fabrication, Provision of hook-up,
commissioning, Topsides major maintenance (TMM), Facilities improvement project

corrosion and base maintenance (TCBM), Project repairing, maintenance and
replacement of piping/structural/vessel and tank. Those activities on this job is taught
me the important of interpersonal skills. As an active person in various activities, I have
the opportunities to lead, initiate and manage as well as developing the ability to
work with people with different level and social backgrounds.

Offshore Shift Supervisor
Scope of work:
• To be acting as a construction supervisor OR superintendent in event of
meeting/courses and non-available at workplace.
• To lead/monitor/supervise for whole the tasks carried out is accordingly with
company/client and project owner requirement.
 To promote Safety program and push for zero accident at work site.
 To review of work procedures, testing procedure, installation procedure and JHA.
 To Execute planed weekly piping work target to meet progress schedule.
 To manage shop welding production.

• As a chairman in HSSE committee meeting/hygiene inspection. To lead the HSSE

Scope of work:
• To liaise with construction shift supervisor/construction supervisor @
Superintendent, field engineer and key personnel to plan the
works/preparation according to work pack/work scope by client.
• To inspect and witness Hydro-testing, leak test, flushing and air drying
activities.
• To review material test certificate and all pressure testing calibration
certificate.
• To supervised and witness Low Pressure Tightness Test of piping system.
• To conduct weekly audit on PTW system, Pre Task Talk, LOTO, QAQC and
Housekeeping.
• To coordination with HSE and QA team for quality inspection and HSSE
measures to ensure safe and quality work place.
• To coordinate site technical queries between Engineering and Construction
team.
• To arrange scaffoldings, material and man power for hook up activity.
• To arrange facilities for back-up sub-contractor working at site.
• To lead the piping team in the installation, erection and testing
of piping works.
• To killing punch-list for ready hydro testing.
• To re-instatement of piping after hydro testing.
• To review drawings such as Isometrics, P & ID, Pipe supports prior
implementation at site.
• To liaise with sub-contractor on work schedule, manpower availability and
work activity.

matters pertaining to piping works.

• To liaise with on board duty CSR/Deputy CSR and other’s personnel in-charge
where’s assigning by client to ongoing project in certain position or categories
• To lead the construction crew to execute the jobs as in work
pack/tasks/details of the project.
• To ensure that the work is carried out according to company/client/activity
owner standard & quality as a policy.
• To support the marine crew in AHT (anchor handling & deploy soft moor
attached to platform or jacket legs as anchor pattern required.
• As a focal point in lifting/hoisting/rigging & scaffolding activity at workplace.
• To involve in pre – shutdown & pre – start-up works on offshore.
• To plan 24hrs tasks ahead, Prepare the PTW where’s relating to jobs was carry
on at platform and jacket.
• Pre-check, visual inspection on all tools & equipment before starting the work.
• To brief the crew about hazards created before, during and after the work
has been done during tool box meeting & pre-jobs discussion at site.
• To monitor, surveillance and supervise the crew closely at worksite.
• To guide the new comer to commence their duty.
• To communicate with PLS or SSB personnel, crew before working at any
location on platform/jacket although there is green or brown field.
• To direct and supervise/coordinate and control all aspect the crew in
erection, modify & dismantling activity onsite.
• To ensure that only trained & competence or qualify scaffolder is allowed in
scaffolding tasks and must have a adequate experience.
• To visually check fitness/quality on scaffolding materials/equipment and fitting
to be used.
• To ensure that scaffolding built up on strongly/firmly ground or base.
• To familiar with BS 1139 accordingly (Malaysian standard).
• To ensure that scaffolder involved directly in scaffolding activity especial
working at height(WAH) are known their rescue procedures during an
emergency case and escape route/designated muster station/alternative.
• To identify/recognise and mitigate the hazards or apply stop work policy if
unsafe work condition.
• All scaffolding structure/platform must be inspected by “scaffolding inspector
“prior to occupy on.
• Ensure that scaffolding tag are install or displayed visible on scaffolding on
green tag mean is safe for use or red tag is structure/platform not safe for use.
• To lead/person in charge the crew in general rigging, hoisting, lifting or
shifting, manual handling and transferring to location.
• To understand with the client quality & standard, HSSE house rules &
procedures included permit to work system (PTW) requirement.
• To ensure that the worksite is surely free from any obstruction & hazards.
• To ensure the crew is wearing appropriate PPE, adequate and suitable on
their tasks with a safe manner.
• To attend the SSP (site safety procedure) & SIMOP simulate operation
cascading meeting held on platform living quarters.
• To leader the HSSE campaign, Poster, Scheme, Program, Slogan, Incentive,
Awareness, and Lesson learn and training at worksite or training Centre ware’s
provided/Approved by client.

• To encourage the crews to attend any HSSE & Trade courses and seminar to
improve their capability and Skills in carrying their duty safely/accordingly.
• All personnel have the duty to “STOP WORK “if the works/jobs & tasks is likely
deemed unsafe condition.
My Working experience base on the following:
• Carrying out Hydro testing works for piping if it’s conformed by RT technician or
welding inspector on site.
• Re-instatement test either using water or N2, it’s accordingly as procedure’s said in
project work pack, for the entire piping after tie –In (TP).
• Purging using N2 for cutting piping line to in forced any kind of gases trap inside the
line before cutting, spading, blind off, spectacle blind reverse, breaking flange ,drop
spools and where’s works or tasks relating to mechanical & piping.
• Purging @ injecting the N2 into vessel, tank and piping line to cease out any
remaining gases or hydrocarbon before any breaking flanges/vessel
opening/entering and spading activity as the SSB procedure.
• Leak test/function test or running test either using the hydraulic or compress air on any
type @ kind of equipment on SHELL facility as per SSB procedures.
• Tank or Vessel installation/Removal, maintenance, services and cleaning an onshore
and offshore especially at SSB & Carigali platform/Jacket and facility.
• Referring to drawing e.g. PI & D, Iso metric drawing, GA, piping, structure, flows
,process and mechanical plan to get actual & right site measurement & location
where it is been installed and removed, is requested by the project field engineer.
• Involving in heavy lifted to hook up & installation the new topside, Vessel, Structure
and tank .etc. from materials barge to platform.
• Planning, Preparing, Setting up & supervising any lifting activity included managing
the work task that is assigned by the leader.
• As a team leader in Assisting client in mechanical & commissioning works prior to
platform, Jacket& plant charge-up and start-up.
• Supervising in lifting activities to connect or disconnect primary gangway from
workboat & work barge onto platform as per gangway sketch approved from PCS
(Production and engineering services & SSB/SSPC.
• As a Frontline &Leadership in executing the job tasks, controlling and minimizing the
work hazards.
• Install ting/Removing & Maintaining the SBM at LCOT Labuan, floating hose, CALM
BOUY & SPM at Middle East (Kuwait).
• Assisting SSB/client vendor to install, remove, maintain& services the turbine GTG,
Moto condensate/glycol/sum caisson pump, fan cooler, cooler plug, ball valve and
others at platform.
• Being a team leader in heavy lifted, Pipe line and marine tasks at cat 1 & cat 3
include an overseas project.
• Assisting by company SSB vendor to carry out Well Head services & maintenance.
• Working together with millwright to install/remove, Align & Adjust the mechanical &
piping material/equipment.
• Presenting the works planning during daily progress meeting (DPR) to the client.

General Responsibilities/Duties While Working As HSE Personnel is based on the
following below :
• Main duties is to ensure the company project HSE plan is properly implemented and
shall meet client’s HSE standard and expectation , As well to monitor the entire
operation shall not contravene to local laws and regulations. The responsibilities are
as followed:
• Implemented approved project HSE management system.
• Develop, Implemented and manage tools for recording of HSE performance
measures.
• Regularly representing HSE department during meeting with client and solve raise
safety issues.

• Conduct HSE audit/inspection and follow-up on remedial actions/re-corrections and
close-out.
• Conduct safety briefing and orientation for new personnel to certificate in the
project.
• Attended safety meeting as specified in project HSE plan.
• Accident/incident investigation and close-out.
• Prepare job safety Analysis [JHA] or risk assessment.
• Conduct Company’s in-house craft training such as rigging, Scaffolding, Welding,
Cutting, Confined space entry, Forklift safe operation and organize any specific
training required during the project period.
• Compiling weekly and monthly HSE reports.
• To monitor/implemented of PTW Systems at workplace.
• Initial and participate in TBM/Pre-jobs discussion conducted by supervisory personnel

Maintenance, Commissioning and hook-up at barge and platform [Offshore].

• To assists barge clerk in manage to develop the suitable hazards control interventions.
• To schedule and coordinates safety activities and program.
• To liaise with Construction Supervisor & Barge Superintendent on critical safety task
requirement and solution.
• To prepare daily, Weekly and Monthly report and statistic.
• To conducted safety tool box talk, Presentation as required during the project period.
• To review and compile daily hazard observations report, Safety walkabout and site
visit by management and on actions as soon as possible.
• To conduct accident, incident and near missed site investigation report/ Conduct
safety Inspection and audit.
• To review barge crew HSSEO performance periodically and develop improvement
action plans.
• To liaise with tower operator on critical weather condition, standby and abandon if
necessary.
• To review in-coming and out-going crew on barge.

• As an In charge, In muster station on head account and lift raft preparation if
emergency case like abandon the barge immediately and emergency boat transfer
& boat to boat.
• To setting up safety induction for new comer and video presentation.
• To lead in inspection of tools & Equipment/safety walk about on site.

• To organize the safety/food committee meeting and Welfare.
• To encourage the crew in safety competition e.g. poster, Slogan, Scheme and

RIGGING DUTY/ROLE & RESPONSIBILITY
• Any statutory requirement or regulation e.g. Latest edition of Department of
Occupation Safety and Health Factories and Machinery 1967 act [ACT 139]
requirement on inspection frequency.
• Understand the project Lifting procedure-MS and requirement during the period of
project, Established strategic approaches to implement project Lifting procedures –
MS by defined rules of team leader and influence them in safe practices.
• Every lifting shall have an assigned Person in- Charged or Banks man to control the
lifting operation.
• All lifting appliances used must have current colour code clearly visible.
• All personal involved in lifting should have up to date and formal training required for
their role, Complete PPE must be worn during lifting operation carried out.
• Ensure all lifting equipment used for transferring of personnel must be certified for man
riding.
• Ensure all lifting equipment should have currently valid certificate of fitness.
• Rigger and Slinger should be formally trained in rigging and slinging method.

• Understand the project Scaffolding procedure-MS and requirement during the period
of project, Established strategic approaches to implement project scaffolding
procedures – MS by defined rules of team leader and influence them in safe
practices (BS 1139).
• All scaffolding material and equipment to use at site must be approved and certified
by SSB/SSPC and PETRONAScarigali Malaysia.
• Scaffolding material & equipment are comfort BS 1139 standard including scaffolding
plank should be certified by SSB/SSPC and colour coding as per project requirement.
• Every erection and modification shall have assigned a person in charged to minimize
damage to equipment.
• All personal involved in erection, modification& dismantling should have up to date
and formal training required for their role, Complete PPE must be worn.
• Ensure scaffolding platform are clearly marked follow is state by category e.g. Very
light duty, Light Duty and Heavy Duty and Scaffolding tagging.
• Must inspect and up to date by Foreman and scaffolding Inspector shall have valid
tagging of fitness.
• Scaffolder should be formally trained in scaffolding erection, Modification and
Dismantling.
• Ensure the scaffolding erected firmly on ground and stability condition. Ensure the

(1) Offshore Shift Supervisor Scope of work: • To be acting as a construction supervisor OR superintendent in event of meeting/courses and non-available at workplace. • To lead/monitor/supervise for whole the tasks carried out is accordingly with company/client and project owner requirement.  To promote Safety program and push for zero accident at work site.  To review of work procedures, testing procedure, installation procedure and JHA.  To Execute planed weekly piping work target to meet progress schedule.  To manage shop welding production.  To represent client in all inspection hold point.  To review and approved cutting plan for shop fabrication of pipe spools and steel structural.  To orientation and dimensional check during and after fabricated against AFC drawing.  To check material received against AFC drawing and mill certificate.  To inspect parallelism, levelness of rotating equipment nozzle connection according to Project and GP standard.  To inspect and witness static equipment installation for levelness, plumpness and internal inspection.

Work Location : Work Boat/Barge @ Site Team. Project : Shell Provision Hook-Up, Construction/Topsides Major Maintenance & Commissioning Client : Shell Sarawak Berhad [SSB/EPA]/Sabah Shell Petroleum Company [SSPC Co. Ltd]. • To assist offshore shift supervisor/construction supervisor and superintendent to lead/monitor/supervise for whole the tasks carried out is accordingly with company/client and project owner requirement. • To ensure that the work/job is perfecting during project turn around according to the work schedule/planning and target. • To co-operate with the offshore field engineer/planner if there is any changes of work pack/AFC drawing/PI & D drawing by project owner/client and OIM/IMO platform offshore installation manager/operation ,manager or onshore team. • To update the project materials/Material taken out (MTO) piping/structure/mechanical/quality/quantity is available on board/onsite, there for the whole project will be performed perfectly without any delay. • To report to duty construction shift supervisor and construction superintendent at worksite/work boat/work barge. • To refer/review the details of client/project AFC drawing/PI&D/updating work scope. • To update the project scope with project planner/engineer to known that whole the project scope/progress/target and period in time. • To attend the project meeting/DPR/ and progress meeting is held on work barge/workboat. • To monitor heavy lifting/scaffolding/blasting & painting/pressurize/hydro testing and other critical activity at worksite. • To have a communication & handover to avoided miss – understanding between workers/supervisor each of shift. • To involve in daily progress/HSSE meeting at designated workplace. • To lead the construction teams to execute the job at site as per HSSE/client procedure/law & regulation. • To verify/check/walk-about/SSP (site safety procedure) meeting upon arrival at work location presentation by client. • To have a closed work with project/construction MATCO to update the COG/inventory/listing of materials availability at work site. • To prepare/submit & update the PTW (permit to work) form to client/representative at site for reviewing and approving.

Page 7 • To conduct daily pre-job discussion/brief the crew about new hazards and how to mitigate before commence a jobs. • To Communicate/Observe/monitor with construction crews/client/vendor and other party. • To monitor the crew closely during their duty to ensure that it is meet the client standard/procedure .Whether it is from HSSE/quality and quantity side. • To ensure that all the is according to the client procedure/Tasks. • To ensure that the workplace is in a safe condition/free from obstruction. (3) Offshore Rigging & Scaffolding Supervisor (Jan.2009 till Dec.201 Position : Rigging/Scaffolding Supervisor. Company : Petra Resources Sdn Bhd. Work Location : Work Boat/Barge and FPSO Gemusut Kakap sabah deep water Project : Shell Provision Hook-Up, Construction/Topsides Major Maintenance & Commissioning Client : Shell Sarawak Berhad [SSB/EPA]/Sabah Shell Petroleum Company [SSPC Co. Ltd]. Scope of work: • To liaise with construction shift supervisor/construction supervisor @ Superintendent, field engineer and key personnel to plan the works/preparation according to work pack/work scope by client. • To inspect and witness Hydro-testing, leak test, flushing and air drying activities. • To review material test certificate and all pressure testing calibration certificate. • To supervised and witness Low Pressure Tightness Test of piping system. • To conduct weekly audit on PTW system, Pre Task Talk, LOTO, QAQC and Housekeeping. • To coordination with HSE and QA team for quality inspection and HSSE measures to ensure safe and quality work place. • To coordinate site technical queries between Engineering and Construction team. • To arrange scaffoldings, material and man power for hook up activity. • To arrange facilities for back-up sub-contractor working at site. • To lead the piping team in the installation, erection and testing of piping works. • To killing punch-list for ready hydro testing. • To re-instatement of piping after hydro testing. • To review drawings such as Isometrics, P & ID, Pipe supports prior implementation at site. • To liaise with sub-contractor on work schedule, manpower availability and work activity. • To attend construction meeting for work progress & discussion regarding matters pertaining to piping works.
