5. ETIOLOGÍA
• Excesiva producción de oxido nítrico (ON) es la base de la vasodilatación.
• Otros mediadores: monóxido de carbono, estrógenos, prostaglandinas,
péptido intestinal vasoactivo, glucagón, sustancia P.
- Hepatopatía mas asociada es la cirrosis hepática.
- Otras: Hipertensión portal no cirrótica, hepatopatías agudas.
- Cualquier daño hepático y sobre todo crónico y con hipertensión portal.

• El SHP empeora el pronóstico del paciente con enfermedad hepática,
disminuye la sobrevivencia.
• Causa de muerte: complicaciones relacionadas a la EH más que

• Peor pronóstico : PaO2 < 60 mmHg: sobrevida : 6 meses, < 50 mmHg: 2 meses

26. CONCLUSIONES
• El HPS afecta al 10-30% de los pacientes evaluados para trasplante hepático
y afecta significativamente el pronóstico.
• Los pacientes con HPS son frecuentemente asintomáticos, lo que resulta en
un bajo reconocimiento de la enfermedad y un retraso en el diagnóstico.
• El estudio consiste en gasometría arterial y ecocardiografía de contraste.
• El uso de biomarcadores es un campo de investigación activa.
• El trasplante hepático es una opción terapéutica eficaz y da como resultado
resolución completa del síndrome.
• La supervivencia a largo plazo en HPS ha cambiado drásticamente desde la
implementación de la política de excepción estándar de MELD.
27. RECOMENDACIONES
• Se recomienda la toma de AGA a pacientes con enfermedad
hepática crónica o grave.
• Priorización correcta de los candidatos a trasplante hepático.
