This sample notice of change of address for a California civil case is used pursuant to the provisions of California Rule of Court 2.200 which requires every attorney or party whose mailing address, telephone number, fax number, or e-mail address, if one was provided, changes while an action is pending must serve on all parties and file a written notice of the change. The sample is in Microsoft Word format and can be easily modified.
