Comprehensive Employee Wellness Program
Preliminary Overview

Introduction:
With any functional and up-to-date employee health and wellness program, the internal health of
employees andtheirfamiliesisof primary importance. However, many of the more successful solutions
concentrate toalarge extentonfitness,weightlossanddiet. Whilethese aspectsof wellnessare of major
importance,theytendnot to address the more life altering issuessuchas majorillnessprevention,very
early detection of serious conditions such as cancer and clinical personal health data.
Furthermore, it is falsely assumed by most of us that the preventative medical coverage offered by
traditional healthinsurance isadequate,thatthe recordkeepingsystemsof ourdoctors are intact. That
is until we encounter major illness first hand, and open our eyes to the deficiencies of the most health
record systems.
One direct example of this is how our medical records, test results and reports seem to get misplaced,
misinterpreted, disorganized and not available to all the medical professionals working on our case.
Whenwe visita doctor’soffice we mustfill outthe same forms withthe same repetitive informationfor
eachnewdoctorwe visitandfindawaytogetpastmedical records(especiallytestresults) intheirhands.
Reasonsforthis disjointednessand “missingcoverage”are manyandvaried. Ourinternal analysisreveals
that the nature of our U.S. healthcare deliverysystem,itsstate of flux andthe clear-cutseparationof the
professional medical community from personal health involvement is an unfortunate, yet avoidable
component. Moreover, lack of collaboration between individuals and multiple professional medical
serviceshasbeencitedasthe mostsignificantcauseforlatedetection,givingrise to unnecessarysuffering
– not to mention the enormous cost in terms of employee down time and medical expenses.
Given the sweeping changes we are witnessing in our healthcare system, and given the impressive
breakthroughsin treatingmajorillnesseswhentheyare caughtearly,itisimperativethat employeesand
theirfamiliesare giventheopportunitytobecome moreactiveandeffectivelyinvolved intheirownhealth
outcomes. While the value of fitness,gooddietandweightmaintenance are notto be underestimated,
they are only a part of what is needed and the GoLifeBook™ Program encompasses all of the necessary
components.

The GoLifeBook™ Next Gen Wellness Program is a membership basedemployee wellness platform that
addresses the prevention of major illness, very early detection of serious health conditions and the
effectiveorganizationof personalhealthinformationinanall new andeffectiveway. It alsoprovides an
extensive knowledge base of relevant educational information on how a member can become more
involvedintheirownhealth,avoidmajorillnessandlate detectionandhow theycanbetterdetermineif
any major illness is beginning. Using optional DNA profiling (genetic testing), along with an employee’s
physician,the programcan provide information tothe memberongeneticissues intheirbloodline that
may place them at a higher risk for specific diseases. All information, including that generated by our
intelligent software can be shared with medical professionals.
From an employer’s view, the program can assist in employee retention by providing benefits for
professional level health monitoring. Information can be integrated into the employer’s HR system so
that a broad range of overall analyses are possible.
The cost of employee health coverage has risen and the liabilities faced by employers are higher than
expected. Also, when employers incentivize and support employee health programs, employee
satisfactionandavailabilityseemstoincrease. Employeeswhofeellike theiremployers“havetheirback”
and care abouttheirhealthare more likelytostayand believeinthe company,forthe long-term. Atthe
same time the cost to support prevention is far less that the cost of cure. This is true financiallyand
emotionally.
Information accumulatedwithinthe programcanbe organized andintegratedwithdownstreamsystems
and other popular informationmanagement platforms. This information can include itemssuch as test
results,familyhistory,graphs,xrays,scansand charts depictingchangesin personal health, alongwitha
full complementof analyticsforemployerandemployee decisionsupport. Additionally, theprogramcan
integrate overall fitness, weight management, diet and health monitoring through a mix of proprietary
and non-proprietary software and hardware interfaces/integration. Everything about the program can
be employer branded.
We strongly believe that in order for families and individuals to prevent catastrophic illnesses, and late
detection, theymustbecome more involvedintheirownhealth. Thisis because most major illnesscan
start without symptoms or pain and become progressively worse without our direct knowledge. The
definitive way to tell if we are affected is through continuous education and diagnostic analysis.
We have devisedaproprietarywaytofacilitatethis. The programisuniqueinthatinorganizesourbodies
into 12 distinct organ systems and addresses each of them separately. Another section of the program
deals specifically with brain health adding the brain as an additional organ system.
For example, our skin should be monitored for melanoma, other skin cancers and a myriad of other
conditions. Using our smart phone interface andour skinconditionanalysisandmonitoringsoftware,a
membercansnapa picture of amole orareaof concernand recorditintotheirown confidential personal
health repository, optionally housed within the employer’s HR system.
Oursoftware will usethe latestpatternrecognitionand otheranalyticstoprovideawealthof information
about the conditionsandoptionallysendthe informationtoone of our participatingphysiciansortothe

physicians of the member’s choice, for a professional opinion. One of the keys with skin conditions is
recording change.

Taking a single image is valuable, however, we know that it is any change in a mole or condition that is
most important. Therefore, members are urged to continue taking snapshots of the condition.
Our intelligent software will rate the severity of the condition, continue to monitor it and provide other
valuable information that a physician(s) can use to better understand what is going on. Also, the skin
conditionmaybe tiedtoothermore seriousconditionselsewhereinthe bodyandoursoftwarealsocross
checks for such situations. Similar features are associated with our cardiovascular system, digestive
system,male andfemale conditionsandso forth. Therefore,itfacilitatesthe employee’sunderstanding
of his or her health on an organ system by system basis.
Finally, through an extensive group buying platform, we can provide your employees with impressive
discounts on services such as lab, scans, studies and so forth that are usually only partially coveredby
insurance, if at all. Employers can use these additional discounted services in various rewards and
performance compensation programs.
Through our growing group of participating physicians, labs, imaging centers and other related
organizations,we cangoa longway tohelpyouremployeesandtheirfamiliesstayhealthy,geteducated
in personal health involvement and avoid the devastating effects of late detection and major illness.
We address employer compliance, comprehensive health support for employees and valuable decision
support information. The program offers a unique tool for the employer that goes far beyond basic
insurance coverage forpreventative activities. We strive to keepemployeesreallyhealthy,sharpandat
the top of their game.
Withrespecttobrainhealth,itisimportantforemployeesto remainsharp,mentallycapable,withstrong
orientation, focus and mental skills. Future intelligent software in being developed by us to assist
employees with brain health related activities, advanced learning skills and the ability to detect such
diseases as age related memory decline and other issues that could affect employee’s work levels and

Health Insurance Considerations
Most of the Affordable HealthInsurance Program planswe evaluatedhave some supportfor self-testing,
disease prevention testing,health monitoringactivitiesandannual physicals. While thisisfarbetterthat
want coverage has been in the past, especially with the elimination of pre-existing conditions, basic
coverage alone has not been shone to significantly improve outcomes.
Many policieshave additional coverage forpreventionandmonitoringavailable atadditional cost. Our
programeducatesemployeesastosuchinsurance issues,the latestevents andonwaystheycanincrease
