This is a presentation of 26 slides from an eight-week project that originated in the Human Factors course taught in the Science Bacc. Program at the Minneapolis College of Art and Design (MCAD) in Spring of 2012. It was a community parntership with MCAD, the Penny George Institute (PGI), and Allina Hospitals and Clinics.
It started on February 8 when Allina representative Jess Roberts conducted a workshop for the class at MCAD titled “What Is Wellness?”
Its official kick-off was March 7 when the class of 11 students visited the Penny George Institute for a tour and an interview with Dr. Greg Plotnikoff.
A team of six MCAD students (with one instructor and one coach) titled Team Micro set as its goal the improvement of patient and provider consultative experiences by a visual communications “rebalancing” that promotes and sustains holistic health outcomes. Deliverables ranged from intake form textual/visual redesign to reconceptualization of patient experiences at PGI as a journey of wellness examined through the eyes of three personas by an "information architecture" of visually expressed links and nodes.
The team create this presentation and two other report documents in order to help PGI fully realize and practice its vision-mission excerpted here:

Assisted Patient Intake
A clinic assistant helps patient complete the MyStory Journey to Wellness
Comprehensive Health History A scribe/assistant is going to be hired. Dr. Plotnikoff’s
supervisor gave the go ahead before we even pitched the idea
Role
The assistant creates a thread of continuity through the patient's journey at Penny
George. Although their role lives outside of the sustainable wellness process the intake
assistant is critical to compiling the patient's comprehensive health history. They send
out questionnaires, make appointments, answer questions and help with the
completion of the health history packet.
