To improve person-centred care for people with
long-term conditions by:
•Learning together – exploring similarities and
differences in understanding of person-centred
care
•Improving together - to deliver more personcentred care
•Sharing the project learning with others

• People accessing services
• Carers/befrienders
• Practitioners from health and social care- across
public, private and third sectors

It will take a grounded/situated approach by exploring the project aims within a given locality through the lens of delivering person-centred care and support to people with long-term conditions.
IRISS will be provide a framework and facilitate 5 or 6 sessions over a 12 month period, providing an opportunity for people to trial and experience new ways of working together. This will include co-production – or …..
Engaging with others as equals -as part of a learning and improving community by:
Valuing all participants – the ambition is to model this in all sessions, but we also reinforce this message by paying all unpaid contributors for their time (so the people who access services and unpaid carers/befrienders are paid just as professional staff are)
Building on the strengths/assets of the individual
Providing peer support to each other
Mutuality and reciprocity
In terms of the PROCESS – this is about facilitation, not delivery (or imposition)
Through this approach we believe you can::
Explore different perspectives; build stronger understandings, networks and relationships
Support action planning and implementation – to find joint, innovative and more creative solutions (than those ID by one sector or individual) with shared ownership/buy in
Shared learning on the process of co-production - through experiencing and reflecting on this.
In terms of experiencing new ways of working ….
For others this will be using improvement methodology – and how improvement can be coproduced (and person-centred) – explorative working and learning for all of us!
Plan, Do, Study and Act as part of a continuous cycle of improvement – starting small to achieve early wins before sequential testing/roll out.
This is the model of improvement prevalent in the health service.
