The Well Family System (WFS) is a case management software that: 1) Provides a complete system to manage multiple maternal, child, and family health programs through intake, assessment, care planning, referral and case management. 2) Connects individuals, families, programs and services over time to streamline service delivery and reduce duplication through a centralized system. 3) Uses risk factors and needs identified at intake to automatically generate individualized plans of care and family support plans that can be shared across an individual's various interactions with programs over their lifetime.
