  5. This activity aims to: a. establish a school baseline of literacy and numeracy levels of learners; b. craft school literacy and numeracy skills intervention plan; and c. identify potential partners to help in improving the literacy and numeracy skills of learners.
  6. Upon completion of the assessment ● SH shall submit the results to the PSDS/DIC ● Intervention Plan based on the analysis of the data gathered ● District Submission - September 16 ● Implementation of the approved IP - September 19, 2022 ● Division Submission - September 23, 2022
  7. Numeracy A. Assessment Tool 1. School shall utilize the teacher-made test in Math in each grade level with 5 test items per quarter. 2. Teachers handling Math subjects are enjoined to prepare the 20-item test as the pre-assessment tool for numeracy and to be quality assured by the school QA team chaired by the school head. 3. The school is responsible for the reproduction of the assessment tool. B. Conduct of the Test 1. All teachers handling Math subjects shall administer the assessment
