Ultrasonografía en el
diagnostico de
aborto
Etiología de los abortos.
**Precoces.
**Tardíos.
Clasificación y diagnóstico
ultrasonograficos.
Diagnostico diferencial:
**Clínica.
**Nivel sérico cuantitativo de HCG-
B.
**Ultrasonografía.
Clínicamente los abortos pueden ser clasificados como:
Amenaza de aborto.
Amenaza de aborto.
Ultrasonografía transvaginal identifica:
**Embrión con vitalidad conservada.
**Área de desprendimiento ovular no mayor del
40% del huevo.
Factores pronósticos a tomar en cuenta:
-Mal pronóstico: pacientes con grandes
hematomas subcoriónicos, con más de 35 años
de edad y con STV antes de las 8 seg.
-Mal pronóstico: Áreas de desprendimiento
centrales VS periféricas.
●RECUERDA… La fusión de las membranas
corioamnioticas ocurre alrededor de las 14sdg,
NO CONFUNDIR CON AREAS DE
DESPRENDIMIENTO OVULAR.
Amenaza de aborto.
Aborto inevitable.
Aborto incompleto.
Características:
**Eliminación parcial del contenido gestacional.
**Restos ovulares: contenido amorfo, cantidad
variable, hiperecogenico, localizado a lo largo
de la línea endometrial.
**Sangre contenida de ecogenicidad variable
según el tiempo transcurrido.
Aborto completo.
Características:
** Resolución espontanea de una gestación inviable.
**Después del sangrado y eliminación del contenido gestacional no hay dilatación
cervical y restos ovulares.
**Disminución progresiva de la hemorragia genital y dolor abdominal.
**Útero con volumen normal o aumentado discretamente y ausencia de contenido
en cavidad uterina.
**Cuidado!! Dx diferencial Embarazo ectópico.
Aborto retenido.
Características:
** Muerte embrionaria seguida de la no eliminación del producto
conceptual por mas de 28 dias.
**Ésta retención prolongada puede ocasionar cuagulopatia e
infección.
**Dx por Us: presencia de embrión si vitalidad o de saco gestacional
anembrionado que aun no ha sido eliminado de forma espontanea.
Aborto retenido.
Aborto habitual.
Características:
** Repetición de 3 perdidas gestacionales consecutivas o más.
**Primaria o secundaria.
**Causas más frecuentes:
Incompetencia istmico cervical:
**Incapacidad de sostener la gestación debido a presión excesiva del
feto y anexos en dirección al canal cervical.
**Primaria: mujer sin hijos, asociada a malformaciones congénitas o
cervicales.
**Secundaria: antecedente de 1 hijo, secundario a traumatismos
cervicales previos como partos, laceraciones vaginales y cervicales,
curetajes uterinos o cx cervicales.
**Clínica: dilatación insidiosa e indolora de cuello uterino que
evoluciona hacia aborto tardío.
**Us transvaginal: medida del tamaño del cuello uterino y OCI.
Acortamiento longitudinal del cuello uterino por debajo de 25mm
(20mm) sugiere el diagnostico.
-DETECCION US TV MAS COMPRESION FUNDICA DEL CUERPO
UTERINO identifica 19% de las pacientes.
Aborto habitual.
Incompetencia istmico cervical:
-Signo del dedo de guante.
-Dilatación del OCI >10mm y
profusión del SG >6mm es señal
de IIC.
**TX cerclaje cervical.
-USTV: estructuras ecogenicas
puntiformes o lineales en la
región cervical, con sombra
acústica posterior.
Aborto infectado.
Características:
** Historia de manipulación intrauterina (aborto provocado), señales de
infección materna, hipertermia, secesión vaginal fétida. Uso de DIU.
**El cuadro ultrasonografico varía dependiendo del tiempo de evolución.
**Us: la presencia de restos ovulares y endomiometritis aparece como
imágenes ecogenicas en la pared miometrial y cavidad uterina.

  • 4. Clasificación y diagnóstico ultrasonograficos. Diagnostico diferencial: **Clínica. **Nivel sérico cuantitativo de HCG- B. **Ultrasonografía. Clínicamente los abortos pueden ser clasificados como:
  • 6. Amenaza de aborto. Ultrasonografía transvaginal identifica: **Embrión con vitalidad conservada. **Área de desprendimiento ovular no mayor del 40% del huevo. Factores pronósticos a tomar en cuenta: -Mal pronóstico: pacientes con grandes hematomas subcoriónicos, con más de 35 años de edad y con STV antes de las 8 seg. -Mal pronóstico: Áreas de desprendimiento centrales VS periféricas. ●RECUERDA… La fusión de las membranas corioamnioticas ocurre alrededor de las 14sdg, NO CONFUNDIR CON AREAS DE DESPRENDIMIENTO OVULAR.
  • 9. Aborto incompleto. Características: **Eliminación parcial del contenido gestacional. **Restos ovulares: contenido amorfo, cantidad variable, hiperecogenico, localizado a lo largo de la línea endometrial. **Sangre contenida de ecogenicidad variable según el tiempo transcurrido.
  • 10. Aborto completo. Características: ** Resolución espontanea de una gestación inviable. **Después del sangrado y eliminación del contenido gestacional no hay dilatación cervical y restos ovulares. **Disminución progresiva de la hemorragia genital y dolor abdominal. **Útero con volumen normal o aumentado discretamente y ausencia de contenido en cavidad uterina. **Cuidado!! Dx diferencial Embarazo ectópico.

Características: ** Muerte embrionaria seguida de la no eliminación del producto conceptual por mas de 28 dias. **Ésta retención prolongada puede ocasionar cuagulopatia e infección. **Dx por Us: presencia de embrión si vitalidad o de saco gestacional anembrionado que aun no ha sido eliminado de forma espontanea.
  • 13. Aborto habitual. Características: ** Repetición de 3 perdidas gestacionales consecutivas o más. **Primaria o secundaria. **Causas más frecuentes: Incompetencia istmico cervical: **Incapacidad de sostener la gestación debido a presión excesiva del feto y anexos en dirección al canal cervical. **Primaria: mujer sin hijos, asociada a malformaciones congénitas o cervicales. **Secundaria: antecedente de 1 hijo, secundario a traumatismos cervicales previos como partos, laceraciones vaginales y cervicales, curetajes uterinos o cx cervicales. **Clínica: dilatación insidiosa e indolora de cuello uterino que evoluciona hacia aborto tardío. **Us transvaginal: medida del tamaño del cuello uterino y OCI. Acortamiento longitudinal del cuello uterino por debajo de 25mm (20mm) sugiere el diagnostico. -DETECCION US TV MAS COMPRESION FUNDICA DEL CUERPO UTERINO identifica 19% de las pacientes.
  • 14. Aborto habitual. Incompetencia istmico cervical: -Signo del dedo de guante. -Dilatación del OCI >10mm y profusión del SG >6mm es señal de IIC. **TX cerclaje cervical. -USTV: estructuras ecogenicas puntiformes o lineales en la región cervical, con sombra acústica posterior.
  • 15. Aborto infectado. Características: ** Historia de manipulación intrauterina (aborto provocado), señales de infección materna, hipertermia, secesión vaginal fétida. Uso de DIU. **El cuadro ultrasonografico varía dependiendo del tiempo de evolución. **Us: la presencia de restos ovulares y endomiometritis aparece como imágenes ecogenicas en la pared miometrial y cavidad uterina.
