5. The Specialty of
General Practice/Family Medicine
• General practitioners/family doctors are specialist physicians trained in the principles of the discipline.
• They are personal doctors, primarily responsible for the provision of comprehensive and continuing
care to every individual seeking medical care irrespective of age, sex and illness.
• They care for individuals in the context of their family, their community, and their culture, always
respecting the autonomy of their patients.
• They recognise they will also have a professional responsibility to their community.
• In negotiating management plans with their patients they integrate physical, psychological, social,
cultural and existential factors, utilising the knowledge and trust engendered by repeated contacts.
• General practitioners/family physicians exercise their professional role by promoting health,
preventing disease providing cure, care, or palliation and promoting patient empowerment and
self-management. This is done either directly or through the services of others according to health
needs and the resources available within the community they serve, assisting patients where
necessary in accessing these services.
• They must take the responsibility for developing and maintaining their skills, personal balance and
values as a basis for effective and safe patient care.
• Like other medical professionals, they must take responsibility for continuously monitoring,
maintaining and if necessary improving clinical aspects, services and organisation, patient safety and
patient satisfaction of the care they provide.

• Δίνεται προτεραιότητα στην κάλυψη του πληθυσμού μέσω των
δημόσιων δομών και ΌΧΙ στις προτιμήσεις του πολίτη!
• Στη χώρα μας, με το πυκνότερο δίκτυο ιδιωτών ιατρών στα αστικά
κέντρα στον κόσμο,
φαντάζει πολύ πιο πρόσφορη λύση η σύναψη συμβάσεων του ΕΟΠΥΥ
με αυτούς,
παρά η δημιουργία εξ αρχής νέων κρατικών δομών με παράλληλα
παροπλισμό των ήδη υπαρχόντων δομών και εξοπλισμών…
• Η χρηματοδότηση μέσω ΕΣΠΑ θα μπορούσε να αφορά την παροχή
κινήτρων στους γιατρούς για συνενώσεις και δημιουργία group practices
που αποτελεί διεθνώς την επιθυμητή δομή ΠΦΥ
80. Δημόσιο vs κρατικό…
• Αν αποφασίσεις να δημιουργήσεις κρατικές δομές ΠΦΥ,
παράλληλα με τη λειτουργία ιδιωτικών δομών, και στόχος σου
είναι η ποιότητα της φροντίδας,
δώσε την ευκαιρία να παίξουν με ίσους όρους και
να αναπτύξουν υγιή ανταγωνισμό μεταξύ τους…
Δεν επιτρέπεται η μεροληψία υπέρ του κρατικού…
• Στο τέλος της τετραετίας δες ΠΟΙΟΤΗΤΑ+ ΑΠΟΔΟΤΙΚΟΤΗΤΑ:
- ποιες είναι οι προτιμήσεις και οι απόψεις των χρηστών για την κάθε μορφή
- ποια τα αποτελέσματα τους σε υγειονομικούς δείκτες
- ποιο το κόστος λειτουργίας τους
για να αποφασίσεις την τελική μορφή του συστήματος….
81. Ελευθερία επιλογής οικογενειακού
ιατρού από ασθενή;
Κάθε οικογενειακός γιατρός έχει "χρεωμένα" περίπου 2.000 άτομα.
• Απαραίτητη για ενδυνάμωση του χρήστη και τη συνεχή εγρήγορση του
ιατρού για παροχή υπηρεσιών υψηλής ποιότητας
• Ο ασθενής θα πρέπει να έχει δυνατότητα αλλαγής οικογενειακού ιατρού,
όταν δεν είναι ικανοποιημένος
• Τα χρήματα ακολουθούν τον ασθενή (capitation based payment)
• Ναι μεν επιλογή από τον πολίτη, αλλά από μια μικρή δεξαμενή γιατρών
• Η πλειοψηφία των γιατρών γύρισε την πλάτη
• Μόνο περίπου 15% των πολιτών στα αστικά κέντρα μπορούν να
καλυφθούν από τους διαθέσιμους οικογενειακούς γιατρούς
• Η σχέση ασθενούς- οικογενειακού γιατρού χτίζεται στην πάροδο του
χρόνου και είναι μια σχέση εμπιστοσύνης
82. Λίστα οικογενειακού γιατρού ≤ 2250 πολίτες
• Η λίστα προάγει τη συνέχεια στη φροντίδα. Απαραίτητη για την
ανάπτυξη ευθύνης/ λογοδοσία για την υγεία ορισμένου
πληθυσμού
• Σε καμία περίπτωση πάνω από 2000 άτομα!
Στοχεύοντας στην ποιότητα καλό θα ήταν ακόμα μικρότερο
μέγεθος λίστας
• μέσος πληθυσμός / GP στην Ευρώπη: 1687
• Μέσο μέγεθος λίστας ΟΙ: Turkey (3687), Malta (2500), the
Netherlands (2322), Slovakia (2163); Οι μικρότερες στο
Luxembourg (500), Belgium (718), France (800), Italy (1094) και
Norway (1219)

88. Δεν υπάρχουν αυτά τα λεφτά για να δοθούν,
από τον ΕΟΠΥΥ στην ΠΦΥ…
7.000.000 x 70% x 25 € = 122.500.000 €/ έτος
• Ανακατανομή αποδοτικότερη των σπανίων πόρων που δαπανά ο ΕΟΠΥΥ
(σχεδόν τα χρήματα που δαπανά σε συμπληρώματα διατροφής και επιθέματα…)
• εισαγωγή κλειστών προϋπολογισμών για τον πληθυσμό ευθύνης τους σε
φαρμακευτική δαπάνη και σε δαπάνη εργαστηριακών εξετάσεων
• παροχή κινήτρων σε αυτούς για όσο το δυνατόν αποδοτικότερη
συνταγογράφηση (βλ. Πορτογαλία)
• εισαγωγή συμμετοχής στη δαπάνη για ΟΙ, από 0 - 25 €/ έτος, ανάλογη του
εισοδήματος και αντιστρόφως ανάλογη των αναγκών υγείας
(στα φάρμακα και στις εξετάσεις συμμετοχή θεμιτή;)
• Αυτοχρηματοδότηση της μέσω της οικονομίας που θα επιφέρει από τον
περιορισμό της ακατάλληλης φροντίδας…

Προτιμότερη η συμμετοχή του ασθενούς στο
capitation fee κατ’ αναλογία του εισοδήματος του
από την ανεπαρκή χρηματοδότηση του
εγχειρήματος, που θα το καταδικάσει…
94. Προσαρμογές…
• Περιορισμός του ωραρίου που διατίθεται για τον ΕΟΠΥΥ από τους
συμβεβλημένους – 20 ώρες/ εβδ…
• Γιατροί άνευ ειδικότητας, υπερήλικες γιατροί…
• Γιατροί μεταφέρονται σε ΤοΜΥ από ΚΥ και περιφερικά ιατρεία…
• ΤοΜΥ με πλήρεις ομάδες υγείας, αλλά 1-2 γιατρούς…
• Συμβεβλήμενοι παραμένουν χωρίς την υποχρέωση να δηλώνουν
το ωράριο που διαθέτουν για τον ΕΟΠΥΥ…
• Δεν λειτουργούν τα e-ραντεβού στους συμβεβλημένους…
• Παράταση στην κατ΄ αποκοπή αποζημίωση των συμβεβλημένων
για έξι μήνες- όχι per capita…
• 1800 ευρώ για 20-30 ραντεβού/ μήνα…
Να μην χάσω και αυτούς που έχω….
95. • Υποχρεωτική η δήλωση οικογενειακού
γιατρού
διασύνδεση της υποχρέωσης με την κάλυψη παροχών από
τον ΕΟΠΥΥ(φάρμακα, εξετάσεις, παραπομπές)
• Ελεύθερη η επιλογή του από τον ασθενή –
ακόμα και αμιγώς ιδιώτη ΟΙ
κατά αντιστοιχία με την υποχρεωτική εκπαίδευση και τα
ιδιωτικά σχολεία…
96. Αλλά θέλω κι άλλα κάνω…
• Όχι φυσικοθεραπείες από ΓΙ
• Όχι επιθέματα κατακλίσεων από ΓΙ
• Περιορισμός στη δυνατότητα συνταγογράφησης
οπιούχων αναλγητικών από τους ΓΙ
• Περιορισμός στη δυνατότητα συνταγογράφησης
θεραπευτικής αγωγής για ΧΑΠ, ΓΟΠ από ΓΙ και
παθολόγους- άραγε σύντομα και από τις νεότερες
θεραπείες για το ΣΔ;
• Αποκλεισμός ΓΙ από τη χορήγηση πιστοποιητικού για
άδεια όπλου, κάρτα υγείας αθλητή
• Αποκλείονται οι ΓΙ από το ρόλο του ΟΙ για παιδιά
97. «Η Γενική Ιατρική είναι ιδιαίτερη ιατρική ειδικότητα, η οποία δεν
προσδιορίζεται τόσο από το είδος των νοσημάτων που καλύπτει, αλλά από
τον τελείως διαφορετικό, σε σύγκριση με τις άλλες ιατρικές ειδικότητες,
τρόπο με τον οποίο προσεγγίζει τα προβλήματα υγείας.
Αποτελεί τη βάση κάθε οργανωμένου συστήματος υγείας, αφού είναι η
μόνη που μπορεί να παίξει συντονιστικό ρόλο, απαραίτητο για τη λειτουργία
του συστήματος.
Ο γενικός γιατρός νοιώθει αμήχανος και ανικανοποίητος όταν είναι
υποχρεωμένος να προσφέρει υπηρεσίες έξω από ένα ολοκληρωμένο
σύστημα πρωτοβάθμιας περίθαλψης,
αλλά και το πρόγραμμα εκπαίδευσης στη Γενική Ιατρική δεν μπορεί να
σχεδιαστεί με ακρίβεια όταν δεν είναι γνωστό το ποιες ακριβώς ανάγκες του
συστήματος αναμένεται να καλύψει ο μελλοντικός γενικός γιατρός»
