2. General principles of motor
control
Motor control is provided by the following
structures: frontal lobe, basal nuclei,
cerebellum, thalamus, brainstem nuclei
and spinal cord.
According to Bernstein“s theory of
movements, the motor-control system is
comprised of 5 structural/functional levels
3. 1) Rubrospinal level
paleokinetic regulation
1) Thalamo-pallidar level
level of synergies
1) Pyramido-striatal level
level of spatial fields
1) Parietal-premotor level
level of object actions
1) Higher cortical levels
5. 2. Thalamo-pallidar level (level
of synergies) controls synergies
of muscles, coordination of
highly practiced movements
which involve many muscle
groups and require constant
correction.
6. 3. Pyramido-striatal level (level of
сенсорной информации;
7. 4. Parietal-premotor level (level of
схематизации пространства;
9. Planning of the movement begins in
prefrontal cortex.
The following structures participate as
well:
limbic system,
thalamic nuclei
reticular formation
Parietal association cortex
10. Limbic system is important in
spontaneus movements that
arise due to internal stimulus
(one or another motivation)
11. Parietal Association cortex
plays important role in
movements
that are initiated by external
stimuli (e.g. appearance of
the object in visual field)
17. Each medullary pyramid in humans contains
approximately 1 million fibers. However, only
30—40% of them arise from Betz cells.
The source for 30-40% is prefrontal cortex.
The rest 30—40% are axons of neurons of
parietal lobe (somatocensory cortex) that
connect to interneurons of dorsal horns and
control spinal reflexes and sensory input.
18. Corticospinal fibers excite spinal neurons via
glutamate, although they can facilitate
inhibiting and exciting mechanisms in spinal
cord segments.
19. Differential diagnosis of peripheral
and central paralysis
Peripheral (flaccid) paralysis:
Lower motor neuron lesion
Muscle atrophy
Hypotonia
Loss of tendon reflexes
Reaction of degeneration on electromyography
20. Central paralysis: Upper motor neuron lesion
Little hypotrophy
Spasticity
Increased tendon reflexes
Extended reflexogenic zone
Pathological reflexes (e.g. Babinski response)
Synkinesis, myoclonus
Defense reflexes (flexor reflexes)
Differential diagnosis of peripheral
and central paralysis
21. Global synkinesis
A patient is asked to move intact
extremities (e.g. to raise his hand), and
involuntary movements on the affected side is
seen:
Flexion in elbow joint, adduction to the trunk,
pronation of the forearm, flexion of the hand
and so on
23. Coordination synkinesis
24. Brainstem — spinal cord tracts in
regulation of movements
Ventromedial motor system
Dorsolateral motor system
29. Interstitiospinal tract
Arises from interstitial nucleus of Cajal,
it is connected to medial longitudinal
fascicle
Its activation cause axial rotation of the
body.
Lesion of this tract takes place in
torsion dystonia and supranuclear
weakness.
30. Olivospinal tract
Neurons of olives compare the
desired result of movements
from motor cortex to the real
movement from sensory
systems.
31. Role of cerebellum in
movement control:
2.Feedback controller
Visual, Vestibular, and Somatosensory
information enters the cerebellum to correct
34. Extrapyramidal system
The main fuction of extrapyramidal system is
to regulate resting muscle tone, posture and
automatic movements.
It prepares skeletal muscles to contraction in
response to nerve impulses.
35. Damage to the extrapyramidal
system at any site will result
in
1. rigidity (muscle hypertonia)
2. hypokinesia and
hyperkinesia
Rigidity
41. Classification of tics and tic- like
hyperkinetic disorders
1.Idiopathic tics
а) acute преходяще,
в) chronic простые и сложные
45. Tic is a sudden repetitive movement involving
discrete muscle groups.
It resembles normal movement, can vary in
strength and is not rhythmical
Tics can be voluntarily suppressed and can be
easily imitated
Short contractions of the muscles that are
involved in the movement, or simultaneous
activation of antagonist muscles 50-500 ms.
Long.
46. Mannerism
47. Gesture is a culture-specific
movement that reflects particular
state of mind and can be used for
non-verbal communication.
48. Repetitive Behavior-
49. Motor stereotypies are
Repetitive movements that do not
have a purpose, last for a long time
and interfere with normal activities.
54. Chronic simple or multiple tics —
one or many tics that last the
whole life long and do not change
in frequency or duration
55. Tourette syndrome
Multiple tics
Coprolalia (Uncontrollable cursing and swearing)
Vocal tics (Roaring, coughing, sneezing...)
Echolalia (patients repeat noises and phrases that they
hear)

muscles of lower extremities
Rigidity — hypertonia in flexor muscles of upper
and lower extremities
59. In single passive movement
