Advanced Health Assessment & Diagnostic Reasoning. An annotated bibliography is a
combination of the words annotation and bibliography. An annotation is a set of notes,
comments, or critiques. A bibliography is a list of references that helps a reader identify
sources of information. An annotated bibliography, then, is a list of references that not only
identifies the sources of information but also includes information such as a summary, a
critique or analysis, and an application of those sources' information.Advanced Health

Module 2 Assignment (due in Week 5) will be a paper on nurse leadership during
transitions of care for two selected entities. In preparation for that paper, you will develop
an Annotated Bibliography and submit it to your Instructor for feedback. To prepare: Select
a transition of care (hospital, specialty care, rehabilitation, nursing home, homebound, etc.).
Use the Walden Library and other reputable academic resources to locate 5 scholarly
resources on nurse leadership during this transition of care. Identify what constitutes
effectiveness (i.e., cost, care management, best/effective providers, best setting, sustaining
outcomes) for this transition of care within each setting. What evidence supports this
transition of care or transition of care intervention? Review the Writing Center resources on
annotated bibliographies and scholarly writing. Also review the Week 4 Assignment Rubric
for specific details about the requirements for your annotated bibliography. Assignment
Create an annotated bibliography of the 5 scholarly resources you located on your selected
transition of care. Follow the annotated bibliography format presented in the Writing
Center resource. *Reminder: The School of Nursing requires that all papers submitted
include a title page, introduction, summary, and references. The Sample Paper provided at
the Walden Writing Center provides an example of those required elements (available at
http://writingcenter.waldenu.edu/57.htm). All papers submitted must use this
formatting.Advanced Health Assessment & Diagnostic Reasoning.Advanced Health
Assessment & Diagnostic ReasoningHealth Issues and Risks that are Relevant to the
ChildThere are various health risks and issues associated with underweight in children and
adolescents. According to Ochiaiet al. (2017), underweight in the pediatric population is
associated with health issues such as scoliosis, pubertal delay, osteoporosis, and mental
health disorders. Therefore, the health issues and risks relevant to the child include hair,
skin or teeth problems because lack of adequate nutrients can lead to skin thinning, dry
skin, loss of hair, and poor dental health. In addition, the underweight is associated with
poor health and hence the child is at risk of becoming sick frequently due to lack of the
2. required nutrients and poor immunity (Patel et al., 2018). Consequently, the child may get
sick more often and infections such as flu. The child can also develop anemia because of low
blood counts and experience headaches, dizziness and fatigue. Moreover, the child is likely
to experience impaired and low growth being children require nutrients to develop and
grow healthy bones and thus lack of enough nutrients may lead to poor development(Ochiai
et al., 2017).Advanced Health Assessment & Diagnostic Reasoning.More importantly, the
child is at risk of developing mental health problems associated with body appearance and
body image dissatisfaction. For example, the child in this case study was bullied for having a
different appearance (underweight). Physical appearance is a common reason for bullying
and bullying is associated with body dissatisfaction and social consequences allied to the
appearance, for instance, discrimination and stigma (Waseem et al., 2014). Children who
are bullied because of their weight are at higher risk of developing psychological
morbidities like depression, low self-esteem and anxiety (Ochiaiet al., 2017).Advanced

Assess Weight-Related HealthAdditional information to further evaluate the weight-related
health for this child includes the family history because some individuals have low BMI
because of the physical traits and genetics that run in the family. Secondly, it would be
necessary to assess the metabolism rate for the child because individuals with high
metabolism tend not to gain weight as expected. There is also a need to assess the physical
activities for the child because high levels of physical activities can burn significant calories
resulting in low body weight. More importantly, information regarding if the child has any
chronic illness because some illness and diseases may make it difficult to gain weight while
other conditions can reduce the appetite and hence lead to low weight (Maris & Nicholls,
2016).Advanced Health Assessment & Diagnostic Reasoning.Gathering the Information in a
Sensitive FashionThe healthcare provider should avoid appearing too personal when
gathering information and thus avoid asking overly personal questions. This is because if
the patient feels that the healthcare provider is asking overly personal questions, they may
be reluctant to provide such information. In addition, it would be important to ask
questions respectfully and use open-ended questions in order to give the patient an
opportunity to frame their answers and share information from their own perspective. It is
also important to avoid being judgmental or offeringpersonal opinions when gathering
information.Questions to ask about the Child to Gather More Information1. How would you
describe your diet at home?Advanced Health Assessment & Diagnostic Reasoning.2. Please
tell me of any health problem the child may have had throughout her life?3. How active is
the child physically? Does she engage in extreme physical activities?4. Did the weight loss
for the child occur gradually or suddenly?5. In the last four weeks, was there a day any
person in the family stayed hungry due tolack of money to buy food?6. Within the last year,
is there a time you were worried that food would run out before getting money to purchase
more food?Strategies to Encourage Parents/Caregivers to be Proactive about their Child’s
Health and Weight• The nurse should encourage the caregivers and parents to promote
healthy eating patterns for the child by ensuring that the child is given food frequently but
in small portions and giving nutritious snacks. The parents and caregivers should also
model healthy eating for the child• Parents and caregivers should be encouraged to support
3. changes in the lifestyle behaviors of the child such as taking meals together with the child in
order to encourage the child to eat and finish her food portions (Ochiai et al., 2017).• The
parents and caregivers should actively encourage the child to eat and encourage small
incremental changes that are sustainable and the child is likely to maintain in the long-term.
The parents and caregivers should help improve the self-esteem of the child since she is
being bullied by affirming that she is beautiful but needs to improve her food consumption
to stay healthy (Avis et al., 2015).Advanced Health Assessment & Diagnostic Reasoning.
