This document discusses a scenario for providing primary health and wellbeing services through smart homes in Crete, Greece. It outlines the need for new health services as the population ages. A roadmap is presented showing the evolution of health technologies from episodic treatment to personalized healthcare. The presentation calls for systemic innovation across technology, services, organizations, and skills to develop an integrated network of public and private health providers, including building contractors and telecom companies. Success will require new knowledge in customer needs, services, tools, and business models as well as technical and managerial skills training. A local initiative is proposed to pioneer these smart home health services on Crete.
