   1. Family Condition: My study habits is affected because;
       _____ I cannot make my projects and assignment in my home because my siblings
                and other members disturbing me.
       _____ I had difficulty concentrating my studies because I had lots of household
                chores to be done.
       _____ I cannot study my lesson at home because the home surrounding is very noisy
                including our neighbors.
       _____ I am disturbed by frequent arguments/quarrels of my parents (other family
                members).
       _____ I am disturb of my study of thinking my love ones who is sick.

   2. Technology: My study habits is affected because;
        _____ I am not computer literate.
        _____ I do not have computer in my home or boarding house.
        _____ there are no internet café’ near our vicinity. Or, the internet café’ is quite far
                from our home.
        ______ I do not have laptop, which hard for me to have access sources or references
                while I am researching or studying.
       ______ I feel I am having difficult situation competing the performance with others
                since they have an available technology at hand while studying.

   3. Class Schedule: My study habits is affected because;
       _____ of the succeeding noc shift in my duty schedule.
       _____ I am scheduled in out of town for duty almost every week.
       _____ my lecture schedule makes me very sleepy, which it distracted my
              understanding the lesson.
_____My schedule in class is already late night, and I am already feels exhausted.
_____ My lecture schedule in one subject is arranged in long term period (whole day)
        which it cannot sustain in concentrating my studies.
