Figure 1 describes the conceptual framework of the study wherein the input isconsisting of the following: Age, Sex, Educational attainment, Position (work), Monthlyincome, Family history in this disease, and Length of having the illness, it also includesthe social support scale that end stage renal disease patient had. On the other side, the process being used is through questionnaire, descriptive-survey and statistical treatment such as percentage, mean and tallied. The output is theSelf-care ability of Hemodialysis patients with end stage renal disease. The researchers want to find out the Self-care ability of Hemodialysis patientswith ESRD at Ospital ng Sampaloc prior to the initiation of the treatment; A basis forformulation of teaching plan. The table shows how the researchers researched, surveyedand solved their thesis statement.
