    Authorization and Release Form
You must have three copies completed and NOTARIZED.
How do you get things notarized? Email Office of Student Services for an appointment.

    Application Fee Worksheet
    Complete the worksheet and
Attach a check or money order. Many students do not have checks “in their name” (i.e. they do not have checks with their names printed on them). If you include a check, the check does not have to be “in your name” (i.e. it can be your relative or friend’s check), just make sure to write your name in the Memo Section.
Fingerprinting Receipt. When you get your fingerprints taken (either here or at an officially sanctioned MORPHO TRUST location), you will be given a paper receipt. Keep that receipt and take a picture of it with your phone; you will have to include that paper receipt in your packet. If you lose your receipt, call MORPHO TRUST at 800-528-1358.

  • *We highly encourage you to send these documents in with your application packet at the time of your initial application. If, however, you are still waiting for these documents at the time you need to submit your application, you can submit your application without them (so long as you have the Required Documents in your Packet). You can mail these to the FBBE as soon as you receive them.

    Proof of Citizenship

If you were born in the U.S., you must send in a certified copy of your birth certificate, not the original.
If you were born in Florida, you must mail in an application for a certified copy, which is available at www.floridahealth.gov. You must receive the certificate at home and mail it in with your application packet.
If you are a non-naturalized U.S. citizen who was born abroad (to U.S. parents), the documentation requirement is different; see me.
If you were born outside the U.S., but are a naturalized U.S. citizen, you must provide a photocopy of your certificate of naturalization (and the Bar will submit it to USCIS for verification). For those whose naturalization certificate is in your parent’s name only, see me.
If you are not a U.S. citizen, you must provide a photocopy of the documents that grant your status (and the Bar will submit to USCIS for verification).

    1. Undergraduate and graduate school transcripts: ask that the school mail the transcripts directly to:

    2. Take a screenshot of the request form (if your school uses online requests) and include that screenshot in your application packet. If your school uses paper request form, keep a copy of that request and include the copy in your application packet.

Supplemental Items Must be submitted in hard copy if you are unable to upload them with the application. These documents apply only to applicants who answer “yes” to certain questions.
    Bar Applications to other jurisdictions
    Letters to doctors
    Copies of other litigation documents
    Checking account records
    Certificates of good standing
    Bankruptcy documents

    Financial Affidavit

Your application will not be complete until the FBBE has received all of these documents.
  • The Florida Bar Application requires disclosure of substance use disorders, such as dependency, addiction and abuse. Severe mental health disorders are required to be disclosed. Severe mental health disorders include, but are not limited to, schizophrenia, psychotic disorders, bipolar disorder and major depressive mood disorder.

The bar application does not require you to disclose treatment for adjustment disorders, general anxiety, general depression and attention deficit disorder. Nor does it require you to disclose stress, marital or grief counseling. It is important to emphasize that the board encourages applicants experiencing difficulty of coping with the stress of law school and daily life to seek counseling through Florida Lawyer’s Assistance, your school’s counseling center or a private treatment provider. No matter what else you may hear, you will not be penalized for seeking counseling. Be assured that you are not required to report such counseling and, if you do, the board will not investigate it.
