Many able bodied household members in Rukungiri district have died of HIV/AIDS. Consequently there are many house heads (often young and married) whose partners died. This study investigated the impact of HIV/AIDS on rural household welfare in Rukungiri district. A control group approach was used. Data was collected by the use of questionnaires and empirical observation from both the affected households and unaffected households. The study aimed at finding out how HIV/AIDS has led to depletion of households’ productive assets. A binary logistic regression analysis was used to establish whether there is a significant difference in the sources of income for medical expenses between the affected households and unaffected households with the aim of finding out the extent to which HIV/AIDS has led to depletion of households’ productive assets and consequent deterioration in households’ welfare. The study has established that HIV/AIDS has led to deterioration in the welfare of the affected households through exhaustion of savings and increased borrowing, with the end result of depletion of productive assets leaving the survivors with minimal means of survival. Productive assets commonly depleted include; land, cattle, goats, chicken, sewing machines, wheelbarrows and bicycles. However, affected households have adopted many coping strategies including those that aim at improving food security, raising and supplementing their incomes so as to maintain their expenditure patterns, and coping with loss of labour. Since emergency traditional indigenous groups are already operating in all the communities in the district acting as a source of psychosocial support to individuals and communities affected by HIV/AIDS with their activities such as assisting with burial ceremonies, communal farming, supporting sick patients, rebuilding dwellings and rehabilitating farms, supporting survivors and creating income generating activities and providing material support, the study recommends government support in collaboration with donor agencies to provide them with training in HIV/AIDS home care support and enough funds to enable them meet their requirements. Government in collaboration with NGOs operating in the area should introduce short-term training courses in the affected areas to empower the survivors with practical skills and equip them with capital that can enable them create jobs for themselves. Further, improving agricultural yields through teaching them modern farming practices, provision of free anti-retroviral therapies by the government to all the affected households, accompanied by provision of food rations for boosting nutritional requirements would reduce the depletion of productive assets of these households.

Within the context of the HIV and AIDS pandemic and food insecurity, the high prevalence of under-nutrition, micronutrient deficiencies and emergent over-nutrition presents a complex series of challenges. Over the years, significant gains have been made with regards to scaling up nutrition, with the development of policies and guidelines for the implementation of nutrition strategies. There is now a renewed focus on specific priority groups for nutrition interventions to have a bigger impact.

Marama Pala Ngātiawa ki Kapiti, Kaiwhakahaere/Executive Director (INA), an HIV positive Māori woman, plans to promote the visibility of Indigenous People’s HIV and AIDS issues; to increase the support from local, regional and international Indigenous people. “History shows us that HIV is not openly discussed at Indigenous conferences. It’s an issue that gets left off the agenda.” says Marama Pala, “Even though Indigenous Peoples experience high rates of HIV and AIDS, it needs to be made visible at all Indigenous conferences and gatherings. It’s time that all Indigenous peoples became aware that we are a marginalised community vulnerable to HIV” Indigenous people globally continue to under represented in HIV prevention, research, policy and funding initiatives, and this contributes to ongoing HIV disparities and resource allocation. The stigma within Indigenous communities further marginalises this health issue, making it difficult to discuss. “I hope am able to stress the importance of HIV, speaking openly about HIV and facing the stigma of HIV with my Indigenous and aboriginal brothers and sisters” urges Marama. “Silence No More...for the sake of my HIV negative children; we need to eliminate stigma and discrimination and make our people safe.”

A transition period regarding in vitro medical device (IVD) regulation in the European Union (EU) is upon us. The former IVDD regulations are phasing out and IVDR 2017/746 has already taken its place as the acting regulation for IVD manufacturers but also lab developed tests (LDTs) and health institutions. Thoracotomy is the surgical procedure with an incision made to access the pleural space and the contents of the thoracic cavity. Given the structures to be accessed with in the cavity, different incisions have been established to easy the procedure and these incisions qualify the types of thoracotomies to be studied hereunder. PRE OP OPTIMIZATION FOR SPECIFIC FACTORS The outcome of surgical procedures is not measured only by clinical end points but also shorter stays and lower costs. Patients’ discharge is delayed commonly due to inadequate pain relief, infection, arrhythmias, prolonged air leak and debility. Many complications that occur from thoracic operations can be anticipated. An aggressive preoperative work up mitigates morbidity and shortens convalescence. APPROACH CONSIDERATIONS There are about three principles that can guides the choice of the thoracotomy incision to be used I. Adequate exposure must be achieved. The choice of incision is aided by a thorough understanding of the surface anatomy and a comprehensive review of the radiographic images that are obtained preoperatively. II. Chest-wall function and appearance should be preserved to the extent possible. This principle include non-spreading video-assisted thoracoscopic surgery (VATS) procedures, muscle-sparing techniques, avoidance of excessive rib retraction, and rib preservation when possible. III. The third principle is that closure must be meticulous and appropriate. Strict layered closure is the rule for thoracic surgical incisions. Every effort should be made to approximate the individual divided chest-wall muscles in appropriate layers; otherwise, a significant delay in the recovery of range of motion (ROM) may result. Care must be taken to avoid over approximating the ribs and to prevent an override; this will help minimize postoperative pain. POST-OPERATIVE CARE AND MONITORING
