Presenta la misma señal que el LCR en todas las secuencias. Es hiperintenso en T2, hipointenso en FLAIR, T1 y difusión, hiperintenso en los mapas ADC y no realza tras la administración de contraste . El diagnóstico diferencial hay que realizarlo con el quiste epidermoide, el hematoma subdural crónico y el quiste porencefálico.

El diagnóstico diferencial debe hacerse fundamentalmente con el quiste aracnoideo, el quiste dermoide y los quistes parasitarios de la neurocisticercosis.
QUISTE EPIDERMOIDE FOSA MEDIA IZDA DE 4X3 CM CON MINIMO EFECTO MASA. HIPO T1, HIPER T2, NO SUPRIME COMPLETAMENTE EN FLAIR Y RESTRINGE EN LA DIFUSION.
Anejos: folic. Pilosos, gl. Sebaceas, gl. Sudoriparas.
Con menor frecuencia, en la fosa posterior, vermis y IV ventrículo.
Cefalea en los mas jovenes, convulsiones en mas mayores.
A mayor tamaño mas prob de q se rompa.
Si se rompe gotas de grasa se observan, nivel graso en ventriculos.

1. Sagittal postcontrast T1-weighted MRimage shows rounded mass (solid arrow) separate from and just above the pituitary gland (open arrow). This cyst has moderately high protein content and is isointense with brain, not CSF.Location is typical for a Rathke cleft cyst, confirmed at surgery.

the velum interpositum is the normally narrow space between the roof of the third ventricle and the fornices at the inferior margin of the lateral ventricles.
A cavum velum interpositum (CVI) is a normal variation where there is a dilated CSF space involving the velum interpositum, which extends below the splenium of the corpus callosum and columns of the forinces and above the internal cerebral veins. It is triangular in shape, the apex pointing anteriorly reaching as far forward as the foramina of Monro 1,4

1. Coronal graphic shows PVSs as they accompany penetrating arteries into the basal ganglia, subcortical white matter. Normal PVSs cluster around the anterior commissure but occur in all
areas.
2. Axial T2WI MR obtained on 3T scan in a 60 yo shows prominent but completely normal PVSs in the basal ganglia (open arrows), subinsular cortex (arrows) and white matter (curved arrows).
3. Transverse contrast-enhanced T1-weighted MR image shows typical nonenhancing enlarged PVSs in right basal ganglia. The cluster of variably sized cysts is a common appearance.
4. Transverse T2-weighted MR image shows multiple bizarre-appearing cysts (arrows) in centrum semiovale and subcortical white matter of both hemispheres. The cysts vary in size and focally expand but otherwise spare the overlying cortex.

Pineal cysts are most often mistaken for—and may be indistinguishable from—a benign pineal parenchymal neoplasm called a pineocytoma. Pineocytomas are more likely to have solid components, but it may be impossible to distinguish the two with imaging studies alone

Algunos realce en anillo

3.Axial FLAIRMR in the same case shows the cyst (arrow) does not suppress and is hyperintense to brain. Note absence of transependymal CSFflow around atria of lateral ventricle.

Transverse T1-weighted MR image shows innumerable tiny low-signal-intensity neurocyticercosis cysts in brain parenchyma and subarachnoid spaces. Most contain small “dot” that represents the scolex (arrows).
Vesicular:1. The cyst contents appear similar to CSF on the T1- and T2-weighted images and do not demonstrate fluid restriction on the diffusion-weighted image. There is no surrounding edema on the fluid-attenuated inversion recovery image or enhancement after the intravenous administration of contrast material. The round structure within the cyst represents the scolex (small arrows in a–c).
Fase III: curación
Los quistes porencefálicos congénitos se originan por procesos encefaloclásticos perinatales o fetales secundarios a infecciones o infartos intrauterinos. Los quistes porencefálicos adquiridos se deben a traumatismos, cirugía, isquemia o infección.
El sistema ventricular suele estar aumentado de tamaño.
Puede haber remodelación ósea por la pulsación.
A moderate size cyst in the right parieto-occipito-temporal junction measuring 5 x 4 x 6 cm has a small communication with the occipital horn of the right lateral ventricle. It follows CSF signal in all sequences, with no diffusion restriction or enhancement. It is not lined by a grey matter and is associated with a small amount of adjacent FLAIR hyperintensity. It exerts mild mass effect on the adjacent parenchyma with slight remodelling of the overlying calvarium noted.
