Duties & Responsibilities: _____________________________________________________________________
• Serves as a liaison between patient, PCP, Insurance companies and specialty providers.
• Provides information related to referrals, short-term case work and case management services which included
client assessment, planning, patient education, advocacy and follow-up’s.
• Arranges client-confirmed services; Assist patient in scheduling specialist appointment (as needed); monitors
and documents client needs, progress and success.
• Responsible for expenses to accounts and cost centers by analyzing invoice/expense reports; recording
entries.
• Assess and document a client’s physical and mental abilities, financial, social and environmental needs, and
formulate an effective action plan.
• Identified and maintained contact with community service providers to inform families of benefits, programs
and utilized data entry systems to identify problems and develop solutions.
• Maintained and monitored data and referrals in a timely manner also developed and maintained systems for
organizing referrals and actively explore ways to improve quality of referrals.
• Answers all questions regarding complex referrals from Referral Clerks and communicates with them as
needed.
