 A person’s body temperature says a lot about their health. For
example,
 Body temperatureis thedegree of heatmaintainedby the
body or it is thebalance betweenheatproduced in the
tissues and heatlost to the environment.
 Measured in theunits of degrees.
 Expressed as C and F
 Normal value is 37degree C and 98.6 F
 Homeothermic
 HEAT PRODUCED–HEATLOSS = BODYTEMPERATURE
 THEBRAIN:
 Hypothalamus(heat lossand heat gain centre)is the control centre
for temperature. It detects temperature changes in the blood.
 SKIN:
 helps in regulating body temperature which hastiny holes or pores on
its outer surface. When metabolic rate increases sweat comes from
these pores and it is one way to maintainnormal body temperature.
 Heat is continually produced in the body as a by-product of the chemical
reactions calledmetabolism.
 Age : For new born, the temperature -control mechanisms are
immature. An infant' s temperature may respond drasticallyto changes
in the environment.
 Exercise: Muscle activitycausesincreased metabolism by increasing
carbohydrate and fat breakdown. Any form of exercise canincrease
heat production and thus body temperature.
 Hormonelevel: Women generally experience greater fluctuationsin
body temperature than men. Hormonal variations during menstrual
cycle causebody temperature fluctuation.eg, Menopause.
 Circadian rhythm: Body temperature normally changes 0.5� to
1� C during 24 hour period. The temperature is usuallylowest
between 1.00 AM and 4.00 AM.
 Stress :Physical and emotionalstress increases
body temperature throughhormonal and neuralstimulation.
These physiological changesincrease metabolism,which
increases heatproduction.
 Environment: Environmentinfluencesbody temperature.In
a very warm room, thebody temperaturewillbe elevated. In
a cold weather , thebody temperature may be low because of
extensive radiant and conductive heatloss.
 A condition in which thenormal regulation orexchange
between heatproduced by thebody and heatlost by the
body is disturbed in such a condition either body temperature
increases above thenormal or decreases below the
normal set of point.
Fever – (100.4F)
Hyperthermia – ABOVE 105F
Heat stroke – 104F AND HIGHER
Hypothermia – 95F AND BELOW
Frostbite – BELOW 92F
 Fever is defined as having a temperatureabove thenormal
rangedue to an increase in thebody's temperature set point.

 SEVERITY
 A fever can be:
 low grade, from 100.5–102.1°F or 38.1–39°C
 moderate, from 102.2–104.0°F or 39.1–40°C
 high, from 104.1–106.0°F to or 40.1-41.1°C
 hyperpyrexia, above 106.0°F or 41.1°C
 Length of time
 A fever canbe:
 acute if it lasts less than7 days
 sub-acute, if it lasts up to 14 days
 chronic or persistent, if it persists for over 14 days
 Fevers that existfor days or weeks with no explanation are called
fevers of undetermined origin (FUO).
 Sustained– constant body temperature
 Intermittent–once in 24 hrs
 Remittent–spikes and falls but withoutreturningto normal
limits
 Relapsing– fever and return to normal limits
 A virus
 A bacterial infection
 Allergies
 Heat exhaustion
 Certain inflammatory conditions such as rheumatoid arthritis
 A malignant tumor
 Some medications, such as antibiotics and drugs used to treat high blood
pressure or seizures or alcohol withdrawal.
 Some immunizations, such as the diphtheria, tetanus andacellular
pertussis (DTaP)or pneumococcal vaccine
 Headache,bodyache
 Vomiting
 Chills
 Sweating,shivering
 Tachycardia, palpitations
 Tachypnoea
 Weakness
 Loss of appetite
 Irritability
 Dehydration
 unconsciousness
 History collection
 Physical examination
 cbc/electrolytes/LFT/blood c/s / sputumc/s / urine analysis/
urine c/s/ ESR
 Chest radiograph
 Miscellaneous
 Acetaminophen every 6 hrs
 Ibuprofen every 6 hrs
 Naproxen (NSAID)
 Aspirin should not be used for fever in children and
adolescents especially during chicken pox (causereye’s
syndrome )
 Feverof unknown origin(FUO),refers to a condition in which fever of
at least 101F and that lastsfor about more than three weeks or occurs
frequently without explanation.
 CLASSIC:
 3 outpatientvisit or 3 days in hospital withoutelucidation of
a cause.
 NOSOCOMIAL:
 Fever develops on several occasions in a hospitalized patient
who is receiving acute care and in whom infectionwas not
manifestedon admission. 3 days of investigationand at least
2 days incubation of culture
 NEUTROPENIC:
 Neutrophilcount <500/ml
 3 days of investigation
 2 days of incubationof culture
 HIV ASSOCIATED FUO:
 >4 weeks for outpatientsor >3 days for hospitalized patients
 Approp. Investigationover 3 days and 2 days incubationof
culture.
 History collection and physical examination
 CBC/CRP/ESR/SERUM PROTEINELECTROPOROSIS/TIBC
total iron binding capacity /VDRL venereal disease related lab
test
 Vancomycin/pipercillin/tazobactum/imipenem/meropenem
 Hyperthermiais a condition where an individual's body temperature is
elevated beyond normal due to failedthermoregulation.
 Hyperthermia is a true medical emergency with rapid progression
to multiple organ system failure and death.
Hyperthermia is caused from overexertion or extended periods of
time spent in hot conditions.
A. HEAT STROKE
B. DRUG INDUCED HYPERTHERMIA
C. ENDOCRINOPATHY
D. CENTRAL NERVOUS SYSTEM DAMAGE
 also known as sun stroke, is atype of severe heat illness that results
in a body temperature greater than 40.0 °C (104.0 °F) or prolonged
exposure to sun or high environmental temperature.
 HEAT EXHAUTION:
 Heat exhaustion isa condition whose symptoms may include heavy
sweating and a rapid pulse, a result of your body overheating.
 Heatstroke can be divided into two forms;
 Classicheatstroke or (nonexertional) - It develops over a period of
days (usuallycoinciding with a heat wave) and typically presents with
nausea,vomiting, headache, and a deteriorating mental status.
 Exertionalheatstroke- usuallydevelops rapidly in ayoung, vigorously
exercising individual who have not acclimatized to a hot environment
leads to severe headache to seizures and collapse
B. DRUG INDUCED HYPERTHERMIA:
Due to increased use of psychotic
drugs
C. ENDOCRINOPATHY:
Thyrotoxicosis and pheochromocytoma
can lead to increases thermogenesis
D. CENTRAL NERVOUS SYSTEM DAMAGE:
Cerebral hemorrhage, statusepileptics, hypothalamic injury.
 ASSESS FOR Rectal or core temperatures greater than104
degrees;
 hypotension; mentalchanges,
 severe headache,ataxia,seizures, and coma;
 tachycardia; tachypnea;
 and profuse or absent (usually) sweating can all be seen.
 In severe heatstrokedisseminatedintravascular coagulation
may occur.
 removal from the heat, clothing removedand if possible should beimmersed in
cool water.
 If immersion is impossible the patient should have icepacks applied to the groin,
axilla, and neck area
 patient should befanned. Fanning the patient while sprinkling with watermay
increase evaporative losses until the patient's temperature is below 39 C (102.2 F).
 Massaging the extremities may behelpful.
 These patients are severely dehydrated and should be bolused with 20 mg/kg of
NS. Further replacement of fluids and monitoring should reflect the patient's
condition.
 Heat syncope:This is characterized by sudden dizziness in hot weather.
 Heat rash:The body physically manifests red bumps and the skin
becomes itchy and irritated.
 Heat cramps:The muscles tighten in the stomach, arms, and legs as a
result of elevated temperatures. At this point, it's time to drink water, rest,
and try to cool off.
 Heat edema:The ankles and feet swell due to the body retaining fluids.
Try to elevate the legs to relieve symptoms.
 muscleCramps, fatigue,dizziness, headache,nausea,vomiting,
andweakness.
 The heart rate may be elevated, and the skinis reddened.
 The skinmay be moist ifsweating is still occurring, or it may be dry if
sweating hasstopped.
 Confusion and mental changes may develop, and seizures can occur
with brain damage.
 Ultimately, coma and death may ensue.

 Over the counter medications

 Iv medications
 Monitor continuous body temperature
 Assess for skincolor
 Provide adequate nutrition
 Monitor intakeand output chart
 Reduce physical activity
 Tepid sponging
 Remove excess blankets
 Perform hygienic practices
 Administer medications
 INTERNALCOOLING TECHNIQUES:Ice water gastric or rectal lavage
associated with complications.
 EXTERNALCOOLING TECHNIQUES:external application
 CONDUCTIVECOOLING TECHNIQUES:hypothermic blanket, ice bath
or ice packs to neck, axillaand groin.
 CONNECTIVECOOLING TECHNIQUES:use of fans and air conditioning
 EVAPORATIVECOOLING TECHNIQUE:misting the skin with tepid water
or apply a single layer to bare skin.
 Hypothermia is a medical emergency that occurs when your body
loses heat faster than it can produce heat, causinga dangerously low
body temperature, occurs asyour body temperature fallsbelow 95 F
(35 C).
 your heart, nervous system and other organs can't work normally.
 can eventually lead to complete failure of your heart and respiratory
system and eventually to death.
 Hypothermia is often caused by exposure to cold weather or
immersion in cold water.
 Exposure to cold environment
 Occupational exposure
eg. Hunters,sailors,climbers
 Neurologic injury
 Sepsis
 Endocrine dysfunction
 Medications likeethanol,barbiturates,benzodiazepines,
antidepressentsanaesthetics.
 FIRST STAGE: Shivering, reduced circulation
 SECOND STAGE: Slow, weak pulse, slowed breathing, lack of
coordination, irritability, confusionand sleepy behavior
 ADVANCED STAGE: Slow, weak or absentrespiration and
pulse theperson may lose consciousness.
 Rectal temperatureshould be monitored
 Lab: glucose testingto rule out hypoglycemia
 Electrolyte levels
 Potassiumlevels
 Coagulationfactors
 Arterial blood gas
 Whiteblood cellcount
 Passive rewarming: cover with heatedblankets
 Blood rewarming: blood may be drawn warmed and
recirculated in thebody (hemodialysis machine)
 Warm IV fluids: a warmed iv solutionof salt water putin to
a vein help warm theblood
 Airway rewarming: use of humidifiedoxygen will raise the
body temperature
 Irrigation: a warm salt water is introduced into theaffected
parts of the body (pleura, peritoneal cavity)
 Thiamineadministration
 Wet clothing should be removed
 Excessive movementandnasogastrictube should be avoided.
 Intravenous medications should be withhelduntilthepatient
is warmed.
 Use of amidarone is needed.
 Frostbite is damage to skinand tissuecaused by exposure to freezing
temperatures – typicallyand temperature below -0.55C (91F)
 Frostbite can affect any parts of the body but the extremities such as
hands, feet, ears, nose and lips are most likelyto be affected.
 Frostnip. Frostnip is a mild form of frostbite. As
your skin warms, you may feel pain and tingling. Frostnip doesn't permanently
damage the skin.
 Superficial frostbite. Superficial frostbite appears as reddened skin that turns
white or pale.Your skin may begin to feel warm at this stage, the surface of your
skin may appear mottled.
 Deep (severe) frostbite. As frostbite progresses, it affects all layers of the skin,
including the tissues that lie below. Your skin turns white or bluish gray and losing
all sensation of cold, pain or discomfort in the affected area. Joints ormuscles may
no longer work. Large blisters form 24 to 48 hours after rewarming. Afterward, the
area turns black and hardas the tissue dies.

 Signs and symptoms of frostbite include:
 At first, cold skin and a prickling feeling
 Numbness
 Red, white, bluish-whiteor grayish-yellow skin
 Hard or waxy-looking skin
 Clumsinessdue to joint and muscle stiffness
 Blistering afterrewarming, in severe cases
 Frostbite is most common on thefingers, toes, nose, ears,
cheeks and chin.
 Increased sensitivity to cold
 Long-term numbness in the affected area
 Excessivesweating (hyperhidrosis)
 Changes in skin color
 Changes in or loss of nails
 Joint stiffness (frostbite arthritis)
 Growth defects in children, if frostbite damages a bone's growth plate
 Infection
 Gangrene
 Tetanus
 Appearance of the skin
 Rapid rewarmingof injured tissue
 Topical antimicrobial cream application
 Antibiotics
 Painmanagement.
 Wound care.
 Surgery - Removal of damaged tissue(debridement)
 Monitoring of digits to evaluateperfusion untilamputationof
theaffectedareas.

  • 5.  A person’s body temperature says a lot about their health. For example,
  • 6.  Body temperatureis thedegree of heatmaintainedby the body or it is thebalance betweenheatproduced in the tissues and heatlost to the environment.  Measured in theunits of degrees.  Expressed as C and F  Normal value is 37degree C and 98.6 F  Homeothermic

  • 10.  THEBRAIN:  Hypothalamus(heat lossand heat gain centre)is the control centre for temperature. It detects temperature changes in the blood.  SKIN:  helps in regulating body temperature which hastiny holes or pores on its outer surface. When metabolic rate increases sweat comes from these pores and it is one way to maintainnormal body temperature.

  • 15.  Heat is continually produced in the body as a by-product of the chemical reactions calledmetabolism.

  • 17.  Age : For new born, the temperature -control mechanisms are immature. An infant' s temperature may respond drasticallyto changes in the environment.  Exercise: Muscle activitycausesincreased metabolism by increasing carbohydrate and fat breakdown. Any form of exercise canincrease heat production and thus body temperature.
  • 18.  Hormonelevel: Women generally experience greater fluctuationsin body temperature than men. Hormonal variations during menstrual cycle causebody temperature fluctuation.eg, Menopause.  Circadian rhythm: Body temperature normally changes 0.5� to 1� C during 24 hour period. The temperature is usuallylowest between 1.00 AM and 4.00 AM.
  • 19.  Stress :Physical and emotionalstress increases body temperature throughhormonal and neuralstimulation. These physiological changesincrease metabolism,which increases heatproduction.  Environment: Environmentinfluencesbody temperature.In a very warm room, thebody temperaturewillbe elevated. In a cold weather , thebody temperature may be low because of extensive radiant and conductive heatloss.

  • 25.  Length of time  A fever canbe:  acute if it lasts less than7 days  sub-acute, if it lasts up to 14 days  chronic or persistent, if it persists for over 14 days  Fevers that existfor days or weeks with no explanation are called fevers of undetermined origin (FUO).

  • 31.  Acetaminophen every 6 hrs  Ibuprofen every 6 hrs  Naproxen (NSAID)  Aspirin should not be used for fever in children and adolescents especially during chicken pox (causereye’s syndrome )
  • 32.  Feverof unknown origin(FUO),refers to a condition in which fever of at least 101F and that lastsfor about more than three weeks or occurs frequently without explanation.

  • 42.  Heatstroke can be divided into two forms;  Classicheatstroke or (nonexertional) - It develops over a period of days (usuallycoinciding with a heat wave) and typically presents with nausea,vomiting, headache, and a deteriorating mental status.  Exertionalheatstroke- usuallydevelops rapidly in ayoung, vigorously exercising individual who have not acclimatized to a hot environment leads to severe headache to seizures and collapse

  • 45.  ASSESS FOR Rectal or core temperatures greater than104 degrees;  hypotension; mentalchanges,  severe headache,ataxia,seizures, and coma;  tachycardia; tachypnea;  and profuse or absent (usually) sweating can all be seen.  In severe heatstrokedisseminatedintravascular coagulation may occur.
  • 46.  removal from the heat, clothing removedand if possible should beimmersed in cool water.  If immersion is impossible the patient should have icepacks applied to the groin, axilla, and neck area  patient should befanned. Fanning the patient while sprinkling with watermay increase evaporative losses until the patient's temperature is below 39 C (102.2 F).  Massaging the extremities may behelpful.  These patients are severely dehydrated and should be bolused with 20 mg/kg of NS. Further replacement of fluids and monitoring should reflect the patient's condition.
  • 47.  Heat syncope:This is characterized by sudden dizziness in hot weather.  Heat rash:The body physically manifests red bumps and the skin becomes itchy and irritated.  Heat cramps:The muscles tighten in the stomach, arms, and legs as a result of elevated temperatures. At this point, it's time to drink water, rest, and try to cool off.  Heat edema:The ankles and feet swell due to the body retaining fluids. Try to elevate the legs to relieve symptoms.
  • 48.  muscleCramps, fatigue,dizziness, headache,nausea,vomiting, andweakness.  The heart rate may be elevated, and the skinis reddened.  The skinmay be moist ifsweating is still occurring, or it may be dry if sweating hasstopped.  Confusion and mental changes may develop, and seizures can occur with brain damage.  Ultimately, coma and death may ensue.

  • 52.  INTERNALCOOLING TECHNIQUES:Ice water gastric or rectal lavage associated with complications.  EXTERNALCOOLING TECHNIQUES:external application  CONDUCTIVECOOLING TECHNIQUES:hypothermic blanket, ice bath or ice packs to neck, axillaand groin.  CONNECTIVECOOLING TECHNIQUES:use of fans and air conditioning  EVAPORATIVECOOLING TECHNIQUE:misting the skin with tepid water or apply a single layer to bare skin.
  • 53.  Hypothermia is a medical emergency that occurs when your body loses heat faster than it can produce heat, causinga dangerously low body temperature, occurs asyour body temperature fallsbelow 95 F (35 C).  your heart, nervous system and other organs can't work normally.  can eventually lead to complete failure of your heart and respiratory system and eventually to death.  Hypothermia is often caused by exposure to cold weather or immersion in cold water.

  • 57.  Passive rewarming: cover with heatedblankets  Blood rewarming: blood may be drawn warmed and recirculated in thebody (hemodialysis machine)  Warm IV fluids: a warmed iv solutionof salt water putin to a vein help warm theblood  Airway rewarming: use of humidifiedoxygen will raise the body temperature  Irrigation: a warm salt water is introduced into theaffected parts of the body (pleura, peritoneal cavity)
  • 58.  Thiamineadministration  Wet clothing should be removed  Excessive movementandnasogastrictube should be avoided.  Intravenous medications should be withhelduntilthepatient is warmed.  Use of amidarone is needed.
  • 59.  Frostbite is damage to skinand tissuecaused by exposure to freezing temperatures – typicallyand temperature below -0.55C (91F)  Frostbite can affect any parts of the body but the extremities such as hands, feet, ears, nose and lips are most likelyto be affected.
  • 60.  Frostnip. Frostnip is a mild form of frostbite. As your skin warms, you may feel pain and tingling. Frostnip doesn't permanently damage the skin.  Superficial frostbite. Superficial frostbite appears as reddened skin that turns white or pale.Your skin may begin to feel warm at this stage, the surface of your skin may appear mottled.  Deep (severe) frostbite. As frostbite progresses, it affects all layers of the skin, including the tissues that lie below. Your skin turns white or bluish gray and losing all sensation of cold, pain or discomfort in the affected area. Joints ormuscles may no longer work. Large blisters form 24 to 48 hours after rewarming. Afterward, the area turns black and hardas the tissue dies.
