Meeting the objectives of Meaningful Use is important for complying with government initiatives, but it is equally important for the practice to get the most practical use for their investment.

To register you will be required to go to
the CMS website.
If you are an EP, you must have an active
National Provider Identifier (NPI) and have
a National Plan and Provider Enumeration
System (NPPES) web user account. Use
your NPPES User ID and Password to log
into this system.
3. Measure 1
More than 30 percent of all unique
patients with active medications, seen by
the EP, have at least one medication order
entered using CPOE.
Measure 1 is achieved by using
ePrescribing within eMedRec.

4. Measure 2
Implement drug-drug and drug-allergy
checks.
Measure 2 is achieved by using

5. Measure 3
More than 80 percent of all unique
patients seen by the EP have at least one
entry (or an indication that no problems
are known) for the patient recorded as
structured data.

patient Problem List within eMedRec.

6. Measure 4
More than 40 percent of all permissible
prescriptions written by the EP are
transmitted electronically using certified
EHR technology.

7. Measure 5
More than 80 percent of all unique patients seen
by the EP have at least one entry (or an
indication that the patient is not currently
prescribed any medication) recorded as
structured data.
Measure 5 is achieved by populating the patient
Medication List within eMedRec. This is most
efficiently achieved within the ePrescribing
module.

8. Measure 6
More than 80 percent of all unique patients seen by the
EP have at least one entry (or an indication that the
patient has no known medication allergies) recorded as
structured data.
Version 7 of eMedRec includes a grid for adding
Allergies to medications or other items directly in the
Medication List via drop down lists. (The prior versions
had allergies written free form in a text field.) The
Allergies section of the Medication List will need to be
updated for ALL patients.

9. Measure 7
Record ALL of the following demographics:
(A) Preferred language
(B) Gender
(C) Race
(D) Ethnicity
(E) Date of birth
More than 50 percent of all unique patients seen by the
EP have demographics recorded as structured data.
Each of the demographic items may be recorded in the
Patient Add/Edit screen.

10. Measure 8
Record and chart changes in the following vital signs:
(A) Height
(B) Weight
(C) Blood pressure
(D) Calculate and display body mass index (BMI)
(E) Plot and display growth charts for children 2-20
years, including BMI
for more than 50 percent of all unique patients age 2 and
over.
Each of these Vital Signs are recorded and calculated in
the patient Vitals List within eMedRec.

11. Measure 9
More than 50 percent of all unique patients 13
years old or older, seen by the EP, have
smoking status recorded as structured data.
Record the Smoking status in the Patient Social
History by Editing the list and selecting a Status
from the drop down.

12. Measure 10
Successfully report to CMS ambulatory clinical
quality measures, selected by CMS, in the
manner specified by CMS.
Reporting functionality is included in eMedRec
V7 to report the rates of compliance with various
measures.

13. Measure 11
Implement one clinical decision support rule
relevant to specialty or high clinical priority along
with the ability to track compliance with that rule.
Compliance with this measure may be achieved
by utilizing the Health Maintenance tool within
eMedRec (configured for the practice specialty,
if applicable).
Attest Yes if Health Maintenance is
implemented.
14. Measure 12
Provide patients with an electronic copy of their health
information (including diagnostic test results, problem
list, medication lists, medication allergies) upon request.
More than 50 percent of all patients who request an
electronic copy of their health information are provided it
within 3 business days.
Electronic health record reports may be created in
industry standard CCR (Continuity of Care Record) using
the CCR tool.
If the practice has had no patient requests for electronic
copies of their charts they are excluded from the
measure.
15. Measure 13
Provide clinical summaries to patients for each office
visit. Clinical summaries are to be provided to patients,
for more than 50 percent of all office visits, within 3
business days.
The patient needs to be provided with a copy of the
Office Visit Note, created within eMedRec, after the
patient visit. A Clinical Summary Note or Form will be
created for use by the practice.

16. Measure 14
Capability to exchange key clinical information
(this information includes: problem list;
medication list; medication allergies; and
diagnostic test results), among providers of care
and patient authorized entities electronically.
Use of the CCR Import and Export feature,
described above, will enable this function.
This is a Yes/No question on the attestation
which will be answered Yes.
17. Measure 15
Protect electronic health information created or
maintained by the certified EHR technology through the
implementation of appropriate technical capabilities.
Conduct or review a security risk analysis in accordance
with the requirements under 45 CFR 164.308(a)(1) and
implement security updates as necessary and correct
identified security deficiencies as part of its risk
management process.
Holt Systems will conduct a security risk analysis with
the practice IT team to assure compliance.
