We develop and sustain meaningful interactive relationships with varied communities (that is,
patients, senior citizens, veterans, those with disabilities, education, civic, and business groups),
so that all members understand and contribute to the work of supporting our communities.

Bring communities together, supporting health
and wellbeing, enabling communities to access
health information, providing care and support,
and empowering individuals to connect and
make a positive difference to their lives.
Target audience
Simple answer our target audience is vulnerable populations.
Which includes the economically disadvantaged, racial and ethnic
minorities, the uninsured, low-income children, the elderly, the
houseless, diasabled & returning citizens, those with human
immunodeficiency virus (HIV), and those with other chronic
health conditions, including severe mental illness.
Proposed solution
● Serve, support, sustain a shared sense of health and wellness in all interest groups
of the community
● Maintain the Health is wealth approach. Healthier communities become resilient
when they fully adopt these types of care models in their relationships across all
segments
● Speak to various insecurities such; Food, Housing, Medicine, Transportation
Determine barriers and resources associated with addressing the problem
● Develop the best action steps for addressing the problem
● Civic Fusion: Maintain a network of community partners that help, or are helping,
address community care
● Package resources for stakeholders in the community to utilize as their own
Deliverables
1.Covid 19 resources and preparedness kit.
2. Pharmacy that delivers, discount copay program and $ 15.00 Kroger gift card
for eligible patients.
3. Monthly food cards if eligible
4. Transportation to any medical facility or provider within a 50 mile radius (
varies with plan).
5. Hygiene products
6. Paper products toilet paper, Kleenex and paper towels.
7. Veteran resources housing and medical
