1 HSE Responsibilities
According to Company HSE policy, Management is commitment to work safely I all
on going operations. In this regards there is some responsibilities of line managers,
supervisor, employees, visitor and contractors, Personnel, as referred to in this handbook,
include SZS Managers, Supervisors, Foremen, Employees, and contractors have the
following safety responsibilities:
2. Manager / Supervisor Responsibilities
a. Establish and communicate safety rules and standards to all employees and
contractors.
b. Provide appropriate Information, instruction, resources; complete inspected
tools, and adequate training for employees.
c. Award contracts using safety, health, and environmental criteria.
d. Conduct and document safety meetings.
e. Identify & correct unsafe acts and conditions promptly.
f. Conduct routine / non routine safety and environmental inspections and tours.
g. Maintain facility emergency plans and conduct regular emergency drills.
h. Investigate injuries and other incidents and promptly provide corrective
actions.
i. Coordinate with HSE Dept for non-routine activity at least 48 hours before to
develop the control measure to prevent any incident.
3. Employee Responsibilities
a. Comply with applicable rules, standards, and safe work practices.
b. Communicate safety rules and standards to contractors and coworkers.
c. Maintain knowledge of safety requirements, including emergency response
action.
d. Participate in safety meetings.
e. Operate, inspect, and maintain facilities in a safe condition.
f. Report injuries, unsafe conditions, near misses, and incidents immediately.
g. Cooperate & assist in incident investigations as necessary.
4. Contractor Responsibilities
a. Comply with all SZS rules, policies, and procedures.
b. Report injuries, spills, unsafe acts and conditions, near misses, and incidents

immediately.
c. Hold pre-job meetings and other safety meetings during the job.
d. Assist in incident investigations as necessary.
5. Report Incidents
a. Immediately report all work related injuries/illnesses or vehicle collisions,
no matter how slight, to HSE Dept.
b. Immediately report all fires, spills.
c. Immediately report any unsafe condition, practice, near miss, or incident to your
supervisor and HSE Department.
6. Follow Safe Practices
1. According to safe work practice, always develop safe work method, identify the
hazard of related activities, and develop the control measure to prevent incident
and injuries, perform activities as per approved plan and after any changes in
work procedure review the job safety analysis.

2. Erect barricades/flagging around hazardous work areas, such as holes in
decking and floor surfaces, trenches, road crossings, and overhead hazardous
work.
3. Use only proper tools and equipment maintained in a safe working condition.
Homemade tools/ incomplete tools are prohibited.
4. Maintain good housekeeping in your work area at all times.
5. Smoking is permitted in designated areas only. Matches and lighters are
prohibited in restricted areas.
6. Eating or resting is permitted in designated areas only.
7. Use proper manual lifting techniques, or obtain assistance or mechanical lifting
aids when lifting loads.
8. All operating machinery ( e.g Grinder, Drill machine, ) and electrical switchgear
shall have all safety guards, switches, and alarms in place and functional.
9. Whenever a safety device is removed from service and/or defeated, the
appropriate supervisor and affected parties shall be notified, the device tagged,
and the action properly documented.
10. All chemical or hazardous material containers shall be properly labeled and
stored. Drums shall be stored in secondary containment areas or on drum
containment pallets.
11. Operators of vehicles shall comply with all traffic requirements, particularly speed
limits
12. All persons in vehicles shall wear seat belts at all times.
13. When ascending or descending stairways, use the handrail and take only one
step at a time.
7. Avoid Unsafe Activities
1. Climbing or standing on equipment, an unstable surface to perform work is
prohibited.
2. Approved fall protection measures (e.g. safety harnesses or scaffolding) shall
be used if the working height is greater than 1.8 m (6 ft) from platform level.
3. Running in work areas is prohibited, except during emergencies.
4. Finger rings, wristwatches, jewelry, loose clothing, unsecured long hair, and other
loose accessories shall not be worn within an arm's reach of rotating equipment or
electrical switchgear.
5. Do not apply compressed air or other gases to yourself or others.
6. Non routine activity is prohibited in any area or on any equipment without
consent of the SZS HSE Department.
7. Horseplay or fighting in SZS premises is prohibited.

8. Protect Yourself .
1. Approved hard hats without defects shall be worn in designated areas. Metallic
hard hats are prohibited.
2. Approved safety footwear shall be worn in field operations, process areas, and
other designated areas.
3. Approved safety eyewear with side protection shall be worn where designated.
4. Approved additional hazard-specific eye/face protection (e.g., goggles and/or a
face shield) shall be worn where foreign objects may injure the eye or face (e.g.,
grinding, welding, drilling, or scraping).
5. Proper personal protective equipment (PPE) prescribed by the Chemical Hazard
Bulletin (CHB) or Material Safety Data Sheet (MSDS) shall be worn when
handling chemicals or hazardous materials.
6. Approved hearing protection shall be worn in high noise areas (i.e., 85 decibels
[dBA] or higher) e.g hammering activity inside the containers, Generator areas.
7. Proper hand protection (e.g., gloves, latex coated gloves) shall be worn when
performing tasks that may present a hand injury risk.

9. General Safety Precautions for Hot Work activity.
______________________________________________
a. All Operators and Helpers of Grinding, cutting, welding machine shall
be experienced and certified.
b. All Operator of Portable Tool and helpers shall use proper PPE according to Job
activity.
c. All equipment shall be visually inspected by the user daily and Site Supervisor
and mark Inspected Tags.
d. Site Supervisor is responsible for Defective equipment shall be removed and
replaced from site.
e. All welding, cutting and grinding operations shall have proper ventilation.
f. All unsecured work pieces shall be properly clamped.
g. All equipment shall be turned off when not in use.
h. A portable fire extinguisher shall be available within 3 m (10 ft) Combustibles
around the Hot work area shall be removed / protected.
i. A Trained fire watch shall be maintained during operations and for 30 minutes
afterwards due to re-ignite hazard.
j. Grinding, Cutting, welding, are not allowed in O2 enriched environments (i.e.,
O2 concentration greater than 23.5%).

10. Cutting, Grinding Safety Rules
Further refer to "Hot Work Safety" section.
a. Grinding Machine and abrasive wheels should always be used in accordance
with the manufacturer instructions.
b. Always ensure the maximum speed marked on
the abrasive wheel in greater than rated speed of
the grinder.
c. Never use cutting disc to grinding and grinding
disc for cutting job
d. Safety Guard must cover 50 % Grinding Wheel.
Keep guard between you face and Wheel.
e. Do not use the grinding wheels that are larges
then the maximum recommended size, or worn
down wheels from other grinders.
f. Grinding wheel must be inspected (No crack,
edge break etc) before use for operation.
g. Never use the grinder wheel power tool without
wheel safety guard.

h. Power tool electrical outlets shall be the 3-
pronged type with ground fault protection
device.
i. Wear all required PPEs such as transparent
face shield, hearing protection (Ear Plug) as
well as safety gloves, Coverall (avoid the
loose dress due to entanglement hazard)
and safety Shoes and perform safe activity.
j. Run newly mounted discs at operating speed
one minutes before operations

11. Electric Arc Welding Operations
Further refer to "Electrical Safety" section.
a. Electric welding machines shall be properly bonded and grounded and shall
be inspected.
b. Electric welding machine electrical outlets shall be the 3-pronged type with
ground fault protection device.
c. Welding cables shall be continuous within 3 m (10 ft) of the electrode holder.
d. All damaged welding cables shall be removed from service; temporary repairs
and taped joints are prohibited.
e. Proper welding screens shall be used near other personnel.
f. GFCI (Ground fault circuit interrupted Device must be installed with all
electrical equipment to prevent electrical shock.
g. Employees must be in complete PPEs such as Welding shield, leather
gloves, slaves, cotton/ jeans Base cover all, safety shoes. etc.
12. Gas Welding and Gas Cutting, Operations
Further refer to “Compressed Gas Cylinders" section.
a. Lighted torches shall not be left unattended.
b. Compressed type fittings shall be utilized on hose connections.
c. All connections/equipment shall be checked for leaks before use (e.g., soap
solution).
d. Torch must be select as per Job requirement.

e. Flashback arrestors shall be placed at O2/acetylene cylinder regulators as
shown below in Figure

h. Only strikers shall be used to light a torch; never light a torch with matches or a
lighter.
i. Gas regulators shall be turned off and hoses bled off at the end of shift or task.
j. Hoses shall be protected from potential damage during operations.
k. Ensure 15 feet distance between both cylinder during working.

l. When opening an O2 or fuel cylinder valve, always crack it open first.
m. Stand to the side of a regulator, not in front of it when opening cylinder valves.
n. Acetylene cylinders in use shall have a handle or valve wrench in place at all
times.
o. During operation acetylene gas pressure must be less than 15 PSI.
p. All valve and hose must be hydro test certified.
q. All kind of cylinders must be placed vertical secure position during activity.

13. Chemical Safety
_______________________________________________
a. Labeling of Chemicals
1. Chemicals shall be properly labeled.
2. Chemical Hazard Bulletins (CHBs) or Material Safety Data Sheets (MSDSs)
shall be readily available for each chemical on-site.
b. Use of Chemicals
1. Use appropriate PPE/respiratory equipment as set out in the
relevant MSDS/CHB.
2. Do not smell or taste chemicals or eat, drink,
3. smoke, or chew gum in areas where chemicals are
present.
4. Wash hands thoroughly after handling chemicals.
5. Do not mix incompatible chemicals.
6. When mixing chemicals, use proper lab equipment.
7. In case of eye or skin contact, promptly flush affected
areas with clean water for a prolonged period (15
minutes minimum), remove contaminated clothing, and
seek medical attention.
8. Promptly clean spills while wearing appropriate PPE, and
properly dispose of all contaminated materials.
c. Storing Chemicals
1. Indoor storage of flammable and combustible materials
shall be in approved metal cabinets.
2. Users shall periodically examine chemical containers for
integrity.
3. Chemical storage shall be minimized. Avoid the storage on
bench tops or at high levels
4. Incompatible chemicals shall not be stored near each
other.
5. Toxic substances shall be segregated in a well-identified area with local
exhaust ventilation.
6. Highly toxic chemicals, whose containers have been opened, shall be in
secondary containment.

14. Compress Gas cylinders Safety

a. Handling Cylinders
1. Secure protective caps on all cylinders not in use.
2. Properly secure cylinders in an upright position during movement.
3. Do not use slings or ropes for moving cylinders.
4. All cylinders shall be properly labeled.
5. Do not strike cylinders together.
6. Keep valves on empty cylinders closed at all times.
7. Use cylinder trolleys, material baskets, cylinder racks, and other
proper types of equipment to transport cylinders.
b. Using Cylinders
1. Use approved pressure-reducing regulators with a check valve connected to
the cylinder valve on all cylinders.
2. Always close the cylinder valve before attempting to stop leaks.
3. Do not use oil or grease as a lubricant on valves or attachments.
4. Physical Inspect cylinders for safe condition before use.
5. Protect cylinders from direct flame, sunlight, and other heat sources.
6. Inspect all gauges, regulators, hoses, and cylinders for damage and current
hydro test date.
7. Place flashback arrestors at O2/acetylene cylinder regulators
8. (See "Gas Cutting, Gas welding” section).
c. Storing Cylinders
1. Store cylinders in an upright, secured position; use bottle/
cylinder racks wherever possible.
2. Keep stored O2 cylinders separated from fuel gas cylinders by
(20 ft), noncombustible barrier.
3. Keep cylinders from direct flame, sunlight, and other heat
sources.
4. Do not store cylinders at temperatures exceeding 54 °C (130 °F).
5. Properly identify empty and full cylinders.

15. Crane Lifting Safety
_______________________________________________
This section applies to all types of cranes including mobile and fixed.
a. Inspection
1. All contractor mobile or fixed cranes shall be inspected by the third party prior to
use within SZS premises.
2. All cranes shall have a valid inspection sticker and certificate from approval
authority. Extra lifting equipment used below the hook (e.g., spreader bars,
crane suspended personnel platforms, and man baskets) shall have a valid
inspection from a certified inspector of third party.
b. Operations
1. Details of crane lifts shall be communicated to the affected
personnel in the area.
2. A designated signal man and crane operator shall use
universal hand and/or radio signals.
3. The crane operator's line of sight to the designated signal man
shall not be obstructed.
4. Tag-lines shall be used on all lifts, except when their use may
create a greater hazard.
5. Know the accurate weight of all loads and lifting gear prior
to the lift. Also follow the crane load chart.
6. Avoid load swings over personnel or traffic areas.

7. In positioned lifts, outriggers shall be used with
appropriate pads and mats. Booms shall not be
swung without outriggers being extended in
accordance with the manufacturer's specification.
8. Crane operator shall not leave the crane cab while
the crane is attached to a load.
9. Riggers must be certified and will inspect slings,
fittings, and shackles prior to use.
10. All crane loads shall be rigged by a certified rigger.
11. Crane lifts shall not be allowed at wind speeds
above 32 km/h (20 mph) unless otherwise specified
by the manufacturer.
12. Crane suspended man basket operations shall not be allowed at wind
speeds greater than 25 km/h (15 mph).
13. Cranes shall not be operated any closer than the depth of an excavation.
14. Cranes shall be operated around power lines in accordance with the
distances provided in the "Electrical Safety" section.
9. Crane not placed on wet ground.
10. Critical Lift Plans are required for certain conditions and activities including:
a. Cranes operating around power lines.
b. All loads of 40 tons or greater. All loads exceeding 85% of the rated load
capacity of the crane for that specific lift.
c. Tandem, multiple, or tailing lifts (any lift requiring two or more cranes to lift
one load).
d. Night time and during rain crane lifts is prohibited
e. Follow the load chart by the crane manufacturer
f. Grounding facility must be installed with crane at the time of lifting near
electrical line to avoid any kind of electrical shock.

c. Rigging Practices
1. Personnel shall use accepted rigging
techniques.
2. All crane loads shall be rigged by certified
rigger.
3. All rigging equipment shall be rated for the
load being lifted.
4. Avoid sharp bends in slings; protect slings
from sharp edges and abrasions.
5. Do not stand or walk under suspended
loads.
6. Do not leave suspended loads unattended,
unless the load is properly secured to a load
bearing structural member.
7. Shackles and other connecting devices shall
be completely closed/bolted.
d. Inspection
1. Rigger /Users shall visually inspect all rigging hardware before each use.
2. Rigger/users shall properly maintain all rigging hardware. All defective
components shall be removed from service.
3. Slings shall have their rated capacity indicated on a tag or stamp.

16. Electrical Safety
_______________________________________________
a. Qualifications
1. Only individuals qualified for the type of electrical work to be undertaken shall
repair or install electrical equipment, or work around live circuits.
2. Individuals who need to identify and possibly de-energize an electrical circuit
shall attend electrical hazard recognition training.
b. Work Practices
1. All electrical conductors shall be considered energized unless properly Locked,
Tagged, Cleared, and Tried.
2. De-energize all circuits before beginning work. Use Safety` "Isolation,
Lockout, and Use of Hold Tags" to prevent the electrical circuits from being
inadvertently energized.
3. Use proper PPE including insulated rubber gloves for high voltage work.
Additionally, mats and blankets may be required to provide insulation from
other elements that are energized or grounded.
4. Personnel shall not wear rings, wristwatches, other jewelry, glasses with metal
frames, or other similar metallic objects while working within arm's length of
energized electrical equipment.
5. Metal ladders shall not be used when working on or near electrical
equipment or conductors; use only nonconductive ladders.
6. Never use defective electrical equipment, defective extension cords, or
shop-made extension cords.
7. When opening or closing a disconnect switch, personnel shall stand to the
side, turn head away from switch, and throw switch with a quick upward or
downward single motion.
c. Electrical Lockout and Tag Procedure
1. The following procedures for electrical lockout and tag (Lock, Tag, Clear, Try)
shall be followed when exposed to electrical hazards:
2. Operations shall identify isolating locations and types of Isolating devices
required in determining potential exposure.
3. Qualified personnel doing the work shall notify affected personnel and properly
shut down/de-energize the equipment.

4. Isolation is complete only when no associated control device is capable of
energizing equipment.
5. SZS operations employees or the controlling organization shall always be the
first to install an approved Lock.
6. The person doing the work shall lock out a device using his approved Lock.
Remember to lock open the circuit breaker(s).
7. If the device cannot be physically locked out, it shall, at a minimum, be de-
energized and tagged.
8. Tag the lock with a "DANGER, DO NOT OPERATE" tag. The tag shall contain
date, time, organization, name, badge number, equipment name, phone contact
and signature of the installer, and reason for installation.
9. Other organizations or individuals working on equipment shall also install their
locks and tags at locations identified by operations. Multiple lockout clips
(hasps) shall be used if necessary. A crew lockout is acceptable only when the
key(s) of the person designated to lock out is properly secured and locked by
all parties involved.
10. Each lock shall be keyed separately with no duplicate key available to
ensure removal only by the installer.
11. Clear the area of personnel and tools prior to trying to start the equipment.
12. Before starting work, Try to start or energize the equipment locally. Verify
the equipment cannot be energized.
13. Only the person(s) originally attaching the lock and tag are authorized to remove
the lock and tag. Incoming shifts may attach their own locks and tags and/or
transfer keys after approval by supervision. If person(s) are unavailable, the
Division Head can assume responsibility for removing the lock and tag and
notifying all parties.
14. SZS operations employees shall remove locks and tags when equipment is safe
to energize.
15. When work is complete, affected personnel shall be notified and equipment
placed back in service.

17. Fall Protection Safety
_______________________________________________
Approved fall protection measures (e.g., personal fall arrest systems and scaffolding) shall be
used if the working height is greater than 1.8 m (6 ft) from floor or platform level.
a. Personal Fall Arrest Systems
1. Fall arrest system components shall be properly rated, mutually compatible, and
consist of the following:

3. Users shall inspect fall arrest systems prior to each use.
4. Remove defective components from service.
5. Fall arrest system components shall be protected against cuts or abrasions.
6. Fall arrest systems and components shall not be used to hoist materials.

7. When using full-body harnesses with shock-absorbing lanyards, they shall be
properly secured at least 5.6 m (18 ft) above impact surface and shall not be
reused after stopping a fall.
8. Waist belts and manual locking or non-self-locking snap hooks are prohibited for
use as part of a fall arrest system.
9. Life line must be installed where is not possible to installed guard rail or other fall
protection system which is able to carry 5000 pound weight.
b. Fixed Elevated Work Surfaces
1. Personnel shall inspect all fixed elevated work
surfaces prior to use (i.e., gratings clips).
2. Standing on a guardrail or ladder in close proximity
to the edge of an elevated platform is prohibited.
3. Appropriate clearance from overhead obstructions
while raising or lowering the platform shall be
maintained.
4. Modification of personnel lifts is prohibited without written consent from the lifting
manufacturer.
5. Outriggers shall be used on lifts equipped with outriggers.
6. Personnel shall wear a full-body harness with lanyard attached to an anchor
point on the lift.
7. All entrance gates or chains shall be in their fully closed position and secured
shut before moving or raising the lift.
8. Equipment and tools shall not be carried on personnel lifts designed only for
personnel transfer.

18. Fire Protection Safety
_______________________________________________
a. Procedures in Case of Fire
Summon Help/Sound Alarm.
1. Only trained personnel shall operate fire extinguishing equipment.
2. Area incharge needs to ensure that all personnel are evacuated as per company
emergency response plan.
3. Isolate all electrical supply, fuel sources and/or threatened facilities and close
doors; do not attempt to extinguish gas fires.
4. Never operate an extinguisher in such a manner that may lead problem for other
staff.
5. Test the extinguisher before attempting to extinguish fire.
6. After the fire is extinguished, stand by to ensure that there is no reigniting.
b. Fire Prevention Guidelines
1. Class A fire materials (e.g., paper and wood) storage shall be minimized in
process and electrical areas.
2. Trained personnel shall visually inspect all fire equipment monthly.
3. Report and repair all hydrocarbon liquid or gas leaks immediately.
4. In the event of a hydrocarbon liquid or gas leak, remove sources of ignition
immediately (e.g., shut down engines).
5. Do not use gasoline as a cleaning agent.
6. Always fill portable gasoline containers on the ground and never place gasoline
containers inside stores, containers, vehicle passenger compartments.
7. Plastic cups/buckets shall not be used for collection of hydrocarbon samples.
8. Do not smoke, use cellular phones, other electrical devices while refueling
or within electrically classified areas.

19. Vehicles Safety
______________________________________________
The following safety rules apply to drivers of vehicles supplied by SZS SUPERVISOR and also
vehicles operated by contractors or others at SZS premises. Drivers of vehicles are responsible
for the safe operation of their vehicles, particularly in relation to speed.
a. Before You Can Drive
1. All drivers shall hold a valid driver's license.
2. All drivers shall keep the vehicles Tax paid certification, Insurance certification
Fitness certification before departure from SHY.
3. Drivers who operate SZS vehicles shall have Defensing Driver Improvement
Program certification.
b. Preparing to Drive
1. Drivers shall perform a 360 degree walk-around before getting into the vehicle.
2. Drivers shall inspect the vehicle's safety equipment according to checklist. (e.g.,
spare tires, vehicles tool box, torch,) before driving the vehicle.
3. Drivers shall report any unsafe conditions or any fault either minor immediately
to admin Dept.
4. Drivers shall ensure adequate supplies and communication equipment are
available for remote area driving.
c. While Driving
1. Drivers shall observe speeds in accordance with posted speed limits and driving
conditions.
2. The use or being under influence of intoxicants or medications which cause
impairment while operating a vehicle is prohibited. Using a cell phone, eating, or
drinking is prohibited during driving.
3. Only authorized personnel shall drive or ride in SZS vehicles. All occupants

4. Drivers shall not leave vehicles unattended while the engine is running.
5. Vehicles shall only be operated or parked in designated operations areas by

6. Vehicles shall not be fueled with the engine running or during use of any
electrical device.
7. Drivers shall report all vehicle collisions and moving violations immediately, no
matter how minor.
8. Driver is safe custodian of vehicles and avoid to handover key to loader.
9. Night Driving is prohibited according to company policy.

  • 4. 4 S. ZIA UL HAQ & SONSSZS 1 HSE Responsibilities According to Company HSE policy, Management is commitment to work safely I all on going operations. In this regards there is some responsibilities of line managers, supervisor, employees, visitor and contractors, Personnel, as referred to in this handbook, include SZS Managers, Supervisors, Foremen, Employees, and contractors have the following safety responsibilities: 2. Manager / Supervisor Responsibilities a. Establish and communicate safety rules and standards to all employees and contractors. b. Provide appropriate Information, instruction, resources; complete inspected tools, and adequate training for employees. c. Award contracts using safety, health, and environmental criteria. d. Conduct and document safety meetings. e. Identify & correct unsafe acts and conditions promptly. f. Conduct routine / non routine safety and environmental inspections and tours. g. Maintain facility emergency plans and conduct regular emergency drills. h. Investigate injuries and other incidents and promptly provide corrective actions. i. Coordinate with HSE Dept for non-routine activity at least 48 hours before to develop the control measure to prevent any incident. 3. Employee Responsibilities a. Comply with applicable rules, standards, and safe work practices. b. Communicate safety rules and standards to contractors and coworkers. c. Maintain knowledge of safety requirements, including emergency response action. d. Participate in safety meetings. e. Operate, inspect, and maintain facilities in a safe condition. f. Report injuries, unsafe conditions, near misses, and incidents immediately. g. Cooperate & assist in incident investigations as necessary. 4. Contractor Responsibilities a. Comply with all SZS rules, policies, and procedures. b. Report injuries, spills, unsafe acts and conditions, near misses, and incidents
  • 5. 5 S. ZIA UL HAQ & SONSSZS immediately. c. Hold pre-job meetings and other safety meetings during the job. d. Assist in incident investigations as necessary. 5. Report Incidents a. Immediately report all work related injuries/illnesses or vehicle collisions, no matter how slight, to HSE Dept. b. Immediately report all fires, spills. c. Immediately report any unsafe condition, practice, near miss, or incident to your supervisor and HSE Department. 6. Follow Safe Practices 1. According to safe work practice, always develop safe work method, identify the hazard of related activities, and develop the control measure to prevent incident and injuries, perform activities as per approved plan and after any changes in work procedure review the job safety analysis.
  • 6. 6 S. ZIA UL HAQ & SONSSZS 2. Erect barricades/flagging around hazardous work areas, such as holes in decking and floor surfaces, trenches, road crossings, and overhead hazardous work. 3. Use only proper tools and equipment maintained in a safe working condition. Homemade tools/ incomplete tools are prohibited. 4. Maintain good housekeeping in your work area at all times. 5. Smoking is permitted in designated areas only. Matches and lighters are prohibited in restricted areas. 6. Eating or resting is permitted in designated areas only. 7. Use proper manual lifting techniques, or obtain assistance or mechanical lifting aids when lifting loads. 8. All operating machinery ( e.g Grinder, Drill machine, ) and electrical switchgear shall have all safety guards, switches, and alarms in place and functional. 9. Whenever a safety device is removed from service and/or defeated, the appropriate supervisor and affected parties shall be notified, the device tagged, and the action properly documented. 10. All chemical or hazardous material containers shall be properly labeled and stored. Drums shall be stored in secondary containment areas or on drum containment pallets. 11. Operators of vehicles shall comply with all traffic requirements, particularly speed limits 12. All persons in vehicles shall wear seat belts at all times. 13. When ascending or descending stairways, use the handrail and take only one step at a time. 7. Avoid Unsafe Activities 1. Climbing or standing on equipment, an unstable surface to perform work is prohibited. 2. Approved fall protection measures (e.g. safety harnesses or scaffolding) shall be used if the working height is greater than 1.8 m (6 ft) from platform level. 3. Running in work areas is prohibited, except during emergencies. 4. Finger rings, wristwatches, jewelry, loose clothing, unsecured long hair, and other loose accessories shall not be worn within an arm's reach of rotating equipment or electrical switchgear. 5. Do not apply compressed air or other gases to yourself or others. 6. Non routine activity is prohibited in any area or on any equipment without consent of the SZS HSE Department. 7. Horseplay or fighting in SZS premises is prohibited.
  • 7. 7 S. ZIA UL HAQ & SONSSZS 8. Protect Yourself . 1. Approved hard hats without defects shall be worn in designated areas. Metallic hard hats are prohibited. 2. Approved safety footwear shall be worn in field operations, process areas, and other designated areas. 3. Approved safety eyewear with side protection shall be worn where designated. 4. Approved additional hazard-specific eye/face protection (e.g., goggles and/or a face shield) shall be worn where foreign objects may injure the eye or face (e.g., grinding, welding, drilling, or scraping). 5. Proper personal protective equipment (PPE) prescribed by the Chemical Hazard Bulletin (CHB) or Material Safety Data Sheet (MSDS) shall be worn when handling chemicals or hazardous materials. 6. Approved hearing protection shall be worn in high noise areas (i.e., 85 decibels [dBA] or higher) e.g hammering activity inside the containers, Generator areas. 7. Proper hand protection (e.g., gloves, latex coated gloves) shall be worn when performing tasks that may present a hand injury risk.
  • 8. 8 S. ZIA UL HAQ & SONSSZS 9. General Safety Precautions for Hot Work activity. ______________________________________________ a. All Operators and Helpers of Grinding, cutting, welding machine shall be experienced and certified. b. All Operator of Portable Tool and helpers shall use proper PPE according to Job activity. c. All equipment shall be visually inspected by the user daily and Site Supervisor and mark Inspected Tags. d. Site Supervisor is responsible for Defective equipment shall be removed and replaced from site. e. All welding, cutting and grinding operations shall have proper ventilation. f. All unsecured work pieces shall be properly clamped. g. All equipment shall be turned off when not in use. h. A portable fire extinguisher shall be available within 3 m (10 ft) Combustibles around the Hot work area shall be removed / protected. i. A Trained fire watch shall be maintained during operations and for 30 minutes afterwards due to re-ignite hazard. j. Grinding, Cutting, welding, are not allowed in O2 enriched environments (i.e., O2 concentration greater than 23.5%).
  • 9. 9 S. ZIA UL HAQ & SONSSZS 10. Cutting, Grinding Safety Rules Further refer to "Hot Work Safety" section. a. Grinding Machine and abrasive wheels should always be used in accordance with the manufacturer instructions. b. Always ensure the maximum speed marked on the abrasive wheel in greater than rated speed of the grinder. c. Never use cutting disc to grinding and grinding disc for cutting job d. Safety Guard must cover 50 % Grinding Wheel. Keep guard between you face and Wheel. e. Do not use the grinding wheels that are larges then the maximum recommended size, or worn down wheels from other grinders. f. Grinding wheel must be inspected (No crack, edge break etc) before use for operation. g. Never use the grinder wheel power tool without wheel safety guard.
  • 10. 10 S. ZIA UL HAQ & SONSSZS h. Power tool electrical outlets shall be the 3- pronged type with ground fault protection device. i. Wear all required PPEs such as transparent face shield, hearing protection (Ear Plug) as well as safety gloves, Coverall (avoid the loose dress due to entanglement hazard) and safety Shoes and perform safe activity. j. Run newly mounted discs at operating speed one minutes before operations
  • 11. 11 S. ZIA UL HAQ & SONSSZS 11. Electric Arc Welding Operations Further refer to "Electrical Safety" section. a. Electric welding machines shall be properly bonded and grounded and shall be inspected. b. Electric welding machine electrical outlets shall be the 3-pronged type with ground fault protection device. c. Welding cables shall be continuous within 3 m (10 ft) of the electrode holder. d. All damaged welding cables shall be removed from service; temporary repairs and taped joints are prohibited. e. Proper welding screens shall be used near other personnel. f. GFCI (Ground fault circuit interrupted Device must be installed with all electrical equipment to prevent electrical shock. g. Employees must be in complete PPEs such as Welding shield, leather gloves, slaves, cotton/ jeans Base cover all, safety shoes. etc. 12. Gas Welding and Gas Cutting, Operations Further refer to “Compressed Gas Cylinders" section. a. Lighted torches shall not be left unattended. b. Compressed type fittings shall be utilized on hose connections. c. All connections/equipment shall be checked for leaks before use (e.g., soap solution). d. Torch must be select as per Job requirement.
  • 12. 12 S. ZIA UL HAQ & SONSSZS e. Flashback arrestors shall be placed at O2/acetylene cylinder regulators as shown below in Figure ii. Flashback Arrestor Position on O2 / Acetylene h. Only strikers shall be used to light a torch; never light a torch with matches or a lighter. i. Gas regulators shall be turned off and hoses bled off at the end of shift or task. j. Hoses shall be protected from potential damage during operations. k. Ensure 15 feet distance between both cylinder during working.
  • 13. 13 S. ZIA UL HAQ & SONSSZS l. When opening an O2 or fuel cylinder valve, always crack it open first. m. Stand to the side of a regulator, not in front of it when opening cylinder valves. n. Acetylene cylinders in use shall have a handle or valve wrench in place at all times. o. During operation acetylene gas pressure must be less than 15 PSI. p. All valve and hose must be hydro test certified. q. All kind of cylinders must be placed vertical secure position during activity. TANK PRESSURE HOSE PRESSURE Flash Back Arrestor
  • 14. 14 S. ZIA UL HAQ & SONSSZS 13. Chemical Safety _______________________________________________ a. Labeling of Chemicals 1. Chemicals shall be properly labeled. 2. Chemical Hazard Bulletins (CHBs) or Material Safety Data Sheets (MSDSs) shall be readily available for each chemical on-site. b. Use of Chemicals 1. Use appropriate PPE/respiratory equipment as set out in the relevant MSDS/CHB. 2. Do not smell or taste chemicals or eat, drink, 3. smoke, or chew gum in areas where chemicals are present. 4. Wash hands thoroughly after handling chemicals. 5. Do not mix incompatible chemicals. 6. When mixing chemicals, use proper lab equipment. 7. In case of eye or skin contact, promptly flush affected areas with clean water for a prolonged period (15 minutes minimum), remove contaminated clothing, and seek medical attention. 8. Promptly clean spills while wearing appropriate PPE, and properly dispose of all contaminated materials. c. Storing Chemicals 1. Indoor storage of flammable and combustible materials shall be in approved metal cabinets. 2. Users shall periodically examine chemical containers for integrity. 3. Chemical storage shall be minimized. Avoid the storage on bench tops or at high levels 4. Incompatible chemicals shall not be stored near each other. 5. Toxic substances shall be segregated in a well-identified area with local exhaust ventilation. 6. Highly toxic chemicals, whose containers have been opened, shall be in secondary containment.   • 15. 15 S. ZIA UL HAQ & SONSSZS 14. Compress Gas cylinders Safety _______________________________________________ a. Handling Cylinders 1. Secure protective caps on all cylinders not in use. 2. Properly secure cylinders in an upright position during movement. 3. Do not use slings or ropes for moving cylinders. 4. All cylinders shall be properly labeled. 5. Do not strike cylinders together. 6. Keep valves on empty cylinders closed at all times. 7. Use cylinder trolleys, material baskets, cylinder racks, and other proper types of equipment to transport cylinders. b. Using Cylinders 1. Use approved pressure-reducing regulators with a check valve connected to the cylinder valve on all cylinders. 2. Always close the cylinder valve before attempting to stop leaks. 3. Do not use oil or grease as a lubricant on valves or attachments. 4. Physical Inspect cylinders for safe condition before use. 5. Protect cylinders from direct flame, sunlight, and other heat sources. 6. Inspect all gauges, regulators, hoses, and cylinders for damage and current hydro test date. 7. Place flashback arrestors at O2/acetylene cylinder regulators 8. (See "Gas Cutting, Gas welding” section). c. Storing Cylinders 1. Store cylinders in an upright, secured position; use bottle/ cylinder racks wherever possible. 2. Keep stored O2 cylinders separated from fuel gas cylinders by (20 ft), noncombustible barrier. 3. Keep cylinders from direct flame, sunlight, and other heat sources. 4. Do not store cylinders at temperatures exceeding 54 °C (130 °F). 5. Properly identify empty and full cylinders.
  • 16. 16 S. ZIA UL HAQ & SONSSZS 15. Crane Lifting Safety _______________________________________________ This section applies to all types of cranes including mobile and fixed. a. Inspection 1. All contractor mobile or fixed cranes shall be inspected by the third party prior to use within SZS premises. 2. All cranes shall have a valid inspection sticker and certificate from approval authority. Extra lifting equipment used below the hook (e.g., spreader bars, crane suspended personnel platforms, and man baskets) shall have a valid inspection from a certified inspector of third party. b. Operations 1. Details of crane lifts shall be communicated to the affected personnel in the area. 2. A designated signal man and crane operator shall use universal hand and/or radio signals. 3. The crane operator's line of sight to the designated signal man shall not be obstructed. 4. Tag-lines shall be used on all lifts, except when their use may create a greater hazard. 5. Know the accurate weight of all loads and lifting gear prior to the lift. Also follow the crane load chart. 6. Avoid load swings over personnel or traffic areas.
  • 17. 17 S. ZIA UL HAQ & SONSSZS 7. In positioned lifts, outriggers shall be used with appropriate pads and mats. Booms shall not be swung without outriggers being extended in accordance with the manufacturer's specification. 8. Crane operator shall not leave the crane cab while the crane is attached to a load. 9. Riggers must be certified and will inspect slings, fittings, and shackles prior to use. 10. All crane loads shall be rigged by a certified rigger. 11. Crane lifts shall not be allowed at wind speeds above 32 km/h (20 mph) unless otherwise specified by the manufacturer. 12. Crane suspended man basket operations shall not be allowed at wind speeds greater than 25 km/h (15 mph). 13. Cranes shall not be operated any closer than the depth of an excavation. 14. Cranes shall be operated around power lines in accordance with the distances provided in the "Electrical Safety" section. 9. Crane not placed on wet ground. 10. Critical Lift Plans are required for certain conditions and activities including: a. Cranes operating around power lines. b. All loads of 40 tons or greater. All loads exceeding 85% of the rated load capacity of the crane for that specific lift. c. Tandem, multiple, or tailing lifts (any lift requiring two or more cranes to lift one load). d. Night time and during rain crane lifts is prohibited e. Follow the load chart by the crane manufacturer f. Grounding facility must be installed with crane at the time of lifting near electrical line to avoid any kind of electrical shock.   • 18. 18 S. ZIA UL HAQ & SONSSZS c. Rigging Practices 1. Personnel shall use accepted rigging techniques. 2. All crane loads shall be rigged by certified rigger. 3. All rigging equipment shall be rated for the load being lifted. 4. Avoid sharp bends in slings; protect slings from sharp edges and abrasions. 5. Do not stand or walk under suspended loads. 6. Do not leave suspended loads unattended, unless the load is properly secured to a load bearing structural member. 7. Shackles and other connecting devices shall be completely closed/bolted. d. Inspection 1. Rigger /Users shall visually inspect all rigging hardware before each use. 2. Rigger/users shall properly maintain all rigging hardware. All defective components shall be removed from service. 3. Slings shall have their rated capacity indicated on a tag or stamp.
  • 19. 19 S. ZIA UL HAQ & SONSSZS 16. Electrical Safety _______________________________________________ a. Qualifications 1. Only individuals qualified for the type of electrical work to be undertaken shall repair or install electrical equipment, or work around live circuits. 2. Individuals who need to identify and possibly de-energize an electrical circuit shall attend electrical hazard recognition training. b. Work Practices 1. All electrical conductors shall be considered energized unless properly Locked, Tagged, Cleared, and Tried. 2. De-energize all circuits before beginning work. Use Safety` "Isolation, Lockout, and Use of Hold Tags" to prevent the electrical circuits from being inadvertently energized. 3. Use proper PPE including insulated rubber gloves for high voltage work. Additionally, mats and blankets may be required to provide insulation from other elements that are energized or grounded. 4. Personnel shall not wear rings, wristwatches, other jewelry, glasses with metal frames, or other similar metallic objects while working within arm's length of energized electrical equipment. 5. Metal ladders shall not be used when working on or near electrical equipment or conductors; use only nonconductive ladders. 6. Never use defective electrical equipment, defective extension cords, or shop-made extension cords. 7. When opening or closing a disconnect switch, personnel shall stand to the side, turn head away from switch, and throw switch with a quick upward or downward single motion. c. Electrical Lockout and Tag Procedure 1. The following procedures for electrical lockout and tag (Lock, Tag, Clear, Try) shall be followed when exposed to electrical hazards: 2. Operations shall identify isolating locations and types of Isolating devices required in determining potential exposure. 3. Qualified personnel doing the work shall notify affected personnel and properly shut down/de-energize the equipment.
  • 20. 20 S. ZIA UL HAQ & SONSSZS 4. Isolation is complete only when no associated control device is capable of energizing equipment. 5. SZS operations employees or the controlling organization shall always be the first to install an approved Lock. 6. The person doing the work shall lock out a device using his approved Lock. Remember to lock open the circuit breaker(s). 7. If the device cannot be physically locked out, it shall, at a minimum, be de- energized and tagged. 8. Tag the lock with a "DANGER, DO NOT OPERATE" tag. The tag shall contain date, time, organization, name, badge number, equipment name, phone contact and signature of the installer, and reason for installation. 9. Other organizations or individuals working on equipment shall also install their locks and tags at locations identified by operations. Multiple lockout clips (hasps) shall be used if necessary. A crew lockout is acceptable only when the key(s) of the person designated to lock out is properly secured and locked by all parties involved. 10. Each lock shall be keyed separately with no duplicate key available to ensure removal only by the installer. 11. Clear the area of personnel and tools prior to trying to start the equipment. 12. Before starting work, Try to start or energize the equipment locally. Verify the equipment cannot be energized. 13. Only the person(s) originally attaching the lock and tag are authorized to remove the lock and tag. Incoming shifts may attach their own locks and tags and/or transfer keys after approval by supervision. If person(s) are unavailable, the Division Head can assume responsibility for removing the lock and tag and notifying all parties. 14. SZS operations employees shall remove locks and tags when equipment is safe to energize. 15. When work is complete, affected personnel shall be notified and equipment placed back in service.

Fall Protection Safety _______________________________________________ Approved fall protection measures (e.g., personal fall arrest systems and scaffolding) shall be used if the working height is greater than 1.8 m (6 ft) from floor or platform level. a. When using full-body harnesses with shock-absorbing lanyards, they shall be properly secured at least 5.6 m (18 ft) above impact surface and shall not be reused after stopping a fall. 8. Waist belts and manual locking or non-self-locking snap hooks are prohibited for use as part of a fall arrest system. 9. Life line must be installed where is not possible to installed guard rail or other fall protection system which is able to carry 5000 pound weight. b. Fixed Elevated Work Surfaces 1. Personnel shall inspect all fixed elevated work surfaces prior to use (i.e., gratings clips). 2. Standing on a guardrail or ladder in close proximity to the edge of an elevated platform is prohibited. 3. Appropriate clearance from overhead obstructions while raising or lowering the platform shall be maintained. 4. Modification of personnel lifts is prohibited without written consent from the lifting manufacturer. 5. Outriggers shall be used on lifts equipped with outriggers. 6. Personnel shall wear a full-body harness with lanyard attached to an anchor point on the lift. 7. All entrance gates or chains shall be in their fully closed position and secured shut before moving or raising the lift. 8. Equipment and tools shall not be carried on personnel lifts designed only for personnel transfer.
  • 24. 24 S. ZIA UL HAQ & SONSSZS 18. Fire Protection Safety _______________________________________________ a. Procedures in Case of Fire Summon Help/Sound Alarm. 1. Only trained personnel shall operate fire extinguishing equipment. 2. Area incharge needs to ensure that all personnel are evacuated as per company emergency response plan. 3. Isolate all electrical supply, fuel sources and/or threatened facilities and close doors; do not attempt to extinguish gas fires. 4. Never operate an extinguisher in such a manner that may lead problem for other staff. 5. Test the extinguisher before attempting to extinguish fire. 6. After the fire is extinguished, stand by to ensure that there is no reigniting. b. Fire Prevention Guidelines 1. Class A fire materials (e.g., paper and wood) storage shall be minimized in process and electrical areas. 2. Trained personnel shall visually inspect all fire equipment monthly. 3. Report and repair all hydrocarbon liquid or gas leaks immediately. 4. In the event of a hydrocarbon liquid or gas leak, remove sources of ignition immediately (e.g., shut down engines). 5. Do not use gasoline as a cleaning agent. 6. Always fill portable gasoline containers on the ground and never place gasoline containers inside stores, containers, vehicle passenger compartments. 7. Plastic cups/buckets shall not be used for collection of hydrocarbon samples. 8. Do not smoke, use cellular phones, other electrical devices while refueling or within electrically classified areas.

Vehicles Safety ______________________________________________ The following safety rules apply to drivers of vehicles supplied by SZS SUPERVISOR and also vehicles operated by contractors or others at SZS premises. Drivers of vehicles are responsible for the safe operation of their vehicles, particularly in relation to speed. a. Before You Can Drive 1. All drivers shall hold a valid driver's license. 2. All drivers shall keep the vehicles Tax paid certification, Insurance certification Fitness certification before departure from SHY. 3. Drivers who operate SZS vehicles shall have Defensing Driver Improvement Program certification. b. Preparing to Drive 1. Drivers shall perform a 360 degree walk-around before getting into the vehicle. 2. Drivers shall inspect the vehicle's safety equipment according to checklist. (e.g., spare tires, vehicles tool box, torch,) before driving the vehicle. 3. Drivers shall report any unsafe conditions or any fault either minor immediately to admin Dept. 4. Drivers shall ensure adequate supplies and communication equipment are available for remote area driving. c. While Driving 1. Drivers shall observe speeds in accordance with posted speed limits and driving conditions. 2. The use or being under influence of intoxicants or medications which cause impairment while operating a vehicle is prohibited. Using a cell phone, eating, or drinking is prohibited during driving. 3. Only authorized personnel shall drive or ride in SZS vehicles. All occupants shall wear seat belts. 4. Drivers shall not leave vehicles unattended while the engine is running. 5. Vehicles shall only be operated or parked in designated operations areas by Drivers. 6. Vehicles shall not be fueled with the engine running or during use of any electrical device. 7. Drivers shall report all vehicle collisions and moving violations immediately, no matter how minor. 8. Driver is safe custodian of vehicles and avoid to handover key to loader. 9. Night Driving is prohibited according to company policy.
