   We are excited to offer you a paid internship opportunity with _______________ (the
   “Company”).

   As agreed, your internship is expected to start _____________ (Start Date and Time) and
   end __________ (End Date). As an intern, you are responsible for keeping track of your
   internship hours and providing all appropriate paperwork to your educational institution for
   college credit. The internship duration can be extended, shortened or discontinued without
   advance notice by the internship site supervisor or Company. Here is a brief description of the
   internship duties and/or expectations:

   [INSERT JOB DESCRIPTION AND/OR EXPECTATIONS HERE]

   As a paid intern, you will be considered a temporary employee. Temporary employees do not
   qualify for benefits that are given to Company employees such as salary, wages and
   compensation of any kind. You will not qualify for sick leave, vacation pay, holiday pay, 401
   (k) plans, health benefits or any other benefits given to Company employees. You understand
   that participation in this paid internship program is not an offer for full-time employment, and   (Sample from Quickstart Internship System)
   completion of the internship does not entitle you to full-time employment with our Company.
   Your temporary employment status is at-will and can be terminated at anytime by you or the
   Company without cause and without notice.

   By accepting this internship offer you agree to follow all Company policies, procedures and
   ethical standards that apply to non-Company interns. Access to confidential Company
   information, digital documents and trade secrets during the internship is possible. By
   accepting this internship you agree to keep this entire information confidential, refrain from
   using it for your own purposes or from disclosing this information to anyone outside the
   Company. At the conclusion of the internship you agree to return all Company property,
   equipment, documents and digitally stored files, images and videos.

   This temporary employment offer is contingent upon the receipt of proper identification and
   verification of your authorization to work within the United States. This letter describes the
   offer and terms of the paid internship with our Company and can be modified only if both
   parties agree and sign the document after the changes are made.

                    This evaluation form is designed to assist internship supervisors in giving feedback to interns
                    as well as help interns measure their progress. Introduce this form to the intern before they
                    start and explain the evaluation expectations. Supervisors may choose to evaluate interns
                    more than once depending on the length of the internship.

                    Please rate the interns on a scale of 1-5 in the following areas:

                    1   =   Unacceptable Performance
                    2   =   Below Average
                    3   =   Average
                    4   =   Above Average
                    5   =   Excellent
