We are excited to offer you a paid internship opportunity with _______________ (the
“Company”).
As agreed, your internship is expected to start _____________ (Start Date and Time) and
end __________ (End Date). As an intern, you are responsible for keeping track of your
internship hours and providing all appropriate paperwork to your educational institution for
college credit. The internship duration can be extended, shortened or discontinued without
advance notice by the internship site supervisor or Company. Here is a brief description of the
internship duties and/or expectations:

As a paid intern, you will be considered a temporary employee. Temporary employees do not
qualify for benefits that are given to Company employees such as salary, wages and

as well as help interns measure their progress. Introduce this form to the intern before they
