Response one Epidemiology and Other Disciplines Epidemiology is the “study of the distribution and determinants of health and disease, morbidity, injuries, disability, and mortality in populations” (Friis & Sellers, 2014, p. 743). Epidemiology was first developed to understand causes of certain diseases such as smallpox and polio among humans. It now also includes the study of factors associated with non-transmissible diseases like cancer. It is described as interdisciplinary as it borrows elements from many other disciplines including microbiology and sociology. Epidemiology utilizes microbiology to help understand specific disease agents and modes of transmission. Microbiological techniques are borrowed to help in revealing sources of outbreaks and to determine sources. Sociology is equally important in epidemiology to aid in the study of social conditions and disease processes. Social sciences also assist epidemiologists in providing different methods on sampling such as measurement, questionnaire development, design, and delivery (Friis & Sellers, 2014). “Social factors have become more important precisely because epidemiological and biomedical knowledge has shifted the causes and consequences of disease from fate, accident, and bad luck to factors that are under some human control” (Link, 2008, p. 367). Epidemiology differs from other disciplines in its perspective on groups or populations rather than individuals. It contrasts diseases and characteristics relative to different time periods, different places or different groups. It also differs from the physical sciences because it does not investigate the biological mechanism leading from exposure to disease. Epidemiologists can identify modifiable conditions that contribute to the health outcome without also identifying the biological mechanism or agent that lead to the outcome. An example of this is the improvements of environmental hygiene that reduced infectious diseases like cholera, that was possible before the identification of the actual bacteria (Ahrens, Krickeberg, & Pigeot, 2005). Epidemiological studies are crucial to preventing, controlling and eradicating diseases. The research helps us to understand the incidence and prevalence of diseases, the cost of illness, and the burden of disease on society (Friis & Sellers, 2014). O R G ✦ VO L . 2 , N O. 6 ✦ N OV E M B E R / D E C E M B E R 2 0 0 4 576 The Biopsychosocial Model 25 Years Later: Principles, Practice, and Scientifi c Inquiry ABSTRACT The biopsychosocial model is both a philosophy of clinical care and a practical clinical guide. Philosophically, it is a way of understanding how suffering, disease, and illness are affected by multiple levels of organization, from the societal to the molecular. At the practical level, it is a way of understanding the patient’s subjec- tive experience as an essential contributor to accurate diagnosis, health outcomes, and humane care. In this article, we defend the biopsychosocial model as a nec- essary contribution to the scientifi c clinical method, while suggesting 3 clarifi ca- tions: (1) the relationship between mental and physical aspects of health is com- plex—subjective experience depends on but is not reducible to laws of physiology; (2) models of circular causality must be tempered by linear approximations when considering treatment options; and (3) promoting a more participatory clinician- patient relationship is in keeping with current Western cultural tendencies, but may not be universally accepted. We propose a biopsychosocial-oriented clinical prac- tice whose pillars include (1) self-awareness; (2) active cultivation of trust; (3) an emotional style characterized by empathic curiosity; (4) self-calibration as a way to reduce bias; (5) educating the emotions to assist with diagnosis and forming thera- peutic relationships; (6) using informed intuition; and (7) communicating clinical evidence to foster dialogue, not just the mechanical application of protocol. In con- clusion, the value of the biopsychosocial model has not been in the discovery of new scientifi c laws, as the term “new paradigm” would suggest, but rather in guid- ing parsimonious application of medical knowledge to the needs of each patient. He offered a holistic alternative to the prevailing biomedical model that had dominated indus- trialized societies since the mid-20th century.1 His new model came to be known as the biopsychosocial model. He formulated his model at a time when science itself was evolving from an exclusively analytic, reductionis- tic, and specialized endeavor to become more contextual and cross-disci- plinary.2-4 Engel did not deny that the mainstream of biomedical research had fostered important advances .

In BC’s nearly-decade-old “new” curriculum, the curricular competencies describe the processes that students are expected to develop in areas of learning such as mathematics. They reflect the “Do” in the “Know-Do-Understand” model. Under the “Communicating” header falls the curricular competency “Explain and justify mathematical ideas and decisions.” Note that it contains two processes: “Explain mathematical ideas” and “Justify mathematical decisions.” I have broken it down into its separate parts in order to understand--or reveal--its meaning. The first part is commonplace in classrooms. By now, BC math teachers—and students—understand that “Explain mathematical ideas” means more than “Show your work.” Teachers consistently ask “What did you do?” and “How do you know?” This process is about retelling, not just of steps but of thinking. The second part happens less frequently. Think back to the last time that you observed a student make—a necessary precursor to justify—a mathematical decision. “Justify” is about defending. Like “explain,” it involves reasoning; unlike “explain,” it also involves opinion and debate. In order to reinterpret the curricular competency “Explain and justify mathematical ideas and decisions,” I will continue to take apart its constituent part “Justify mathematical decisions” and carefully examine the term “mathematical decisions.” What, exactly, is a “mathematical decision”? Below, I will categorize answers to this question. These categories, and the provided examples, may help to suggest new opportunities for students to justify.
