There are 12 ECG leads
- 6 limb leads (I, II, III, aVR, aVL, aVF); Frontal plane
- 6 precordial leads (V1-V6); Horizontal plane
Each lead has a (+)ve pole and (-)ve pole.
- Limb leads: bipolar (I, II, III) &
unipolar (aVR, aVL, aVF)
- Precordial leads: unipolar
Electrical activity approaching a (+)ve pole will as a
(+)ve [upright] deflection on ECG tracing.

• There is no absolute right way to read an
ECG.
• A prior ECG is your best friend.
• Knowing the patient‟s clinical history is
essential for correct ECG interpretation
(ECG machines make squiggly lines on
paper, doctors make diagnoses).

45. Step 3: The PR Interval
- PR หรือ PQ interval หมายถึง Time from atrium activation to
ventricle activation (antrioventricular, AV, conduction time)

47. First degree AV block
This is the mildest form of heart block. In this case,
the electrical signals from the SA node move
more slowly than normal to the AV node, but all
signals reach the ventricles.
PR > 0.2, remain the same beat-to-beat

48. Second degree AV block (Mobitz)
Some of the electrical signals are not reaching
the ventricles. This causes “dropped beats.”
Mobitz I (Wenkebach): The electrical signals become
increasingly delayed with each heartbeat,
ultimately causing a beat to be missed.
• PR gradually increases, then fail to conduct.

49. Second degree AV block (Mobitz)
Mobitz II: In this type of heart block, some of the
electrical signals do not reach the ventricles. This
is less common, but more serious.
• PR prolonged, P fails to conduct at set interval (i.e.
2:1, 3:1)

50. Third degree AV block
(complete heart block)
This is the most serious type of heart block. In
this condition, no electrical signals from the SA node
are able to reach the ventricles. The ventricles
compensate by contracting on their own.

of the ventricles of the heart due to an
