The efficacy of oral vitamin D supplementation in preventing hip and nonvertebral fractures was investigated in a systematic review of 12 double-blind, randomized, controlled trials that included a total of 19,114 patients (mean age, 79 years) who received either low-dose (400 IU/day) or high-dose (700–800 IU/day) vitamin D therapy and underwent at least 1 year of follow-up. Five of the trials focused on hip fracture prevention (n=9294) and 7 on nonvertebral fracture prevention (n=9820). 1 The primary end point was the reduction in the relative risk of hip fracture or any nonvertebral fracture. The pooled relative risk for any vitamin D dose (400–800 IU/d) was 0.88 (95% confidence interval [CI]: 0.69, 1.13) in preventing hip fractures and 0.83 (95% CI: 0.70, 0.98) in preventing nonvertebral fractures. 1 A meta-regression analysis demonstrated a significant inverse relationship between increased 25(OH)D concentration and decreased risk of hip fracture ( P =0.02) and nonvertebral fracture ( P =0.03). A recent review examined the effect of compliance with vitamin D therapy on the risk of hip fracture in several randomized, controlled clinical trials. 1–6 The estimated mean daily intake of vitamin D in the active treatment group of each study was calculated by adding the amount of vitamin D provided in the study supplement to dietary vitamin intake and multiplying this sum by the percentage of patients who complied with therapy. The purple dots show the relative risk of hip fracture in each group, and the values beneath each dot show the mean DiaSorin equivalent serum concentration of 25(OH)D in each group expressed in nanomols per liter. For the Women’s Health Initiative (WHI) trial, these values were not available. 1 Mean daily vitamin D intake was correlated with a lower risk of hip fracture.
