Regarding vacation time and personal/sick days, and ensuring that they are being reported to HR
and tracked correctly, the following procedures will be put in place, effective immediately.
For Time Off in advance: The Time Off Request Form needs to be filled out by the requester and
given to their direct supervisor for approval. The Supervisor then needs to turn the Time Off
Request Form into HR. If the supervisor is requesting time off, the form needs to be filled out and
turned into HR.
Calling Off: The person calling off needs to call or email their direct Supervisor to inform them that
they will be out of the office that day. The Supervisor then needs to either call or email HR with the
information so that HR can update the Time Off Calendar. If the Supervisor needs to call off, they
need to either call or email HR. Upon returning to work, a Time Off Request Form needs to be filled
