The TRANSAMERICA LIVESTRONG SurvivorPlan provides group cancer and critical illness insurance to help employees pay for treatment costs. Insured employees diagnosed with cancer receive support services from the LIVESTRONG Foundation. The plan offers wellness benefits, coverage for hospital and surgery costs, and pays cash benefits to use as the insured chooses. It supports the LIVESTRONG Foundation's mission of aiding cancer survivors.
1. TRANSAMERICA® LIVESTRONG® SurvivorPlan Overview
TRANSAMERICA LIVESTRONG SurvivorPlan products help employers inspire their employees to choose
healthier lifestyles and reduce the risk of future illness. Insured employees who are diagnosed with cancer
and file a claim are connected with an array of LIVESTRONG resources designed to help them take control
of their treatment and recovery.

insured may have. According to Cancer Facts & Figures 2007, published by the American
Cancer Society, major medical insurance covers about 36 percent of the costs related

to a cancer diagnosis, like hospital fees and medication. Out-of-pocket costs average
63 percent and include deductibles, co-insurance, lodging and transportation as well
as lost wages.1 Even if a company’s major medical plan covers some of these costs,
TRANSAMERICA LIVESTRONG SurvivorPlan insurance pays benefits that covered
employees can use as they see fit.

PRODUCT PARAMETERS: The group cancer-only policy is customizable and must include $100
Wellness benefit within plan design.
The group critical illness policy must include the Cancer category and $100 Wellness benefit within plan
design. GI coverage up to $20,000 is available with minimum benefit of $10,000 (The CI minimum benefit
can be as low as $5,000 if the employer contributes 50% or more of employee premium, the 100+ life group is enrolled
electronically or by spreadsheet, and employees are given buy-up options.)
All riders are available for plan designs.
PLAN DESIGNS: The employer can select up to two group cancer-only plan design options for
its employees. This product has five core modules: Hospital Benefits; Surgery Benefits; Radiation and
Chemotherapy Benefits; Wellness and Miscellaneous Benefits; and Cancer Maintenance Therapy Benefits.
The employer must select a unit value for each of the five core modules along with optional riders.
Critical illness is designed to provide employees with benefits to help pay for common health care needs.
Employers can choose to offer up to two plan design options. Each option provides a lump sum benefit for
certain critical illnesses and additional benefits. The base plan includes Categories One, Two and Three.
Category Four is optional and is selected by the employer along with the Optional Benefit Riders.
Multiple TRANSAMERICA LIVESTRONG SurvivorPlan group cancer-only and group critical illness
products may be sold within an employer group. Since this is supplemental insurance, there are no plan
deductibles. In fact, TRANSAMERICA LIVESTRONG SurvivorPlan insurance is designed to help insureds
defray their out-of-pocket costs, such as deductibles and co-payments.

insureds to use as they see fit. However, the plan provides an evaluation benefit if the insured seeks an evaluation or second opinion from
an approved National Cancer Institute Center.
GROUP PARAMETERS:
G Groups with 1000 lives or greater must purchase the SurvivorPlan group cancer-only and group critical illness (with cancer option)2
products on a GI3 basis.
G Groups with 100 - 1000 lives have the option of choosing SurvivorPlan products on a GI3 Basis. If non–SurvivorPlan products are
chosen they will be offered on an SI basis.
G Groups with less than 100 lives are eligible for SurvivorPlan products but on a Simplified Issue basis.
G Groups domiciled in the following states are not eligible for group cancer-only insurance: Florida, Minnesota, Montana, Vermont and
Puerto Rico.
G Groups domiciled in the following states are not eligible for group critical illness insurance: Connecticut, Florida, Georgia, Minnesota,
Montana, New Jersey, New York, Vermont, Washington and Puerto Rico.
G All SIC codes are eligible for these products.
G Eligible groups include employer groups and unions. These products are designed for new groups and not available for associations or
credit unions.
G With underwriting approval, existing groups are eligible for TRANSAMERICA LIVESTRONG SurvivorPlan products.
G The issue age is 16+ for enrollment as the primary insured. The issue age range for dependent children is birth through age 24, which
varies by state.
COMPREHENSIVE BLOOD SCREENING PROFILE INFORMATION: Upon the employer’s election, employees can choose to
receive a comprehensive blood screening profile as their annual wellness test. This test has a retail value in excess of $400. Employees who
select this optional blood profile must assign their wellness benefit to Quest Diagnostics, and Transamerica will pay Quest directly for the
test.
The insured can choose to share the results of his or her blood profile with his or her personal physician for further analysis and treatment
if needed. If at least 40 employees in a group choose the comprehensive blood screening profile option, the employer can receive an
Employer Wellness Report. It is a broad overview of the health of the employee population and does not include any personally identifiable
information about the participating employees. The employer can use this information to develop a workplace wellness program to enhance
employees’ health and productivity.
PROPOSALS: The proposal system contains a TRANSAMERICA LIVESTRONG SurvivorPlan option. Once selected, it will pull in a
TRANSAMERICA LIVESTRONG cover page detailing the program.
RATES: Standard state group cancer-only and group critical illness rates are available at www.transamericaworksite.com and must include
a $100 Wellness benefit in each plan design.
ENROLLMENT: Available on TransLink® and Common Census. GI groups will be able to enroll via spreadsheet with approval from
Case Management.
BILLING: This program will follow Transamerica’s normal billing procedures for group cancer–only and group critical illness.
COMMISSIONS: TRANSAMERICA LIVESTRONG SurvivorPlan products will have special commission rates in order to allow for
GI underwriting with no participation requirements. New agent numbers and commission schedules will be required. Please contact your

SALES TOOLS: A PowerPoint® presentation is available for producers. Customizable employee materials are available similar to current
group cancer-only and group critical illness materials. Producer and employer brochures are also available on the web site.
CUSTOMER SERVICE & CLAIMS: Administration is provided by our Home Office in Little Rock, Ark., at (888) 763-7474. Upon
submission of a claim, the insured will receive a letter from Transamerica reminding him or her of the availability of the LIVESTRONG
Guidebook at no cost.
