    So far, the present guidelines stipulate that a person suspected of influenza A H1N1 need
to be referred to an identified govt. health facility. He/she needs to be kept in an isolation
facility in that hospital and if found positive, is treated accordingly.

    In order to make the testing facility for H1N1 more accessible at large and due to the
onset of the Influenza season in the country, it has been decided to revise the existing
guidelines.

    Under the new guidelines, any person with flu like symptoms such as fever, cough, sore
throat, cold, running nose etc. should go to a designated Government facility for giving
his/her sample for testing for the H1N1 virus. After clinical assessment, the designated
medical officer would decide on the need for testing. Except for cases that are severe, the
 patient would be allowed to go home (This was not allowed under the existing guidelines).

    The sample of the suspect case would be collected and sent to the notified laboratory for
 testing. If tested as positive for H1N1 and in case the symptoms are mild, the patient would
 be informed and given the option of admission into the hospital or isolation and treatment at
 his own home.

    In case the patient opts for home isolation and treatment, he/she would be provided with
 detailed guidelines / safety measures to be strictly adhered to by the entire household of the
 patient. He/ she would have to provide full contact details of his entire household. The house
 hold and social contacts would be provided with the preventive treatment.

    Notwithstanding the above guidelines, the decision of the doctor of the notified hospital

    These guidelines have been issued by the Government in public interest and shall be

 reviewed from time to time depending on the spread of the pandemic and its severity in the

When providing care to a household member who is sick with influenza, the most
important ways to protect yourself and others who are not sick are to:

· keep the sick person away from other people as much as possible (see “placement of the
sick person”)

· remind the sick person to cover their coughs, and clean their hands with soap and water or

an alcohol-based hand rub often, especially after coughing and/or sneezing.
· have everyone in the household clean their hands often, using soap and water or an alcohol-
based hand rub

Placement of the sick person:

· Keep the sick person in a room separate from the common areas of the house. (For example,
a spare bedroom with its own bathroom, if that’s possible.) Keep the sickroom door closed.
· Unless necessary for medical care, persons with the flu should not leave the home when
they have a fever or during the time that they are most likely to spread their infection to
others
· If persons with the flu need to leave the home (for example, for medical care), they should
cover their nose and mouth when coughing or sneezing and wear a loose-fitting (surgical)
mask if available.
· Have the sick person wear a surgical mask if they need to be in a common area of the house
near other persons.
· If possible, sick persons should use a separate bathroom. This bathroom should be cleaned
daily with household disinfectant
Protect other persons in the home:

· The sick person should not have visitors other than caregivers. A phone call is safer than a
visit.
· If possible, have only one adult in the home take care of the sick person.
· Avoid having pregnant women care for the sick person. (Pregnant women are at increased
risk of influenza-related complications and immunity can be suppressed during pregnancy).
· All persons in the household should clean their hands with soap and water or an alcohol-
based hand rub frequently, including after every contact with the sick person or the person’s
room or bathroom.
· Use paper towels for drying hands after hand washing or dedicate cloth towels to each
person in the household. For example, have different colored towels for each person.
· If possible, consideration should be given to maintaining good ventilation in shared
household areas (e.g., keeping windows open in restrooms, kitchen, bathroom, etc.).

If you are the caregiver:

· Avoid being face-to-face with the sick person. · When holding small children who are sick,

place their chin on your shoulder so that they will not cough in your face. · Clean your hands
with soap and water or use an alcohol-based hand rub after you touch the sick person or
handle used tissues, or laundry. · Talk to your Doctor about taking antiviral medication to

prevent the caregiver from getting the flu. · Monitor yourself and household members for flu

Household Cleaning, Laundry, and Waste Disposal:

(such as bed sheets and towels) by using household laundry soap and tumble dry on a hot

setting. Avoid “hugging” laundry prior to washing it to prevent contaminating yourself. Clean
your hands with soap and water or alcohol-based hand rub right after handling dirty laundry. ·
Eating utensils should be washed either in a dishwasher or by hand with water and soap.
What is the new influenza A(H1N1)?

This is a new influenza A(H1N1) virus that has never before circulated among humans.
This virus is not related to previous or current human seasonal influenza viruses.


How do people become infected with the virus?

The virus is spread from person-to-person. It is transmitted as easily as the normal
seasonal flu and can be passed to other people by exposure to infected droplets expelled
by coughing or sneezing that can be inhaled, or that can contaminate hands or surfaces.
To prevent spread, people who are ill should cover their mouth and nose when coughing
or sneezing, stay home when they are unwell, clean their hands regularly, and keep
some distance from healthy people, as much as possible. There are no known instances
of people getting infected by exposure to pigs or other animals. The place of origin of
the virus is unknown.

Why are we so worried about this flu when hundreds of thousands die every year from
seasonal epidemics?

Seasonal influenza occurs every year and the viruses change each year - but many
                                 ke




people have some immunity to the circulating virus which helps limit infections. Some
                             ar




countries also use seasonal influenza vaccines to reduce illness and deaths. But influenza
A(H1N1) is a new virus and one to which most people have no or little immunity and,
                       em




therefore, this virus could cause more infections than are seen with seasonal flu. WHO
is working closely with manufacturers to expedite the development of a safe and

effective vaccine but it will be some months before it is available. The new influenza
A(H1N1) appears to be as contagious as seasonal influenza, and is spreading fast

particularly among young people (from ages 10 to 45). The severity of the disease ranges
from very mild symptoms to severe illnesses that can result in death. The majority of
people who contract the virus experience the milder disease and recover without
antiviral treatment or medical care. Of the more serious cases, more than half of
hospitalized people had underlying health conditions or weak immune systems.


Most people experience mild illness and recover at home. When should someone seek
medical care?

A person should seek medical care if they experience shortness of breath or difficulty
breathing, or if a fever continues more than three days. For parents with a young child
who is ill, seek medical care if a child has fast or labored breathing, continuing fever or
convulsions (seizures). Supportive care at home - resting, drinking plenty of fluids and
using a pain reliever for aches - is adequate for recovery in most cases. (A non-aspirin
pain reliever should be used by children and young adults because of the risk of Reye's
syndrome.)
Is it safe to eat pork and pork products?

Yes. influenza A(H1N1) has not been shown to be transmissible to people through
eating properly handled and prepared pork (pig meat) or other products derived from
pigs. The influenza A(H1N1) virus is killed by cooking temperatures of 160°F/70°C,
corresponding to the general guidance for the preparation of pork and other meat.


What can I do to protect myself from catching influenza A(H1N1)?

The main route of transmission of the new influenza A(H1N1) virus seems to be similar
to seasonal influenza, via droplets that are expelled by speaking, sneezing or coughing.
You can prevent getting infected by avoiding close contact with people who show
influenza-like symptoms (trying to maintain a distance of about 1 metre if possible) and
taking the following measures:
· avoid touching your mouth and nose;
· clean hands thoroughly with soap and water, or cleanse them with an alcohol-based
hand rub on a regular basis (especially if touching the mouth and nose, or surfaces that

are potentially contaminated);
· avoid close contact with people who might be ill;

· reduce the time spent in crowded settings if possible;

· improve airflow in your living space by opening windows;

· practise good health habits including adequate sleep, eating nutritious food, and
keeping physically active.

What about using a mask? What does WHO recommend?

If you are not sick you do not have to wear a mask. If you are caring for a sick person,

you can wear a mask when you are in close contact with the ill person and dispose of it
immediately after contact, and cleanse your hands thoroughly afterwards. If you are

sick and must travel or be around others, cover your mouth and nose. Using a mask

correctly in all situations is essential. Incorrect use actually increases the chance of
spreading infection.

How do I know if I have influenza A(H1N1)?

You will not be able to tell the difference between seasonal flu and influenza A(H1N1)
without medical help. Typical symptoms to watch for are similar to seasonal viruses and
include fever, cough, headache, body aches, sore throat and runny nose. Only your
medical practitioner and local health authority can confirm a case of influenza
A(H1N1).


What should I do if I think I have the illness?

If you feel unwell, have high fever, cough or sore throat:
· stay at home and keep away from work, school or crowds;
· rest and take plenty of fluids;
· cover your nose and mouth when coughing and sneezing and, if using tissues, make
sure you dispose of them carefully. Clean your hands immediately after with soap and
water or cleanse them with an alcohol-based hand rub;
· if you do not have a tissue close by when you cough or sneeze, cover your mouth as
much as possible with the crook of your elbow;
· use a mask to help you contain the spread of droplets when you are around others, but
be sure to do so correctly;
· inform family and friends about your illness and try to avoid contact with other
people;
· If possible, contact a health professional before traveling to a health facility to discuss
whether a medical examination is necessary.


Should I take an antiviral now just in case I catch the new virus?

No. You should only take an antiviral, such as oseltamivir or zanamivir, if your health
care provider advises you to do so. Individuals should not buy medicines to prevent or
fight this new influenza without a prescription, and they should exercise caution in
buying antivirals over the Internet.


What about breastfeeding? Should I stop if I am ill?

No, not unless your health care provider advises it. Studies on other influenza infections

show that breastfeeding is most likely protective for babies - it passes on helpful

maternal immunities and lowers the risk of respiratory disease. Breastfeeding provides

the best overall nutrition for babies and increases their defense factors to fight illness.

H1N1 Testing Procedure at the Hospital

1. First you will have to meet the Doctor informing him/her that you have come for
H1N1 test.


2. After that you will have to register at the OPD of the Hospital


3. The Hospital guards will be your best friend while you are at the facility and will
guide you through the registration and Lab Procedures


4. Typically the doctor will ask you to get a chest X Ray and Blood test done

5. When you go for the X Ray center make sure that you breathe in when the X Ray is

performed, the technician may forget to tell you this and your X Ray may not be of any
use and you will have to wait for another day.

6. Get your Chest X Ray done before 1:30 pm as the X Ray center closes after that

7. The Blood collection and testing center is adjacent to the OPD and you may have to

go there and give your blood sample. Do not forget to ask the Technician the time when
you can collect the report.

8. After you have given your blood sample and got you chest X Ray done, the doctor will

advise you to go and wait in the Swine Flu Isolation Ward.

9. In the mean time if you are hungry, you are free to go and have food.


10. After you go back to the Swine Flu Isolation Ward, a Intern will take the details
mentioned in the ‘Information that you need to keep handy’ from you, he/she will check
your BP, Heart Beat, Weight and other relevant medical details.


11. In case your Blood report has not come to the Isolation Ward and the ward is under
staffed, you will have to go and collect your report from the Lab, make sure you reach
the Lab before 4 pm.


12. Around 3:30 pm the Doctor will come to the Isolation ward and check your reports
and will examine you for the Flu Symptoms
13. In case you have some of the Flu Symptoms you will be given a mask immediately
and your swab sample will be sent to National Institute for Communicable Diseases,
New Delhi or the National Institute of Virology, Pune.


14. Depending on the severity of your symptoms you may be allowed to go home or you
may be isolated at the Quarantine facility


15. If your tests are normal you will be allowed to go home and you can heave a sigh of
relief that you did not contact the Virus.

1.Each passenger flying in from an International Location is required to fill in a Form which

he or she will be given at the airport.
2.Passengers flying in from USA,Mexico,Germany, Canada,Austria,New Zealand,

He/she needs to be kept in an isolation facility in that hospital and if found positive, is treated accordingly. In order to make the testing facility for H1N1 more accessible at large and due to the onset of the Influenza season in the country, it has been decided to revise the existing guidelines. Under the new guidelines, any person with flu like symptoms such as fever, cough, sore throat, cold, running nose etc. should go to a designated Government facility for giving his/her sample for testing for the H1N1 virus. After clinical assessment, the designated
  • 2. medical officer would decide on the need for testing. Except for cases that are severe, the patient would be allowed to go home (This was not allowed under the existing guidelines). The sample of the suspect case would be collected and sent to the notified laboratory for testing. If tested as positive for H1N1 and in case the symptoms are mild, the patient would be informed and given the option of admission into the hospital or isolation and treatment at his own home. In case the patient opts for home isolation and treatment, he/she would be provided with detailed guidelines / safety measures to be strictly adhered to by the entire household of the patient. He/ she would have to provide full contact details of his entire household. The house hold and social contacts would be provided with the preventive treatment. Notwithstanding the above guidelines, the decision of the doctor of the notified hospital om about admitting the patient would be final. .c ai In case the test is negative, the patient will accordingly be informed. gm These guidelines have been issued by the Government in public interest and shall be g@ reviewed from time to time depending on the spread of the pandemic and its severity in the country.   • 5. What is the new influenza A(H1N1)? This is a new influenza A(H1N1) virus that has never before circulated among humans. This virus is not related to previous or current human seasonal influenza viruses. How do people become infected with the virus? The virus is spread from person-to-person. It is transmitted as easily as the normal seasonal flu and can be passed to other people by exposure to infected droplets expelled by coughing or sneezing that can be inhaled, or that can contaminate hands or surfaces. To prevent spread, people who are ill should cover their mouth and nose when coughing or sneezing, stay home when they are unwell, clean their hands regularly, and keep some distance from healthy people, as much as possible. There are no known instances of people getting infected by exposure to pigs or other animals. After that you will have to register at the OPD of the Hospital 3.
