High Reliability Organizations
All staff with occupations that have potential influ-
ence on the safety and reliability of the overall
organisation, e.g.:
»» Managers
»» Process and project staff
»» Engineers, foremen, technicians
»» Operation and maintenance staff
You develop a sense how devious disasters can be.
You learn how High Reliability Organizations pre-
vent disasters and which of these provisions are
also of value for your organization.
You develop a sense for the importance even you
have for a safe, reliable and cost effective overall
organization.
More and more forms of organizations impose besides operating efficiency considerable requirements on
high reliability. Formerly it were only branches like the nuclear industry, the chemical industry, the oil & gas
industry and the aerospace or shipping industry, that had to take extensive preventive measures against
catastrophes. Today a lot of organizations have to take comprehensive safety and risk management mea-
sures to cope with catastrophic hazards, partially endangering their existence.
This is due to the fact that organizations become ever bigger, more complex and technology-oriented
and have to prevail in a complex and dynamic international environment. The progressing globalization
and pervasion of the world with automation and information technology contribute substantially to these
developments.
Superordinate objective of the seminar is to give the participants an imaginati-
on, what ex ante hardly imaginable, devious progression mechanisms catastro-
phes can have, and to raise their awareness for the importance every employee
has for the safety and reliability of the overall system.

High Reliability Organizations
What are the characteristics of high reliability organizations?
»» High complexity
»» Potential dynamic
»» Inherent risk of disaster
»» How can such organizations be managed?
What distinguishes High Reliability Organizations from ordinary organi-
zations, what do they have in common?
Where is development heading?
Examples for disasters
For important industry branches two disasters are briefly discussed and
the most prominent accident causes are mentioned:
»» Nuclear Industry: Fukushima (2011), Chernobyl (1986)
»» Oil & Gas Industry: Deepwater Horizon (2010), Piper Alpha (1988)
»» Chemical Industry: Bhopal (1984), Seveso (1976)
»» Aviation: JAL Flight 123 (1985), Concorde (2000)
»» Space Flight: Columbia (2003), Challenger (1986)
»» Maritime Transport: Exxon Valdez (1989), Herald of free Enterprise (2012)
Discussion of one of the catastrophes above
Anarbitraryoneoftheexamplesaboveisdiscussedwiththeparticipants,
particularly regarding
»» the accident sequence,
»» the accident causes and
»» the compensatory measures, which would have avoided the accident.
Risk
Management
Role of safety and risk management
What‘s the role of safety and risk management in HRO?
Where is it integrated into the organization?
For what must HRO pay attention beyond normal organizations?
»» Safety Culture
»» Human Performance
»» Latent Conditions, Barrier Concept
»» Single Failure Proofness
»» Resilience
Transferability
Transferability to organizations in general
What can organizations in general learn from HRO?
Which relevancy have these things not only for safety, but also for reliabi-
lity and cost effectiveness?
How can conflicts between safety and cost effectiveness be resolved?
What do individual members of the organization have to observe in this
respect?
