2. Go to http://docs. google .com/

3. Sign in using your gmail or google account.

4. Click Create new

5. Scroll to Form

6. Form template will open.

7. This is an example of a form template.

8. You can now start editing your form.

9. Enter a title of the form that you would like to create.

10. Add a description of your form.

11. Enter a question title.

12. Choose which question type you would want to use.

13. Click the arrow down.

14. Here are some of the question types that you can choose from.

15. For long answers, you may choose paragraph text .

16. Or scale , if you want the recipients to grade something.

17. If you would like to divide your form in sections, go to Section Header.

20. If you created a long form, for instance, you can add Page breaks.

39. On the left side, choose forms under ‘Narrow by type’.

41. You can take a preview first of your chosen template.

47. Enter the email addresses of the people to whom you want to send the form.
