professional expertise
Localise where possible, centralise where
necessary and network where appropriate
People have to change
• To develop services to
support integrated care and
optimise the value added
by hospitals to the whole
system, people have to
change
Integrated whole person care can only be
delivered by a whole system
• Teams working together to
derive solutions have
developed multi-agency
groups at practice level
• Community teams do not
work in isolation – they are
only as good as the
partnerships they work in

Key messages
• Hospitals have a critical role to play in a high value healthcare
system
• We must share a common purpose to optimise value
• A key challenge is the funding – within health and across
borders into the local authority and voluntary sector
• We have to allow technology to deliver information that flows
around the patient
• Integrated care needs to mean urgent care
• If we wait – the first victims will be safety and quality
• The real limit is our imagination
• Not delivering will be a failure of leadership

  • 7. People have to change • To develop services to support integrated care and optimise the value added by hospitals to the whole system, people have to change
  • 8. Integrated whole person care can only be delivered by a whole system • Teams working together to derive solutions have developed multi-agency groups at practice level • Community teams do not work in isolation – they are only as good as the partnerships they work in

Key messages • Hospitals have a critical role to play in a high value healthcare system • We must share a common purpose to optimise value • A key challenge is the funding – within health and across borders into the local authority and voluntary sector • We have to allow technology to deliver information that flows around the patient • Integrated care needs to mean urgent care • If we wait – the first victims will be safety and quality • The real limit is our imagination • Not delivering will be a failure of leadership
