Driving Costs Out Across the Entire Episode of Care-Stuart Morris-Hipkins, Syncera
1. Interview with: Stuart Morris-
Hipkins, SVP & General Manager,
Syncera
“Changes in reimbursement and
alternative payment methods are here
to stay, and likely to expand into other
disease states and specialties. Health-
care is shifting from fee-for-service
towards management and support of
the patient across the entire episode of
care. Hospitals have a lot at stake in
this environment, so they must consider
every single opportunity to reduce
waste, improve efficiency and workflows
in the pre-operative, inter-operative and
post-operative areas, to drive better
patient outcomes,” advises Stuart
Morris-Hipkins, SVP & General Manager,
Syncera. “Hospitals can save millions
just by reducing sterilization costs.
Studies have shown that, depending on
the procedure, 60-85 percent of
instruments are actually never used in
surgery and do not need to be
sterilized,” he points out.
Syncera is a solution provider company
at the marcus evans ACO & Payer
Leadership Summit Fall 2016, in Los
Angeles, California, September 12-13.
What do physicians, hospitals, ACOs
and payers need in this new
environment? How can they be
proactive?
They need to understand the changes
that are occurring in the market and
their true impact. What is the true cost?
How do we actually manage the patient
across the episode? How do we capture
the data? They need a practical plan,
perhaps an episode care manager to
lead the hospital through the changes.
There are a number of elements to
consider, so they must get organized
now. They need to own the approach
themselves in order to really improve
the patient experience and outcome.
Gainsharing and alignment are
motivational tools they can use to focus
on their performance.
Within an episode of care, where
are the key cost savings opportuni-
ties?
You have to look at the episode in a
holistic manner, across pre-op, inter-op
and post-op. At every point in the
journey there is an opportunity to
improve efficiency and provide value
and reduce costs. In the pre-operative
phase, educating patients before they
arrive for surgery, and reviewing
protocols based on patient co-
morbidities to optimize outcomes are
going to be paramount. Inter-op, there
are huge opportunities for savings
around instrument streamlining and
sterilization, around device selection,
and implant waste reduction. Then for
the post acute care phase, making sure
the patient has the appropriate amount
of service and working with the patient
to ensure his/her expectations are clear
prior to surgery is important. Hospitals
have to truly measure outcomes and
costs, and look at every step, to be in a
good position in the future.
What role can the Syncera platform
play here?
There are two major elements to our
offering. First, our technology platform
S2 Procedure Performance reduces the
training time of technicians and nurses
by 40 percent. With S2 we are able to
digitize surgeon preference cards and
analyze which instruments are used and
not used for each procedure. When
those instruments are removed from the
trays, a hospital can realize significant
savings in sterilization costs each time
the procedure is performed. Trays after
trays of instruments go off to surgery,
when only a small fraction of them will
be used. Average savings are USD 150-
400 per patient case, just by looking at
instrumentation, and that does not
include the reduction in training time.
The S2 technology can be used across
procedures for any specialty.
Secondly, we look across the spectrum
to improve the supply chain, help
hospitals understand where their costs
are, where they have waste and provide
real tangible dollar savings. If hospitals
performing 700 hip and knee replace-
ments annually use hip and knee
implants through the Syncera model,
they can drive savings of around USD 4
million over a three-year period.
