Revue "Pharmacien demain" n°27 Octobre 2016 L'immunothérapie est une nouvelle stratégie de lutte contre le cancer. La promesse d'améliorer considérablement la prise en charge de nombreux cancers grâce à l’immunité du patient est ancienne, notamment avec des premières expériences durant les années 1960 portant sur l’injection de particules bactériennes dans des tumeurs. Mais il a fallu attendre les années 2000 pour qu’elle démontre son efficacité clinique avec plusieurs produits aujourd’hui sur le marché. Cette stratégie est fondée sur la meilleure connaissance des interactions entre le cancer et le système immunitaire de l'hôte. Ces relations suivent une chronologie récemment mise en évidence : au début du processus de cancérogenèse, la tumeur naissante va afficher de nombreux antigènes à sa surface, associés pour certains aux mutations de la tumorigénèse. Ces antigènes vont être reconnus par le système immunitaire inné (cellules NK, macrophages, cellules dendritiques) et adaptatif (lymphocytes). L'immunité va alors contrôler la tumeur en éliminant une grande partie des cellules cancéreuses par des actions cytotoxiques et la sécrétion d’anticorps. Mais en même temps, ce processus va provoquer une sélection des cellules affichant moins d'antigènes, avec des mutations au niveau du Complexe Majeur d'Histocompatibilité de type 1 qui porte les antigènes de surface ou de la machinerie intracellulaire préparant les antigènes. Revue "Pharmacien demain" n°27 Octobre 2016 Le système immunitaire est capable de contrôler une tumeur tant que celle-ci est composée de moins d’un million de cellules tumorales - la taille d’une tête d’épingle. En revanche, lorsque la tumeur est plus développée, le système immunitaire se trouve dépassé et faillit à son rôle protecteur. Dans la lutte contre le cancer, les immunothérapies sont actuellement au cœur de l'actualité. Vaccins Thérapeutiques Il existe deux grands types d’immunothérapies anti-cancéreuses : les vaccins thérapeutiques et l’utilisation d’anticorps. En premier lieu, donc, les vaccins thérapeutiques. Contrairement aux vaccins prophylactiques dont la nature est essentiellement préventive, ils sont administrés comme traitement aux personnes déjà atteintes par une maladie. Ainsi le vaccin contre la rage de Pasteur qui a permis de guérir Joseph Meister était un vaccin thérapeutique. En oncologie, le premier vaccin de ce type a été développé en 1893. Il consistait en l'injection au patient cancéreux d'une "bouillie de bactéries". Dans le cas d'une réponse par le corps à cette injection de bactéries, on supposait la potentialité d'une réponse similaire contre le cancer. Malgré quelques résultats, la vaccination thérapeutique moderne n'a vraiment démarré qu'autour des années 1991, et ne s'est pas avérée concluante. Aux États-Unis, un vaccin très onéreux pour traiter le cancer de la prostate avait été commercialisé mais peu de patients avaient été traités et la firme a rapidement fait faillite.
