POSE: a new endoscopic incisionless
weight loss operation for patients 3-5 stones
                overweight

What is the POSE PROCEDURE?
Dr. Elliot Goodman now offers a new and exciting weight loss procedure which is
performed through an endoscope WITHOUT any abdominal incisions. New
surgical tools now allow surgeons to reduce the capacity of the stomach through
the patient’s mouth without making abdominal incisions into the body. This
operation is known as the “POSE” procedure, which stands for Primary Obesity
Surgery, Endolumenal.

How do I find out if I’m eligible for this new incisionless, POSE procedure?
After an initial screening, you will undergo a series of evaluations including
nutritional and dietary counseling. In addition, a full medical examination is
necessary to determine if you are a candidate. Typically patients who are 3-5
stones above ideal body weight (BMI 27-35) who have tried unsuccessfully to
lose weight with several less invasive methods are potential candidates for
POSE.
What does the POSE procedure entail?
The POSE procedure is performed using a small flexible endoscope and the
Incisionless Operating Platform (IOP). The scope and the IOP instruments are
inserted through the mouth into the stomach the same way as a standard
endoscope. Tissue anchors are used to create multiple tissue folds in the
stomach to reduce its capacity. These folds in your stomach reduce the amount
of food it can hold so you feel fuller faster during meals and eat less. POSE may
also reduce the production of certain stomach hormones thought to stimulate
appetite, thereby further reducing calorie intake. To realize the full benefits of the
procedure, you must commit to following the diet, exercise and nutritional
counseling programs that your doctor prescribes.

What instruments are used?
The POSE procedure is performed with an Incisionless Operating Platform (IOP)
and a small flexible endoscope for visualization. The IOP consists of a long
narrow access platform and specialized surgical tools (that grab tissue, hold it in
place and insert anchors and sutures), all of which have received CE mark.

What are the benefits of an incisionless procedure?
The entire procedure is performed through the mouth and inside the stomach –
not through cuts into the abdomen. By eliminating skin incisions, this new
procedure may provide important advantages to patients, including a reduced
risk of infection and associated complications, minimized pain, shorter hospital
stays, no visible external scars, plus faster recovery times. Additionally, no one
needs to know that you have had a procedure as you can return to normal
activities within a few days.

What type of side effects can I expect?
Some patients have complained of a temporary sore throat and nausea following
the procedure. These symptoms usually disappear within a few days after the
procedure. You must discuss with your doctor the full range of side-effects that
may be encountered in this procedure.

Is it safe?
All procedures have some risk involved, although to date no major complications
have been reported. The procedure is new and long-term data is not yet
available. These incisionless procedures are typically reversible by you surgeon
should you encounter a problem.

What is the success rate?
It is too soon to estimate the likely procedure success rate. However, to date, the
procedure has been well tolerated and patients typically begin to see weight loss
in the first 4 weeks and an earlier feeling of fullness right away. Your success will
depend to a large degree on your commitment to follow the diet, exercise and
nutritional counseling programs your doctor prescribes.
Early experience with these incisionless weight loss procedures show that on
average, patients lose around one third of their excess weight after 6 months.
Continuation of weight loss or maintenance of the new reduced weight is
dependent on the determination of the “New-You” to eat less and lead a more
physically active lifestyle.

What is the recovery process?
Most patients will return to normal activity within a few days of their procedure.
Your doctor will give you specific instructions. In addition, you will be required to
follow a post-procedure diet and exercise plan, similar to the regimen prescribed
following weight loss surgeries. To help you on your journey, follow-up
appointments with your doctor will be required.

What will the procedure cost?
The cost of the procedure will be around £7000, depending on which after-care
package you choose. All hospital charges and consultant fees for the POSE
procedure are included in the basic price.
