plan of care - level access for field exposure

1. PLAN OF CARE 11/13/2015
6:45 – Arrival time
6:50 – Attend on student pre-conference.
7:00 – Joined on staff endorsement.
7:05 – Buddy up with a health care aide assigned to client. Informing the care aide what I will do specifically
for the client, that I will be doing some assessments.
8:00 – If patient is awake, reintroduced self to the client. Establish rapport immediately with simple talks.
8:30 - Do a quick priority assessment on the client if given the chance. Do vital signs for pulse, respiratory
rate, and blood pressure. Do the other assessments with client permission, such as: peripheral vascular
assessment, abdominal assessment, and respiratory assessment.
8:35 – Do transfer assessment. Assist care aide with transfer from bed to wheelchair. Assist with the
personal care to resident, such as assisting with toilet needs, bathing if schedule, oral hygiene and care of
skin and nails.
8:40 – Assist the health care aide in bringing the client to the toilet.
9:00 – Assist client in grooming/dressing.
9:10 – Do the medication, if given the chance. Check patient chart for new updates.
9:30–10:30 – Client accompaniment arrive.
10:30–11:30 – Client on mass service.
11:30-12:00 - Assist client in transporting her to the dining room.
12:40 – Assist client in returning her back to her room.
12:45 – Say goodbye to the client.
13:00 – Attend on student post-conference.
