Dear Sec 4s,
You are approaching the end of your secondary school journey.
It is time to capture even more good memories in West Spring.
Growing up in this digital age, your means of communication with each
other after your graduation will mostly be over the Internet. Many years
back, our older generations did not have social media like Facebook,
Twitter, and Instagram like the way you communicate now.

Using forms
SHARING OPINIONS
ON THE INTERNET
We know there are other ways of sharing your opinions on the Internet.
1) Creating your own blog
2) Writing an email

3) Posting your comments (youtube, facebook, blogs, forums)
However, we are focusing on sharing opinions, preferences and factual data
through filling forms.
FORMS
1) Offline form: Hardcopy, MS Excel
2) Online form: Using websites that offer form-making
PAGE 172: A GOOD FORM
1. A good form includes components such as labels, data
fields, instructions, messages and validation.
2. A label that includes details such as name, age and gender
informs what the corresponding data field means, while
data field refers to the part for user to enter the data.
3. Sometimes, it is also necessary to provide instructions so that
users know what and how to complete the form.
4. Messages include system error message and/or user-defined
error message as defined in Problem Set 10. The messages
tell us whether the data input is valid. This brings us to a
process called ‗data validation‘.
A GOOD FORM
DATA CAN BE CAPTURED
IN THE FOLLOWING 4 WAYS:
(1) Text field - for data to be typed
(2) List – select data from a list of choices
(3) Check box – select one or more choices
(4) Radio button – select one choice from all the
choices
PAGE 173: ONLINE FORM
1. An online form is useful as it can be created quickly and can be shared
online with anyone with internet access.
2. Usually, such online platform allows the creation of forms and it comes with
templates that people can use.
3. Alternatively, you can also customise and create your own form.
4. Once the form is created, you can share with its intended audience and
tabulate the data by downloading their responses.
5. All of these are done online, without the need to install any programs into
your computer.

LET’S CREATE A FORM!
Step 1: Create an account with Gmail.
Step 2: Go to http://drive.google.com
Step 3: Click on Create FORM.
You can add any questions you'd like.

After choosing Form
1) Title your form ―Sample Quiz‖ or whatever you would like
2) You can choose your theme
FORM SETTINGS
• You can also organize your form by adding headers and dividing your
form into several pages.
• If you‘d like those who complete your form to see a progress bar
indicating how much of your form they‘ve completed, check the box
labeled ―Show progress bar at the bottom of form pages.‖
FORM (PAGE 1 OF 1)
• Default question 1 is multiple choice.
• Click on the arrow next to the words ―multiple choice‖ and change it to a
text box or other question types.
• Required Question?

• Click DONE! to confirm your question.
ADDING MORE QUESTIONS
• Click on add item.

• The question title is the column headers in your spreadsheet (responses).
• The help text does NOT get exported to the spreadsheet and is meant to let
you give examples or clarifying information to help the user fill out the form.

TYPE

DESCRIPTION

Text

Allows students to enter text. It is a small box but they can enter a lot of
text into the small box. Recommendation to use it for short or single
word answers.

Paragraph Text

Exactly the same as the text box except the box is larger. Use this for
longer free response answers.

Multiple Choice

Choose one answer only. This is the only question type that allows for
branching questions. If they choose A go to page 2 if they choose B go
to page 3.

Checkboxes

Allow for more than one answer to be chosen. ―Which of the following
are polygons‖ or ―What do you want on your sandwich‖? Likely there are
multiple check boxes to selected.

Choose from a List

A drop down list.

TYPE

DESCRIPTION

Scale

Rate something. ―On a scale of 1 to 10 how much time did you study for
this quiz?‖

Grid

Allows to create a rubric. It is made up of many scale and can be
customised.

Section Header

This is not a question. If you want to give some directions or context
before the question use this option. Notice, you can NOT embed
pictures into a Google Form.

Page Break

Create different pages of the quiz so you do not have to have 50
questions all on the same screen.

Add an image
• To add an image to your form, click the Insert menu, and select Image. Once
you've uploaded the image, you can give it a title and specify what text will appear
when someone hovers over the image.
• Images in forms aren't attached to or associated with form questions. You can
change the position of an image by dragging it up or down in your form.
Add a video
• To add a video to your form, click the Insert menu, and select Video. To pick a
video, either provide a YouTube video‘s URL, or use the search box to search for a
video. After selecting one, you can add a title and a caption to the video. You can
click and drag one of the video's corners to resize it, and you can align the video to
the left, right, or center of the page by choosing one of the alignment options.
EDITING QUESTIONS
• There are three editing buttons on the right-hand-side.
The Pencil for editing
The Squares to duplicate questions
The Rubbish bin to remove a question
CONFIRMATION PAGE
By default, the message to display to the user at the end of the form is

“Your response has been recorded.”

There are other settings you can adjust as well:
Show link to submit another response
— Checking this box will allow users to submit as many form responses as they'd like.
Publish and show a link to the results of this form
— Checking this box will give respondents access to the form's summary of responses.
Allow responders to edit responses after submitting
— Checking this box will allow respondents to change their answers to your form.

Create the form on Page 179 of your Textbook.
CHOOSE RESPONSE DESTINATION
• You can choose either to send responses to a spreadsheet, or you can store
them only in Forms.
SHARE USE OF FORM BY ADDING
COLLABORATORS
• Click File and select Add collaborators....
• You can specify individual collaborators with
whom you'd like to share your form for editing.
• To add a collaborator, begin typing his or her
name in the "Invite people" text box.
SEND YOUR FORM TO RESPONDENTS
You can send it to respondents via email or by sharing the form via social media.
Steps:
1) Just SEND form.
2) You can choose to share a link to your form,
or
3) you can add names, email addresses,
and groups
4) Click Done.
5) The people you‘ve added will receive an email
directing them to your form.
VIEW RESPONSES
• Once you‘ve created your form and sent it to recipients, you‘re able to view
the responses you‘ve received in three different ways:
1) a summary of responses
2) in a separate spreadsheet OR

3) as a downloaded CSV.
ACCEPTING RESPONSES
• To stop your form from collecting new responses, click the Accepting
Responses button in the toolbar to toggle responses off.
• The toolbar button will then say "Not accepting responses."
• If you‘d like to re-open your form to responses again later, you can click this
button again to toggle responses back on.
15 MINUTES ACTIVITY
Settings:

Add collaborator: cpacloudserver@gmail.com
Send form to at least 3 friends to fill up the form.
Once there are 3 – 10 responses, stop your form.
Print a copy of your downloaded spreadsheet of responses.

Write your name, register no. and class and hand in.

  • 2. GRADUATION YEAR 2014 Dear Sec 4s, You are approaching the end of your secondary school journey. It is time to capture even more good memories in West Spring. Growing up in this digital age, your means of communication with each other after your graduation will mostly be over the Internet. Many years back, our older generations did not have social media like Facebook, Twitter, and Instagram like the way you communicate now.
  • 3. GRADUATION YEAR 2014 Continue… Do you know that the information on the social media is stored online? You use the information/data directly or for further analysis or updating. Sometimes, we make new posts just to share resources so that another friend can use that resource.
  • 4. 1 MINUTE ACTIVITY Think about what you and your friends may share with each other over the Internet…
  • 5. TODAY WE WOULD LIKE TO LEARN ABOUT FORMS Sharing opinions Using forms
  • 6. SHARING OPINIONS ON THE INTERNET We know there are other ways of sharing your opinions on the Internet. 1) Creating your own blog 2) Writing an email 3) Posting your comments (youtube, facebook, blogs, forums) However, we are focusing on sharing opinions, preferences and factual data through filling forms.
  • 7. FORMS 1) Offline form: Hardcopy, MS Excel 2) Online form: Using websites that offer form-making
  • 8. PAGE 172: A GOOD FORM 1. A good form includes components such as labels, data fields, instructions, messages and validation. 2. A label that includes details such as name, age and gender informs what the corresponding data field means, while data field refers to the part for user to enter the data. 3. Sometimes, it is also necessary to provide instructions so that users know what and how to complete the form. 4. Messages include system error message and/or user-defined error message as defined in Problem Set 10. The messages tell us whether the data input is valid. This brings us to a process called ‗data validation‘.
  • 10. DATA CAN BE CAPTURED IN THE FOLLOWING 4 WAYS: (1) Text field - for data to be typed (2) List – select data from a list of choices (3) Check box – select one or more choices (4) Radio button – select one choice from all the choices
  • 11. PAGE 173: ONLINE FORM 1. An online form is useful as it can be created quickly and can be shared online with anyone with internet access. 2. Usually, such online platform allows the creation of forms and it comes with templates that people can use. 3. Alternatively, you can also customise and create your own form. 4. Once the form is created, you can share with its intended audience and tabulate the data by downloading their responses. 5. All of these are done online, without the need to install any programs into your computer.

  • 14. LET’S CREATE A FORM! Step 1: Create an account with Gmail. Step 2: Go to http://drive.google.com Step 3: Click on Create FORM. You can add any questions you'd like.

  • 16. After choosing Form 1) Title your form ―Sample Quiz‖ or whatever you would like 2) You can choose your theme
  • 17. FORM SETTINGS • You can also organize your form by adding headers and dividing your form into several pages. • If you‘d like those who complete your form to see a progress bar indicating how much of your form they‘ve completed, check the box labeled ―Show progress bar at the bottom of form pages.‖
  • 18. FORM (PAGE 1 OF 1) • Default question 1 is multiple choice. • Click on the arrow next to the words ―multiple choice‖ and change it to a text box or other question types. • Required Question? • Click DONE! to confirm your question.
  • 19. ADDING MORE QUESTIONS • Click on add item. • The question title is the column headers in your spreadsheet (responses). • The help text does NOT get exported to the spreadsheet and is meant to let you give examples or clarifying information to help the user fill out the form.
  • 20. QUESTION TYPES 1/4 TYPE DESCRIPTION Text Allows students to enter text. It is a small box but they can enter a lot of text into the small box. Recommendation to use it for short or single word answers. Paragraph Text Exactly the same as the text box except the box is larger. Use this for longer free response answers. Multiple Choice Choose one answer only. This is the only question type that allows for branching questions. If they choose A go to page 2 if they choose B go to page 3. Checkboxes Allow for more than one answer to be chosen. ―Which of the following are polygons‖ or ―What do you want on your sandwich‖? Likely there are multiple check boxes to selected. Choose from a List A drop down list.
  • 21. QUESTION TYPES 2/4 TYPE DESCRIPTION Scale Rate something. ―On a scale of 1 to 10 how much time did you study for this quiz?‖ Grid Allows to create a rubric. It is made up of many scale and can be customised. Section Header This is not a question. If you want to give some directions or context before the question use this option. Notice, you can NOT embed pictures into a Google Form. Page Break Create different pages of the quiz so you do not have to have 50 questions all on the same screen.
  • 23. IMAGE AND VIDEO ARE NOT QUESTION TYPES 4/4 Add an image • To add an image to your form, click the Insert menu, and select Image. Once you've uploaded the image, you can give it a title and specify what text will appear when someone hovers over the image. • Images in forms aren't attached to or associated with form questions. You can change the position of an image by dragging it up or down in your form. Add a video • To add a video to your form, click the Insert menu, and select Video. To pick a video, either provide a YouTube video‘s URL, or use the search box to search for a video. After selecting one, you can add a title and a caption to the video. You can click and drag one of the video's corners to resize it, and you can align the video to the left, right, or center of the page by choosing one of the alignment options.
  • 24. EDITING QUESTIONS • There are three editing buttons on the right-hand-side. The Pencil for editing The Squares to duplicate questions The Rubbish bin to remove a question
  • 25. CONFIRMATION PAGE By default, the message to display to the user at the end of the form is “Your response has been recorded.” There are other settings you can adjust as well: Show link to submit another response — Checking this box will allow users to submit as many form responses as they'd like. Publish and show a link to the results of this form — Checking this box will give respondents access to the form's summary of responses. Allow responders to edit responses after submitting — Checking this box will allow respondents to change their answers to your form.

  • 27. IMAGINE YOU ARE ORGANISING AN EXCURSION WITH YOUR CLASSMATES Try using Google Form to create a similar form! 
  • 28. 15 MINUTES ACTIVITY Create the form on Page 179 of your Textbook.
  • 29. CHOOSE RESPONSE DESTINATION • You can choose either to send responses to a spreadsheet, or you can store them only in Forms.
  • 30. SHARE USE OF FORM BY ADDING COLLABORATORS • Click File and select Add collaborators.... • You can specify individual collaborators with whom you'd like to share your form for editing. • To add a collaborator, begin typing his or her name in the "Invite people" text box.
  • 31. SEND YOUR FORM TO RESPONDENTS You can send it to respondents via email or by sharing the form via social media. Steps: 1) Just SEND form. 2) You can choose to share a link to your form, or 3) you can add names, email addresses, and groups 4) Click Done. 5) The people you‘ve added will receive an email directing them to your form.
  • 32. VIEW RESPONSES • Once you‘ve created your form and sent it to recipients, you‘re able to view the responses you‘ve received in three different ways: 1) a summary of responses 2) in a separate spreadsheet OR 3) as a downloaded CSV.
  • 33. ACCEPTING RESPONSES • To stop your form from collecting new responses, click the Accepting Responses button in the toolbar to toggle responses off. • The toolbar button will then say "Not accepting responses." • If you‘d like to re-open your form to responses again later, you can click this button again to toggle responses back on.
  • 34. 15 MINUTES ACTIVITY Settings: Add collaborator: cpacloudserver@gmail.com Send form to at least 3 friends to fill up the form. Once there are 3 – 10 responses, stop your form. Print a copy of your downloaded spreadsheet of responses. Write your name, register no. and class and hand in.
