3. Why rehabilitation technology
for the arm in Multiple Sclerosis?
Multiple Sclerosis (MS)
• a chronic, progressive disease of the CNS
• a major cause of disability among (young) adults
• prevalence MS in Belgium: approximately 1/1000
• In the ARM
Symptoms of muscle weakness, spasticity, incoordination, …
• In MS,
about ¾ are confronted with reduced manual dexterity
while ½ experience substantial impact on activities of daily life.
[Johansson et al., 2007, Journal of Neurology]
4. Prevalence of arm dysfunction in MS
MILD MODERATE SEVERE
67% 93%
KATZ 15% 59%
ADL Index,
Instrumental
98%
96%
219 MS patients
[Johansson et al., 2007, Journal of Neurology]
5. Why rehabilitation technology
for the arm in Multiple Sclerosis?
Multiple Sclerosis (MS)
• Bilateral symptoms
dysfunction of the arm leads to reduced independence in performing
activities of daily life (ADL), and high economic cost due to
dependency [Kobelt et al., 2006, Journal]
• Multiple symptoms may occur
Motor, Sensory, Visual, Cognitive, ….
Motor systems in leg, trunk, arm, eyes
Lack of therapy time Therapy intensity & specificity
dedicated to the arm
due to multiplicity of symptoms
6. MS, as chronic progressive disease:
TRAINING POTENTIAL?
Systematic REVIEWS & META-ANALYSES regarding EXERCISE THERAPY

Scler; Motl et al (2008) Mult Scler; Snook et al(2009) Clin Rehab, Snook et al (2009) NNR..
Impairment Activity Participation
Muscle strength, Walking Self-efficacy
Endurance, Functional mobility Quality of Life
Energy/less fatigue
7. Why rehabilitation technology
for the arm in CVA?
• STROKE, a major cause of disability
Fe. The Netherlands:
Incidence: 16,2/10000
Prevalence: 25.000
Increased incidence is expected due to ageing population, putting a
stress on health care services
• Arm recovery is generally poor:
about 65% is unable to incoorporate arm in daily function
arm motor function explains 50% of variance in functional autonomy
• Bimodal distribution of arm paresis
Mild Motricity Index >19/item (MRC 3), finger movements
Severe

1. Gebruik van robots & virtuele leeromgeving bij armrevalidatie
2. Meten van de kwaliteit van armbewegingen (ambulant)
Revalidatierobotica & Bewegingsregistratie
12. Projectdoelstellingen Interreg IV
bij personen met beperkte armfunctie
Kennisverdieping MS
Revalidatierobotica & Bewegingsregistratie
Kennisverbreding CVA
13. Bewegingsregistratie
• Draagbaar systeem,
bruikbaar in ZH en thuis
• Meten van
– Bewegingskwaliteit
hoe wordt een beweging
uitgevoerd?
– Bewegingskwantiteit van
specifieke bewegingen
voor/na interventies
14. Projectdoelstellingen Interreg IV
bij personen met beperkte armfunctie
Kennisverdieping MS
Revalidatierobotica & Bewegingsregistratie
Kennisverbreding CVA
15. HARDWARE
Haptic Master
• Ruime workspace
• 3D
• Sterke krachtenterugkoppeling
– verplaatsen van arm
– optimaal pad
– voorwerpen met gewicht
– reële contouren van omgeving
Proprioceptieve feedback
Software na beperkte aanpassing
ook geldig voor Phantom/Falcon
16. HARDWARE
• Haptic Master
• Gimbal (MOOG)
Aangepaste connectie
• Sling (FOCAL)
Extra anti-zwaartekracht
ondersteuning
voor patiënten met
ernstige parese van de arm
18. Virtuele Leeromgeving: Wat oefenen?
Identify training preferences
by using the Motor Activity Log (MAL)
Stroke:
Timmermans et al (2009) Disabil & Rehab
MS: Adapted version (bilateral impairment)
Interreg : 17 MS patients with arm paresis
Lamers, Timmermans et al (in preparation)
29. Verwachte resultaten
Toename van...
• therapiemogelijkheden voor personen met een ernstige parese
• therapietijd voor de patiënt, die semi-autonoom kan oefenen
• een beter inzicht in de (verandering van de)
bewegingsmogelijkheden van de patiënt
30. Innovatie in revalidatierobotica?
• Toekomstgerichte benadering!
