Le decisioni politiche, l’organizzazione e le istituzioni dei tempi riflettono e determinano il
percorso della Sanità italiana, segnando e scandendo le tappe.
1865 - “sistema di sanità amministrativa”

• Il SSN è costituito dal complesso delle funzioni, delle strutture, dei servizi e delle

e psichica di tutta la popolazione senza distinzioni di condizioni individuali e sociali
e secondo modalità che assicurino l’eguaglianza dei cittadini nei confronti del
servizi.
• L’attuazione del SSN compete allo Stato, alle Regioni ed agli Enti territoriali,
garantendo la partecipazione dei cittadini.

Per un approcio olistico, integrato alla prevenzione delle malattie croniche e alla promozione della salute un sistema di monitoraggio dei comportamenti legati alla salute è indispensabile
Strengthening Public Health services and capacities is one of the four pillars for Health 2020 and a key aspect to ensure its delivery and implementation. Throughout the consultation process, a lot of attention was paid to the coherence between the Health2020 and the European Action Plan for Strengthening Public Health. In order to achieve this, we all need to take action to strengthen public health, functions, infrastructures and capacities , but also increase the capacities and performance of health systems given an increased focus to primary prevention and health promotion. Strengthening Primary Health Care in line with the Alma Ata Charter for this is very important.
At the Regional Committee 61 in Baku, a Framework for Action was approved with corresponding Resolution – which was the basis for the development of the Action Plan which is written around the 10 Essential Public Health Operations. These 10 Essential Public Health Operations were carefully reviewed during the consultation process, including at an WHO expert meeting with experts of a number of member states, including: Prof Leventhal from Israel; Drs Paulssen and Mali Strand from Norway; and Dr Korotkova Anna from Russia and also partners like ASPHER and EUPHA and the Observatory. I would like to draw particular attention to the EPHO number 4, which following feedback from consultations, was strengthened to address the social determinants and health inequity, therefore, bringing it fully in line with the H2020 – this also provides a stronger focus on addressing the wider determinants for noncommunicabe diseases, that account for over 80% of deaths within the region.
The ten EPHO form the basis of the European Action Plan. Although there are ten main operations identified for strengthening public health services, in the health systems division, we recognise that it is often more effective and efficient to delivery services in an integrated way, rather than in vertical programmes. Therefore, we have clustered the 10 EPHOs: EPHOS 1- 5: The first five EPHOs can be described as Core EPHOs – that is operations that need public health skills and expertise to deliver them, the first 2 operations can be clustered as ‘ Public Health Intelligence ’ - based upon monitoring and surviellance, and with research findings from EPHO 10, we can use this information to make assessments, in order to make Intelligent decisions in policies and plans Whilst, EPHOs 3, 4 and 5, can be described as core public health services in the areas of health protection, disease prevention and health promotion – these are illustrated as overlapping circles, to emphasise the importance of developing an integrated approach between these main services EPHOs 6- 10: Can be described as Enablers. In particular, health workforce development needs to be strengthened in order to ensure the delivery of public health services across the Region. Human Resources for Health remains a top priority for the Regional Office and my Division in particular.
A key step for the future is to strengthen financing for implementing the European Action Plan. The estimated additional cost for WHO to Govern and co-ordinate the implementation of the EAP each year is $1,200,000 , and includes setting up and running a steering group, working groups for the EPHOS and an expert advisory committee, publications and reports for the regional committee. The overall estimated additional cost for WHO Europe to implement the European Action Plan is 3,400,000 USD per year, the largest part being for salary costs. This includes the annual cost of delivering all the EPHOs and would cover the co-ordination and governance costs by the WHO. It is important to emphasize that the governance of HSS including public health will go hand in hand with the governance of H2020.
we have suffered about 20 structural reorganisations in the last 40 years and they have made very, very little difference we need systems for managing heart failure or depression. We need systems for managing risk in pregnancy or falls in the elderly. Hospitals, Health Centres and the Professions are structures dating back to the Middle Ages. We may need them still but above all we must ensure that the structures of the future follow the functional need to deliver services in the 21 st century. share all the knowledge with them and make their responsibilities clear as well as your own and that of the health service. Systems, structures and professions will change but the needs of patient will remain the one constant in the 21 st century.
