1. ECG C*@#os
Dr. Bastuardo
Emergenciólogo
Universidad de Madera
2. Caso 1
• 3 ECG de pacientes distintos
• Que diagnóstico?
• Tratamiento?
A) Manejo médico
B) PTCA
3. New ECG criteria for emergent reperfusion
Fig. 1 Acute MI in the presence of left main coronary artery occlusion. Note the STE greater than 2 mm (0.2 mV) in lead aVR and multilead STD 1 mm or greater (0.1 mV) in leads I, II, III,
aVF, and V3 through V6.
4. Fig. 2 Acute MI in the presence of occluded proximal LAD. Note the STE greater than 1 mm (0.1 mV) in lead aVR and multilead STD greater than 1 mm (0.1 mV) in leads V3 through V5.
5. Fig. 3 Acute MI and cardiac arrest in presence of multivessel coronary artery disease. Note the STE greater than 1 mm (0.1 mV) in lead aVR, STD greater than 1 mm (0.1 mV) in leads V2
and V6, and STD greater than 3 mm (0.3 mV) in leads V3 through V5.
6. aVR
• Si esta asociado a IDST pared lateral indica
obstrucción proximal de LMCA
• Mortalidad
• Terapia médica: 70%
• PTCA: 40%
• Conversar con hemodinamista para manejo
agresivo como IAM c/SDST
7. Caso 2
siempre
13. Caso 3
• 45 años, consulta por dolor torácico 30
min duración
• ECG…...
18. Perdida balance T precordial
Nueva T alta en V1
• T en V1 siempre es negativa o plana
• Ojo si NUEVA o > V6
• Manifestación más precoz
• Enfermedad coronaria o isquemia aguda
