This document discusses how geolocated health data can be used to help communities. It provides examples from the 1854 Broad Street cholera outbreak where John Snow was able to identify the water pump handle as the source by geolocating cases, and a current project in California called the HOME Project that aims to help elderly patients navigate healthcare options using their location data. The document advocates for greater use of coordinated, location-based health information and services to improve care, especially for vulnerable populations.
