ORGANISATIONAL CHANGE IN HSE&MC DEPARTMENT
Following recent redundancies and a review of the HSE&MC structure and overall KEM
organization, there will be a number of changes to the organizational and reporting
structure within the HSE&MC department. These changes will be effective from Monday 5
July, 2004.
Occupational Health & Safety Systems
The role of Superintendent Safety Systems has been made redundant with the departure of
Adrian Larkins. Responsibilities for this role shall now be shared by Dame Sianipar
(Occupational Health Officer) and Petrus Satian (Safety Officer) who shall both report direct
to myself. Management of the clinic together with internal and external reporting functions
shall be the responsibility of Dame with assistance from Petrus. Simon Connolly will also be
available to provide advice and assistance as required and as noted in a separate
announcement from the Process Plant. PROPOSAL FOR CHANGE :
• The management of the safety section will be incorporated into the role of the Safety
Officer as per the role responsibilities identified in appendix 1.
• Safety Officer Petrus Satian has held this position in the safety section for four years and
during that time been involved in all aspects of the management of the section. He has

demonstrated competence in the Training KesKem System, Statistical Analysis, Report
Writing, Accident Investigation and Audit and Inspection processes.
• The role will report directly to the Manager HSE - Mine Closure
Current Responsibilities
• Identify safety improvements for recommendation to management.
• Identify generic safety needs and develop appropriate safety programs.
• Conduct generic safety training to nominated department personnel and contractors.
• Conduct annual review of the site HSE induction program and the KesKem program.
• Maintain site safety training records.
• Collate and disseminate site safety statistics and information.
• Develop monthly safety themes along with supporting ZAP sheets and distribute to
departments
• Contribute to achieving KEM's HSE targets through participation in AKAR, section and
departmental safety meetings.
• Respond to requests from departments for safety information.
• Assist departments when required in incident and accident investigations.
Additional Responsibilities
• Chair the weekly OHS section meeting in rotation with the Occupational Health
Coordinator & Fire Supervisor.
• Complete the weekly OHS Managers report in rotation with Occupational Health
Coordinator.
• Complete monthly safety manager’s report.
• Complete monthly safety HSE committee report.
• As required assist PD and HSE – Mine Closure manager in six monthly audits.
• Assist the Occupational Health coordinator in the classification of personal injury
accidents.
• Assist the Occupational Health coordinator with the completion of six monthly Health,
Safety & Environmental Reports for Rio Tinto London.
• Completed the monthly Rio Tinto Safety report and send to London by the 5 th
working
day of the month.
• Assist the Occupational Health Coordinator in reporting significant incidents into the Rio
Tinto portal.
• Develop and present the monthly Safety report to the HSE committee.
• I the event of a lost time injury and in consultation with the Kapala Teknik complete
mines department significant incident reports
