They are being asked to change the way they practice. To use EHRs in ways they never have before.
To document many more things in much more detail to ensure that clinical documentation:
Reflects whether a patient was admitted with a high risk condition or infection or acquired it at the hospital
Provides specific details required for Meaningful Use quality scoring and attestation reporting
Support medical necessity with appropriate diagnoses and treatment reasons
The information they must capture in clinical documentation to support these major trends and initiatives can be overwhelming.
And physicians are on the go and looking to either use their own mobile devices (which may challenge information security systems). In essence, they demand easier access to document care.
