A private pond is a body of water located entirely on the land
of an applicant, with no surface water discharge or a discharge
that can be controlled to prevent chemical loss, and without
access by the public.

The permit is returned to whomever mailed it in, unless otherwise
noted
Applicant must carry the permit when applying treatment
The permit is on the last page of the application

Must be filled out and posted during application
16. After Treatment
30 days after treatment occurs, a treatment record
needs to be sent to central intake
If multiple treatment occurs on the same site,
multiple treatment records need to be sent in
The treatment records will be placed in SharePoint
