Benefits Changes
► Medical
o The deductibles for PPO 1 and PPO 2 are changing in 2014
o Two sets of medical rates based on Healthy Approach participation
► Pharmacy
o Express Scripts will remain as the pharmacy vendor and you will now have a
separate ID card for prescriptions
► Behavioral Health
o This will now be administered by Blue Cross Blue Shield of Massachusetts
► Wellness
o New wellness program will be offered through Interactive Health Solutions
► Employee Assistance Program
o New addition to the benefits package through ComPsych
► Long Term Care
o Will no longer be offered
► 401(K) Match
o Xerium match changing to 100% of the first 3% of eligible compensation, and
50% of the next 2% of eligible compensation

► ACA/”Obamacare” Individual Mandate requires you to have insurance or
pay a penalty

Important Information
► All medical plan participants will be receiving new ID
cards. Please use your current ID card until you receive
your new one.
► The pharmacy benefit will remain with Express Scripts
however you will have a separate ID card for Express
Scripts in 2014. Please use your BCBS ID card until you
receive your new Express Scripts ID card.
► Behavioral Health coverage will be administered by
Blue Cross Blue Shield of Massachusetts beginning
on January 1, 2014. Please continue to use Value
Options for any behavioral health needs until then.

We are excited to announce that Xerium is partnering with Interactive Health
Solutions as our wellness vendor! The Healthy Approach Wellness program
promises to be a great way for all employees to live healthier lives!
►Your health and wellbeing is important to Xerium so we want to encourage all
members to live a healthy lifestyle with several incentives for participating in the

19. Health Care Flexible Spending Accounts (HCFSA) help

Things To Note…
►Set aside up to $2,500

►Expenses paid with pre-tax dollars
►There is a 2 ½ month grace period for health care claims (i.e. you can incur
claims between 1/1/14 and 3/15/14, and submit prior to 4/30/14 to use your
2013 FSA balance)

► Xerium provides several types of life insurance to help ensure you and your
family’s financial protection. Coverage for these benefits are 100%
company paid!
o Basic Life and Basic AD&D
• Coverage is provided based on your previous year’s gross earnings (note: if you were
newly hired and haven't worked an entire calendar year, it is equal to your annualized
pay rate upon hire)
– Salaried employees- 2 times annual earnings
– Hourly employees- 1 time annual earnings
o Business Travel Accident
• Insurance protection while traveling on company business
• Coverage is 1.5 times annual earnings or $100,000 (to a max of $2,500,000)

27. Long Term Care Insurance
► Effective January 1, 2014 Xerium will no longer offer
LongTerm Care Insurance.
► Contact a member of the Human Resources Team in
order to obtain a Conintuation Application

29. 2014 Retirement Savings and Investment Plan- 401(K)
Xerium’s Retirement Savings and Investment Plan is one of your most valuable
benefits!
• Some of the benefits include:
– New Match: Xerium matches 100% of the first 3% of eligible compensation and 50% of
the next 2% of your eligible compensation.
– Your plan contributions from 1%-99% are deducted from your paycheck
– You are fully, 100%, vested in all contributions made to the Plan including the company match.
– You have the option of contributing on a pre-tax or post tax basis

coincident with the completion of 2 months of service
