defaulted to the Healthy Approach

on your participation in the Health

► Medical
o The deductibles for PPO 1 and PPO 2 are changing in 2014
o Two sets of medical rates based on Healthy Approach participation

o Express Scripts will remain as the pharmacy vendor and you will now have a
separate ID card for prescriptions
► Behavioral Health
o This will now be administered by Blue Cross Blue Shield of Massachusetts
► Wellness
o New wellness program will be offered through Interactive Health Solutions

o New addition to the benefits package through ComPsych
► Long Term Care
o Will no longer be offered
► 401(K) Match
o Xerium match changing to 100% of the first 3% of eligible compensation, and

► ACA/”Obamacare” Individual Mandate requires you to have insurance or

pay a penalty
o Your choices to meet the individual mandate are:
• Maintain coverage through Xerium or other qualified medical coverage
• Purchase coverage through the marketplace at full post tax cost (no subsidy available)

Important Information
► All medical plan participants will be receiving new ID

cards. Please use your current ID card until you receive
your new one.
► The pharmacy benefit will remain with Express Scripts
however you will have a separate ID card for Express
Scripts in 2014. Please use your BCBS ID card until you
receive your new Express Scripts ID card.
► Behavioral Health coverage will be administered by
Blue Cross Blue Shield of Massachusetts beginning
on January 1, 2014. Please continue to use Value
Options for any behavioral health needs until then.

We are excited to announce that Xerium is partnering with Interactive Health
Solutions as our wellness vendor! The Healthy Approach Wellness program
promises to be a great way for all employees to live healthier lives!
►Your health and wellbeing is important to Xerium so we want to encourage all

members to live a healthy lifestyle with several incentives for participating in the
program.
►Interactive Health will be hosting health evaluations that are convenient,
comprehensive and always confidential.
o These evaluations will test many variables including cardiovascular disease
risks, diabetes, and much more!

Xerium is pleased to announce the addition of an Employee Assistance
Program (EAP) to our benefits offering!
►EAP is a confidential counseling service offered through our new vendor,

ComPsych, that is available to all employees and their immediate family

o ComPsych’s Family Source Guidance

► ComPsych offers up to 6 counseling sessions, free of charge, per issue in

Health Care Flexible Spending Accounts (HCFSA) help
you pay for health care expenses tax free!
Things To Note…
►Set aside up to $2,500
►Deducted from payroll
►Expenses paid with pre-tax dollars
►There is a 2 ½ month grace period for health care claims (i.e. you can incur

claims between 1/1/14 and 3/15/14, and submit prior to 4/30/14 to use your
2013 FSA balance)
►You must complete the appropriate section of your 2014 Employee Benefit
Enrollment Form if you wish to participate in 2014, regardless of if you were
enrolled previously
o Reminder: You cannot change your election amount until the next open
enrollment unless you experience a qualifying event

Life and Disability
► Xerium provides several types of life insurance to help ensure you and your

family’s financial protection. Coverage for these benefits are 100%
company paid!
o Basic Life and Basic AD&D
• Coverage is provided based on your previous year’s gross earnings (note: if you were
newly hired and haven't worked an entire calendar year, it is equal to your annualized
pay rate upon hire)
– Salaried employees- 2 times annual earnings
– Hourly employees- 1 time annual earnings

o Business Travel Accident
• Insurance protection while traveling on company business
• Coverage is 1.5 times annual earnings or $100,000 (to a max of $2,500,000)

► Short Term Disability
o Provides income protection for
when you are deemed unable to
complete the essential duties of
your job
o Covers your first 26 weeks of injury
or illness as long as you remain
disabled
► Long Term Disability
o Provides income protection if you
are deemed totally disabled

• Any amount purchased above $300,000 will require a Personal Health Application

• Amounts from $25,000 to $250,000 can be purchased in addition to the Xerium
AD&D plan
• These amounts must be in increments of $25,000
Dependent Life
• Employees may purchase additional life coverage for their spouse, children or both
– Spouse: $10,000
– Child: $5,000

LongTerm Care Insurance.
► Contact a member of the Human Resources Team in
order to obtain a Conintuation Application

2014 Retirement Savings and Investment Plan- 401(K)
Xerium’s Retirement Savings and Investment Plan is one of your most valuable
benefits!
• Some of the benefits include:
– New Match: Xerium matches 100% of the first 3% of eligible compensation and 50% of
the next 2% of your eligible compensation.
– Your plan contributions from 1%-99% are deducted from your paycheck
– You are fully, 100%, vested in all contributions made to the Plan including the company match.
– You have the option of contributing on a pre-tax or post tax basis

►Who can participate?
• You are eligible to participate on the first of the month that immediately follows or is
coincident with the completion of 2 months of service

  • 2. 2014 Open Enrollment Enrollment Dates and Actions ► Enroll for benefits between November 4th and November 15th o Your current medical election has defaulted to the Healthy Approach cost/month.   • 3. 2014 Open Enrollment Benefits Changes ► Medical o The deductibles for PPO 1 and PPO 2 are changing in 2014 o Two sets of medical rates based on Healthy Approach participation ► Pharmacy o Express Scripts will remain as the pharmacy vendor and you will now have a separate ID card for prescriptions ► Behavioral Health o This will now be administered by Blue Cross Blue Shield of Massachusetts ► Wellness o New wellness program will be offered through Interactive Health Solutions ► Employee Assistance Program o New addition to the benefits package through ComPsych ► Long Term Care o Will no longer be offered ► 401(K) Match o Xerium match changing to 100% of the first 3% of eligible compensation, and 50% of the next 2% of eligible compensation 3
  • 5. 2014 Open Enrollment Health Reform Update ► ACA/”Obamacare” Individual Mandate requires you to have insurance or pay a penalty o Your choices to meet the individual mandate are: • Maintain coverage through Xerium or other qualified medical coverage • Purchase coverage through the marketplace at full post tax cost (no subsidy available) https://www.healthcare.gov/marketplace/individual/ • No coverage subject to the following IRS penalties*: Tax Year % of Income Flat Dollar Amount Per Adult 2014 Per Minor Family Maximum $95 $47.50 $285 1% *Penalty is greater of “flat dollar amount” or “1% of income” 5
