• El cáncer de ovario requiere la atención por
equipos multidisciplinares bien entrenados
• La cirugía primaria con el fin de obtener una
citorreducción óptima es el abordaje estándar
• La citorreducción óptima es el factor pronóstico
más importante en cáncer de ovario avanzado
44. CONCLUSIONES II
• El tratamiento de primera línea recomendado
consiste en Paclitaxel y Carboplatino
administrados en pauta trisemanal
• Se aceptan esquemas que sean superiores a
Paclitaxel-Carboplatino en al menos un ensayo
clínico aleatorizado:
– Intraperitoneal
– Dosis densas de Paclitaxel
– Bevacizumab durante el tratamiento seguido de
mantenimiento

• El tratamiento de la recaída debe ser
individualizado
• Las pacientes con recaída precoz son candidatas
para ensayos clínicos o en su defecto, ser
tratadas con monoterapia secuencial
• Las pacientes con recaídas tardías deben recibir
un doblete de platino. La selección depende
mayormente de la toxicidad

platino para pacientes con alergia a carboplatino
o sensibilidad intermedia

ovario son los antiangiogénicos y los inhibidores

• El uso eficiente de los nuevos fármacos dirigidos
requiere el empleo de biomarcadores predictores
de respuesta
Los cuadros suboclusivos son frecuentes en la evolución de la enfermedad. En esta circunstancia, a veces se resuelven con medidas conservadoras y endoprótesis. Ocasionalmente, cuando las anteriores medidas fallan se requiere de cirugía derivativa con el objeto de mejorar la QoL y permitir la permanencia en domicilio
El CEO es una enfermedad quimiosensible. Con esquemas de primera línea se consiguen RC en un 70-80% Sin embargo, la mayoría recaen posteriormente tras una mediana de unos 12-18m
Casi 40% no completaron tto por mielotoxicidad
La recaída es habitualmente incurable y tiene intención especialmente sintomática con objetivos en controlar los síntomas asociados a la enfermedad, mantener o mejorar la QoL, aumentar el TTP e incrementar la OS.

Tumor angiogenesis is a complex process based on the concept that a tumor requires a vascular blood supply to grow beyond 1 to 2 mm Tumor initiates recruitment of its own blood supply by switching the balance between pro-angiogenic and anti-angiogenic factors.Angiogenesis precedes and is necessary for tumor progression and metastasis. Angiogenesis precedes and is necessary for tumor progression and metastasis. The classical angiogenic switch. The angiogenic switch is a discrete step in tumour development that can occur at different stages in the tumour-progression pathway, depending on the nature of the tumour and its microenvironment. Most tumours start growing as avascular nodules (dormant) ( a ) until they reach a steady-state level of proliferating and apoptosing cells. The initiation of angiogenesis, or the ‘angiogenic switch’, has to occur to ensure exponential tumour growth. The switch begins with perivascular detachment and vessel dilation ( b ), followed by angiogenic sprouting ( c ), new vessel formation and maturation, and the recruitment of perivascular cells ( d ). Blood-vessel formation will continue as long as the tumour grows, and the blood vessels specifically feed hypoxic and necrotic areas of the tumour to provide it with essential nutrients and oxygen ( e ). Historically, tumour-associated angiogenesis was thought to go through two phases, which are separated by the ‘angiogenic switch’ (FIG. 3). The first is defined as an avascular phase, which corresponds to small and occult lesions of not more than 1–2 mm in diameter. These lesions stay dormant by reaching a steady state between proliferation and apoptosis. Dormant tumours have been discovered during autopsies of individuals who died of causes other than cancer35. This supports the notion that only a very small subset of dormant tumours enter the second phase — the vascular phase — in which exponential tumour growth ensues. This same principle applies to tumour metastasis. Dormant metastases are a clinically difficult problem, as they can sometimes become activated by removal of the primary tumour10.
PARP, poly(adenosine diphosphate ribose) polymerase. The graphic on this slide shows the mechanism and consequences of PARP-1 inhibition. In a number of different tumor cell types, PARP-1 is upregulated in an effort to repair DNA damage. As mentioned previously, one of the key mechanisms by which single-strand breaks are repaired is base excision repair. In the presence of a PARP-1 inhibitor, cells are unable to repair single-strand DNA damage via base excision repair and single-strand breaks accumulate over time. At DNA replication forks, the accumulated damage accelerates into double-strand DNA breakage subsequently leading to cell death or apoptosis pathways. In the absence of PARP inhibition, DNA damage can be repaired and, as a result, cells will survive and proliferate.
